The aim of this study is to report on the growing frequency of neurocysticercosis (NCC) in Madrid (Spain) related to changes in migratory flows. We investigated the origin of 20 patients with NCC seen over a period of 13-years (1990- 2002). Patients age and disease activity were compared with a second group of 13 patients observed in the same hospital in 1980-1989. Eighty per cent of the patients observed in the first group were foreign immigrants, all but one from South-American countries. They were young (mean age: 31.4 +- 7.7 years) and most had evidence of active disease. In contrast, only 15 % of the 13 patients observed during 1980-1989 were foreign immigrants. Recently seen Spanish-born patients were elderly (mean age: 67.3 +- 2 years), had emigrated many years earlier from rural areas where the disease was formerly prevalent, and had inactive forms of the disease. We conclude that the recent increase in the frequency of NCC in Madrid reflects a rapid growing of immigrant population from countries where the disorder is highly prevalent. Transmission of the parasite from asymptomatic T. solium carriers represents a challenge to disease prevention. Our observations should alert other European countries that may become potential destinations for similar inbound migratory flows.
Formerly an endemic disorder, the frequency of neurocysticercosis (NCC) in Spain has been declining during recent decades until reaching its near extinction. However, the strong migratory flow during recent years towards large cities from countries where NCC is highly prevalent, particularly the Andean area of South America, has been followed by a growing increase ot this infestation among immigrants. Since NCC is commonly acquired by direct contamination from carriers of the tapeworm Taenia solium, there may be an emergence of NCC among Spanish-born population unless preventive measures are taken.
The role of the male rat pubococcygeus muscle (Pcm) in the micturition reflex was analyzed. Anatomical features of Pcm, electrical stimulation of its nerve, electrical recording and stimulation of the muscle and cystometrograms were carried out. Results showed that Pcm has fibers attached to the ventrolateral part of the external urethral sphincter, and that its activity contributes to hold the tail in the midline. Pcm shows activity during fluid expulsion in cystometrograms and spontaneous micturitions. This activity produced reflex inhibition of detrusor contraction and was not the cause of intravesical high frequency oscillations. Thus, it is proposed that Pcm activity produces the discharge of its afferents which in turn activates a spinal reflex to promote continence.
1.897 patients with upper gastrointestinal bleeding (UGIB) were studied and the cause was diagnosed in 1.756 (92.6%). The most frequently found pathology was the acute bleeding gastropathy (24.5%). Considering the gastric duodenal and anastomotic ulcer as a whole, in 47.5% of the cases the ulcer was observed. Both pathologies together make out that 72 out of 100 patients with UGIB have bleeding due to an ulcer or gastritis. It comes out that the low incidence of neoplasy as a consequence of UGIB and that the 31% of the diagnosed pathology could not have been diagnosed by X-ray.
