[Psychosomatic changes in video terminal operators].

The incidence of psychosomatic symptoms in a group of VDT operators is evaluated and compared with a control group (people not working at VDTs) by using questionnaires. The dta show that some psychosomatic symptoms are much more frequent in VDT operators in comparison with the control group.

[Emergency ultrasound cardiography in acute precordial pain of cardiovascular origin].

In emergency surgery, the essential prerequisites for success are speed, promptitude and precision. For this reason diagnosis must be equally prompt and precise and may require not only clinical examination but also a number of instrumental examinations designed to confirm or even to formulate the diagnosis which is usually the case in precordialgias of cardiovascular origin. In such syndromes ultrasound cardiography is particularly advantageous for the following reasons: it is non-invasive, provides immediate results and any number of examinations can be performed on the patient in bed. Since such ultrasound techniques are also reliably accurate, they constitute the method of choice when the patient's condition is critical, when a serial study of a single patient is required and when the results are required immediately, as is the case in emergency heart surgery. Four types of ultrasound cardiography are currently available for the diagnosis of precordialgias of cardiovascular origin: 1) one dimensional M-mode echocardiography; 2) two dimensional real time echocardiography; 3) Doppler ultrasound cardiography; 4) the echo-Doppler system in which Doppler ultrasound cardiography is combined with one or two dimensional echocardiography. Acute precordial pain of cardiovascular origin may be due to the following pathological conditions: 1) ischaemic cardiopathy especially acute myocardial infarction and transitory myocardial ischaemia; 2) acute pericarditis; 3) aortic stenosis; 4) idiopathic hypertrophic subaortic stenosis; 5) mitral prolapse; 6) dissecting aneurysm of the aorta; 7) pulmonary thromboembolism. In all these cases the single and two dimensional image and Doppler ultrasound cardiography provide highly sensitive and specific information that is, in some cases, decisive for diagnosis and in others confirms the diagnosis already formulated. In addition these techniques may provide valuable prognostic data. Ultrasound cardiography is indeed useful in all cardiological emergencies, such as those caused by cardiomegaly, new and developing murmurs, peripheral embolisms, cardiac traumas and arrhythmias. It is therefore suggested that every Emergency and Intensive Care Unit should be able to use the resources offered by ultrasound cardiography in diagnosis.

[Evaluation of stress indicators in 2 groups of video display terminal operators].

The results of a study on 68 VDT operators are hereby presented. Various biochemical indexes are evaluated as markers of stress. In particular, urinary catecholamines and their metabolites (E, NE, DA, HVA, VMA), ACTH, cortisol, NEFA and fructosamine have been tested by the Authors. Each assay has been performed both in basal conditions, and after the completion of subjects workshift, taking into account the normal hormonal biorhythms. The data which emerged have been analyzed by statistical method ("Student t" with coupled data). Each subject data was confronted before and after workshift. The number of hours of VDT exposure and the different types of duties have been considered and evaluated. The results show that, although the data are all included within the normal range, some of the tested parameters after workshift are significantly higher, compared with the same parameters referring to the basal condition. In particular, Epinephrine and Norepinephrine seem to be the most effective markers of stress. Analogous studies will be pursued in the future by the Authors on a larger number of VDT operators.

[Upper digestive hemorrhage (observations on 201 cases)].

The results obtained in 201 cases of haemorrhage of the higher tract of the digestive tube are reported. The role of early endoscopy (less than 24 h) and that of a "rigorous" selection of patients for surgery treatment are emphasized. The value of surgical resection in serious haemorrhage of chronic peptic ulcus is confirmed by the satisfactory results obtained.

[Bronchogenic carcinoma (personal observations)].

The authors report the results achieved in the treatment of 62 patients with pulmonary cancer "non-microcytoma". The survival rate was 20.9% at 5 years (actuarial). The best prognosis is for patients with epidermoid cancer: 35.5%. The authors emphasize the importance of the exact stage of the neoplasm and the evaluation of the postoperative cardiopulmonary functional adaptation in the prognostic evaluation of resection for pulmonary carcinoma.

[Agenesis of the gallbladder and cystic duct (author's transl)].

The authors describe a case of agenesis of the gallbladder and cystic duct diagnosed at operative cholangiography; they discuss the embryological, etiopathogenetic and clinical aspects of this abnormality, and pertinent treatment policies. They stress the need for operative teleradiocholangiomanometry for the correct diagnosis and treatment of the disorder.

[Intraoperative evaluation of the high pressure zone in anti-reflux plastic operations].

It has been done a manometric intraoperating study on 18 patients (7 acalasical megaoesophagi, 6 herniae of hiatus, 5 duodenal ulcers) who had undergone an operation of modified Dor's anti-reflux plastics. After Haller H.P.Z. pressure has gone down of 13 mmHg, and has gone up again of 5,5 mmHg after modified Dor; after plastics H.P.Z. average intraoperating pressure has been of 18 mmHg. The posterior plastics of hiatus and the reconstruction of His' angle for hernia of hiatus modify scarcely H.P.Z. pressure. During a superselective vagotomy we have recorded a modest reduction of H.P.Z. pressure, of which average intraoperating values after vagotomy and anti-reflux plastics have been of 16,5 mmHg. At a distance of 30 months from the operation it has been done a follow up on 70% of patients that had undergone an operation of anti-reflux plastics and also 18 patients that had had en intraoperating inspection. However it has been, a degradation of manometric values, the results have been satisfactory over 90% of cases. The authors insist on the utility of intraoperating manometry for a valuation of the completenes and the efficacy of the operation.
