[Linear dermatosis].

Some typical distributions of skin lesions represent a helpful sign for the differential diagnosis in dermatology. Indeed, some dermatoses, affecting the skin, nails or mucosae, display an annular or linear disposition. The linear pattern can be explained by several factors. The dermatosis can follow either a nerve traject, vascular or lymphatic vessels, or embryological lines. Exogenous factors, including traumatisms or automutilations, can also contribute to this particular distribution. In this review, the principal linear dermatoses are discussed together with their pathogenesis.

[Molecular photobiology].

Photochemical reactions are numerous in the skin. They generate reactive oxygen species and other biochemical alterations as well. According to their nature, the molecular components of the skin which have been altered by these mechanisms can be repaired with various degrees of efficacy.

[Photoexacerbated dermatoses].

A series of viral and bacterial diseases are photoaggravated. Some autoimmune connective tissue disorders including lupus erythematosus and dermatomyositis are also affected. This category of photoexacerbated diseases also encompasses some cases of atopic dermatitis, lichen and rosacea.

[How I treat ... basal cell carcinoma by imiquimod].

Basal cell carcinoma is the most frequent cancer in humans. Several clinical types are distinguished. They are bound to distinct evolutive prognosis. The surgical excision is the treatment of choice which is rarely followed by recurrence. However, when the lesion is superficial and non aggressive and when the body site is adequate, topical applications of imiquimod can provoke the neoplastic regression. This type of immunotherapy brings 70 to 90% complete remission. A medical follow-up of the treated site is mandatory for a couple of years.

[Pharma clinics. How I treat ... recurrent herpes labialis].

Recurrent herpes labialis is a frequent disorder. It occurs following the reactivation of the Herpesvirus (HSV1 and more rarely HSV2) inside the Gasser ganglion. Treatment and prevention of recurrent labial herpes are targeted by specific antiviral agents. Spectacular benefits obtained in the immunocompromised patients are less convincing in otherwise healthy subjects. Other prospective ways of therapy are under consideration, including lipopeptides and physical means aiming at modifying the cutaneous pH.

[Aroma and perfume allergy: anathema for some epicurean appeal?].

Aromas and fragrances are present in many cosmetics, some topical drugs, food and various hygiene, household and industrial products. They can be responsible for contact dermatitis. Multiple sensitizations can even involve in various combinations some fragrance compounds, a given degradation product or a contaminant. The diagnosis relies on clinical examination and oriented anamnesis. A histological examination is sometimes necessary. Specific path testing brings insight on the culprit chemical compounds.

Allergic reactions to decorative skin paintings.

BACKGROUND: Dye mixtures containing henna, P-phenylenediamine (PPD), and indigo are applied to the skin in various parts of the world for religious purposes, social recognition or fun. AIM: To identify eventual allergens responsible for contact sensitization to skin decoration in three young children. Lesions remained strictly limited to the site painted during the summer holiday. None of the children had a previous history of contact dermatitis, but two of them suffered from severe seasonal allergic rhinoconjunctivitis. FINDINGS: Patch testing revealed severe contact hypersensitivity to PPD in all three patients, and one child presented a positive prick test to 1% henna in water. The eruptions resolved following spontaneous elimination of the dyes and topical applications of corticosteroids, but left residual depigmentation. CONCLUSION: PPD is the main allergen identified in allergic reactions to decorative skin paintings.

[How I explore ... the risk of metastasis from melanoma].

The development of new therapeutic protocols for malignant melanoma calls for an upgraded assessment of the individual risk to develop metastases. The predictive sentinel lymph node method is best practiced in a multidisciplinary approach. The selective lymphadenectomy of the first draining lymph node is followed by minutely detailed histologic examination combined with immunohistochemistry.

[Factual approach to the treatment of genital herpes].

Genital herpes is a sexually transmitted disease. After the primary infection, the virus establishes a life-long latency in the sacral dorsal root ganglia. Recurrences may occur at an unpredictable rate. The clinical signs are not always easy to recognize and viral identification techniques may be helpful such as immunohistochemistry and in situ hybridization on Tzanck smears and muco-cutaneous biopsies. The treatment of genital herpes can follow one of three strategies using antiviral drugs, non-specific immunomodulators, and vaccination. The new oral antiviral drugs decrease the severity of clinical manifestations without, however, providing a definitive cure. In this article recent knowledge about the clinical aspects, differential diagnosis, diagnostic methods, treatment options and management is reviewed.

[Eruptive tropical viroses].

Eruptive tropical virosis are severe diseases which may prove to be sometimes fatal diseases. They are characterized by erythemato-purpuric cutaneous lesions associated with hyperthermia. Polyarthralgia or severe haemorrhage can also be part of the clinical presentation. Rapid diagnosis and preventive measures are mandatory, particularly when human transmission is possible.
