[Clinical and morphological characteristics of long-lasting cholestatic syndrome in patients with hepatitis A in the presence of HBV infection].

Long-standing cholestatic syndrome (LSCS) was diagnosed in 41 patients with hepatitis A and hepatitis A plus chronic persistent hepatitis B (HA + CPHB). Some patients had a cholestatic form of the disease. Morphological and morphometric liver parameters were measured at the height of icterus in 74 patients. It was found that cholestatic syndrome in patients with HA and HA = CPHB is related to reparative regeneration which always has a favourable outcome.

[Splenic rupture in tropical malaria].

The authors have observed splenic rupture in patients mainly with P. vivax malaria in 0.37% of cases. A rare case of splenic rupture in topical P. falciparum malaria is described. The patient underwent splenectomy. He was discharged from hospital after his recovery.

[Algid malaria].

Algid is a rare complication of tropical malaria and it occurs in 0.37% of cases. Algid malaria is characterized by hemodynamic disorders as shock with pronounced metabolic changes and hypothermia. A number of factors are involved in the development of algid malaria. These include: 1. Pathological phenomena that are associated with the changes in the state of red blood cells and lead to impaired microcirculation (cytoadherence, sequestration, rosetting); 2. Tumor necrosis factor (TNF) that provokes hypoglycemia, coagulopathy, and impaired erythropoiesis; 3. Altered acid-alkali balance with the development of metabolic acidosis; 4. Gastrointestinal lesion. Adherence of contaminated red blood cells in the intestinal mucosal vessels induces epithelial ischemic damage. Impaired absorption of liquid and its loss with vomiting and diarrhea result in acute hypovolemia; 5. Algid malaria is associated with the addition of gram-negative septicemia. The paper describes a case of algid malaria.

[Ixodes tick-borne borreliosis with erythema nodosum].

Ixodes tick-borne borrelioses (ITBB) is a group of natural-and-focal transmissible infections caused by Borrelia burgdorferi and characterized by multiple organ dysfunctions and a chronic progressive course. The pathognomonic marker of an acute period of the disease is migratory erythema at the site of tick suction. There may be other skin symptoms: rash on the face, urticaria, transient pointed and minor eruptions. Diagnosis presents some certain difficulties in these cases. The paper gives a description of rare eruptions in ITBB.

[Parasitic carriage of the causative agents of tropical malaria].

In 1993 to 1999, a total of 1020 Russians temporarily living in the Republic of Guinea were examined; of them 26 (2.5%) persons were found to be asymptomatic carriers of P. falciparum. Single studies of cellular and humoral immunity were first conducted in 15 asymptomatic parasitic carriers of P. falciparum. Asymptomatic parasitic carriage of P. falciparum was established in nonimmune persons who developed immunopathological reactions. The nature of immune disorders gives grounds to regard parasitic carriage as a subclinical form of tropical malaria for the first time. By taking into account asymptomatic carriage of P. falciparum, a parasitological blood study should be made in all non-immune persons when they are leaving an endemic focus for Russia.
