[Congenital duodenal obstruction: mortality pattern in the last 30 years].

Data of 57 newborns and infants who underwent on operations having duodenal obstruction were analyzed in a 30 year period between 1955-1986. The patients were classified into 3 prognostic groups regarding to Waterson and the mortality was examined in decades. The mortality showed a continuous decrease during the 30 years, in the last decade this was considerable. Improving operation technique, the advanced peri- and postoperative intensive care are playing role in the decrease of the mortality. In the late follow up, most of the patients were found to be without complaints, however, regarding to some other authors this finding is not precluding the possibility of organic changes, such as delayed gastric passing, gastroesophageal reflux, etc.

Active antireflux mechanism induced by micturition. Experimental study in awake sheep.

The influence of increased bladder pressure on ureteral peristalsis is described in the literature. All experiments were done on anesthetized animals and vesical contractions were electrically induced. We present an experiment on awake sheep. The animals voided spontaneously while a recording of the peristaltic pressure of the lower ureter was recorded. A rise in the peristaltic pressure in both ureters was observed during micturition. We suggest that the rise in ureteral pressure participates in the antireflux mechanism.

The nature of the defect in familial male pseudohermaphroditism in Arabs of Gaza.

Studies in six Arab individuals from Gaza with familial male pseudohermaphroditism (MPH) due to 17-ketoreductase deficiency revealed several metabolic aberrations associated with the disorder. Plasma LH, FSH, testosterone, and androstenedione concentrations were low in the two prepubertal patients. After hCG administration plasma androstenedione increased markedly. The four postpubertal MPH patients had very high plasma gonadotropin and androstenedione concentrations, the latter increasing further after hCG administration. Plasma testosterone concentrations in all six patients were moderately low or normal for age and increased little after hCG administration. Spermatic venous testosterone concentrations, measured in three adults, were within the normal range in two and low in one, while androstenedione concentrations were markedly elevated (15- to 32-fold) in all three patients. Kinetic analyses of progesterone and androstenedione metabolism were performed in testicular tissue of these patients and compared to the results in two control subjects. While testicular tissue from the two prepubertal patients metabolized progesterone only to androstenedione, and that to a limited extent, the tissue from the four postpubertal patients metabolized progesterone to 16 alpha- and 16 beta-hydroxyprogesterone, 17 alpha-hydroxyprogesterone, androstenedione, and testosterone and metabolized androstenedione to testosterone. The Michaelis constants of these reactions were similar in the tissue from the MPH and the control subjects. The production of 16 alpha- plus 16 beta-hydroxyprogesterone was 5.4- to 10.3-fold greater, and 17-hydroxylase activity was 5.8- to 8.1-fold lower in the testes of the postpubertal MPH patients compared to values in the control subjects. The preference of androstenedione production through the delta 4- or delta 5-pathways was examined in the testes of two adult MPH patients using an equimolar concentration of [14C]progesterone and [3H]pregnenolone as substrates. While the flow of substrates in the control testes was equal or slightly greater through the delta 4-pathway, the delta 5-pathway predominated in the testes of the MPH patients. A large amount of dehydroepiandrosterone accumulated when NAD, the cofactor for 3 beta-hydroxysteroid dehydrogenase-isomerase, was omitted, supporting the contention that androstenedione was produced in the testes of the MPH patients mainly through the delta 5-pathway. Additional support for this suggestion was the finding that the 3H/14C ratio in androstenedione and testosterone produced from both substrates was 8 times higher in the testes from MPH patients than in those from the control subjects.(ABSTRACT TRUNCATED AT 250 WORDS)

Male pseudohermaphroditism due to 17 beta-hydroxysteroid dehydrogenase deficiency: gender reassignment in early infancy.

Male pseudohermaphroditism due to 17 beta-hydroxysteroid dehydrogenase (17 beta-HSD) deficiency has a high prevalence within the Arab population of the Gaza strip and is characterised by marked virilization at puberty, leading in many cases to the spontaneous adoption of a male gender role. As a result of this, parents of 7 affected male infants (aged 1-10 months) born with female phenotype requested early gender reassignment. Diagnosis was suspected in 5 on the basis of a positive family history, but confirmed in all cases by the finding of low to normal testosterone levels (30-184 ng/dl) with high delta 4-androstenedione levels (188-808 ng/dl), after hCG. Treatment with im testosterone oenanthate (25-50 mg/dose) was given in one to three 3-months courses and penile size was increased into the normal range without evoking a significant increase in height velocity or skeletal maturation. Five patients underwent the first stage of male genitoplasty between 2 and 3 years of age. This consisted of bilateral orchidopexy, chordee release and penile lengthening - yielding finally an anatomically normal-sized and shaped penis. Androgen responsive male pseudohermaphroditism due to 17 beta-HSD deficiency or a similar defect and diagnosed in infancy should be treated as soon as possible with systemic testosterone before considering any sex change, and in preparation for male genitoplasty. Early gender reassignment according to genetic and gonadal sex is probably the management of choice for these cases since this may result in a normal adjustment to the male gender role, particularly after puberty.

High serum bile acid level in obesity: its decrease during and after total fasting.

Elevated serum bile acid levels were observed in overweight subjects as compared to those with an ideal weight. Fasting for 14 days resulted in a gradual decrease to the normal level of serum cholic, chenodeoxycholic and deoxycholic acid concentrations. There was no change in the serum bile acid level during a two days refeeding period. The findings suggest that, in addition to the beneficial weight reduction, normalization of bile acid homeostasis can be achieved by fasting in obesity.

Renal candidiasis in infancy--a case with fungus ball obstruction.

A sixteen day old infant developed candiduria after surgery on a single functioning, hydronephrotic kidney with ureteropelvic junction stenosis. Masses of candida albicans caused obstruction of the ureter with acute anuria. Endoscopic relief of the obstruction together with aggressive antifungal therapy led to irradication of the fungal infection. This case history emphasizes the fact that candida infection in early childhood should be evaluated carefully. Unfortunately no guidelines are yet available for the indications and the preferred mode of treatment, and length of therapy of infantile renal candidiasis.

[Transplantation of islands of Langerhans: inclusion of living cells into a polymer forming a net-like structure].

A two-component polyacrylic amide gel forming a net-like structure was built up on pancreatic cells. In such a way the transplanted cells could be protected from the damage by immune protein with big molecule. The method was tested in yeast cells before, human erythrocytes, and wether erythrocytes. It was shown that the polymerisation and the inclusion method do not influence essentially the vitality of the examined cells. It was demonstrated by means of the complement fixation reaction that the included erythrocytes cannot be damaged by the immune protein with big molecule. Metabolic processes of the included pancreas beta cells were provable by diffusion. The experiments promise that the included cells survive after the transplantation and the polyacrylic amide gel protects the cells from the impairing effect of the immune protein.

Submucosal renal pelvic calcification simulating a pelvic stone.

We report a case of renal pelvic calcification radiologically simulating a renal pelvic calculus. Radiological and histopathological examination of the resected kidney revealed only submucosal calcification in the renal pelvis. This is the first report in the English literature of a renal pelvic calcification unrelated to amyloidosis.
