[Community-acquired pneumonia in the elderly: the peculiarities of the clinical picture, diagnosis and treatment].

The article presents the results of studying the peculiarities of the clinical picture, diagnosis and treatment for community-acquired pneumonia in the elderly in view of the interaction of combined pathology of respiratory, cardiovascular and urological systems in order to optimize medical-and-diagnostic support of the above-mentioned category of patients in conditions of the pulmonology department of versatile educational establishment. Optimization of all-round inspection of the sick with pneumonia in the advanced age at an initial stage of treatment in conditions of the pulmonology department so as to reveal the aggravation of internal bodies' combined diseases will allow making early correction of aetiotropic, pathogenetic and symptomatic combined pathology therapy available, which reduces the terms of staying at hospital without damage of diagnosis and treatment.

[Peculiarities of inflammatory intoxication syndrome in elderly patients with extrahospital pneumonia].

The aim of the study was to evaluate the clinical and diagnostical features of extrahospital pneumonia (EHP) in elderly patients to optimize their complex medical providence under the conditions of a pulmonological department. In particular, the authors studied the manifestations of endogenous inflammatory intoxication syndrome (EIIS). The subjects, 142 elderly patients (more than 60 years of age) with EHP, and the controls, 125 patients under 40, were treated in the Pulmonology Department, Burdenko Military Hospital, in 2002-2004. The results of the study show that EIIS does not play the leading part in the clinical picture in elderly EHP patients; its manifestations are often not as prominent as those in the young and thus they can only serve as an approximate indicator for complex clinicoradiological examination, undertaken in order to reveal and determine the severity of this dangerous disease.

[The role of humoral markers of inflammation activity in the evaluation of adequacy of antibacterial chemotherapy for community-acquired pneumonia].

To determine the significance of results of dynamic study of C-reactive protein (C-RP) and ceruloplasmin (CP) concentrations in plasma for evaluation of adequacy of community-acquired pneumonia (CAP) launching antibacterial chemotherapy we investigated 97 patients (servicemen on draft). C-RP and CP were determined by immunoturbidimetric method. The blood samples were obtained on admission, on the 3rd-4th day of treatment and on the 7th-8th day of stay in the hospital. The transformations of pulmonary infiltrates were evaluated at the same periods with the use of digital chest fluorography. The strong correlation between transformations of pulmonary infiltrates for serum C-RP level (r=0.79) and CP level (r=0.71) was observed. The dynamic assessment of C-RP and CP levels is a simple and reliable test that permits to use it as an adjunctive procedure in C-RP diagnosis and evaluation of treatment effectiveness. The serum CRP level more than 110 mg/l and CP level higher than 6.3 mg/l should be considered as diagnostic and prognostic markers of severe CAP.

[Community pneumonia in recruits: the algorithm of complex two-staged rehabilitation treatment].

The program of complex recovery therapy and medical rehabilitation (MR) for treating the patients with mild form of community-acquired pneumonia (CAP) is presented. 89 servicemen-recruits with CAP mild form treated during 2003-2005 in the Field Therapy Clinic of Saratov Military Medical Institute were investigated. MR was conducted on the basis of medical station (MS) and lasted for 15 days. Physiological, laboratory and instrumental parameters were evaluated at the day of antibiotic therapy cancel, on discharge from hospital, on the 5-th and 15-th day of MR. The peroxidation activity, intensity of unspecific resistance mechanisms and obstructive impairment of bronchial passages remained during CAP late phases. Only on the 15th day of extensive medical rehabilitation measures in 80,7% of those suffered from CAP the reserve capacity indices of cardiovascular system reached the lower level of the border state of PWC170 test scales, Skibinskiy's and Bogomazov's tests. The proposed two-staged program of medical rehabilitation in CP cases which includes the combined use of actoprotector and antioxidant, restoration of bronchial tree drainage function, early introduction of breathing exercises and cyclotraining allowed achieving the restoration of working and fighting capacity in short terms. The stay in pulmonologic in-patient department became 3 days less.

[Treatment of visceral pathology in patients with trauma].

The problem of visceral pathology in patients with trauma that was intensively investigated by the scientists during the period of Great Patriotic War and in post-war period remains actual in our days. The purpose of investigation is to study the possibilities, peculiarities and efficiency of diagnosis, treatment, rehabilitation and medical examination of the casualties suffering from visceral pathology in different periods after trauma in conditions of therapeutic hospital as well as the peculiarities of their regular medical checkup at the post-hospital stage. It will allow to use more widely the therapeutic beds for the complex medical support of this patient group. 692 patients admitted in different periods after mild mechanical trauma complicated by different visceral diseases were investigated in conditions of therapeutic hospital. After the complex investigation the following conclusion was drawn: the casualties with mild closed injury complicated by visceral pathology even in the early posttraumatic period can be send to the therapeutic hospital where the adequate medical care will be rendered. It is especially important during the simultaneous mass admission of the casualties in the time of natural disaster and catastrophes, in conditions of military operations when the surgical subunits of medical evacuation stages will be full of persons who require the active resuscitation and surgical intervention. Complex medical support of the casualties with visceral changes in the late periods after trauma is the many-sided process connected with solution of therapeutic, rehabilitation, expert-and-diagnostic problems that are fully in the sphere of specialists of therapeutic institutions.

[Use of low-dose digital fluorography in patients with acute infiltrative pulmonary processes].

Basic purpose of issue was the attempt of a rating of a role and place modern digital chest fluorography in complex diagnostics of lower respiratory diseases accompanied acute pulmonary infiltrates. We studied 232 adults who hospitalized with community-acquired pneumonia (CAP) to Saratov Military Medical School Hospital during 1999-2003. In integration of methods of research the role digital chest fluorography as way of primary diagnostics and conducting means of the analysis of technology of rendering of medical aid is emphasized. At the expense of use of the developed algorithm of methods of beam visualization at the control of a course of stationary treatment of the patients CAP it was possible to decrease length of stay for 2.5 day.
