Neonatal lupus syndrome: a case with chondrodysplasia punctata and other unusual manifestations.

We report a case of a newborn infant whose mother had systemic lupus erythematosus (SLE) diagnosed before pregnancy. The child had clinical manifestations of neonatal lupus as well as chondrodysplasia punctata and other findings that resemble the congenital anomalies associated with the use of oral anticoagulants, with no history of exposure. We speculate that the combined action of the different maternal autoantibodies may produce the whole spectrum of manifestations.

[Excretion of promastigotes of Leishmania pifanoi by experimentally infected Lutzomyia youngi].

The increase in the promastigotes population of Leishmania pifanoi in Lutzomyia youngi experimentally infected and kept on 50% sacarose under constant conditions of temperature and humidity is described. Two stages in the differentiation and growth of the parasites are recognised between two and twenty-four hours after meals. The pleomorphic differentiation of the amastigotes in short promastigotes which multiply by binary division for 60 hours, when the rupture of the peritrophic membrane occurs, takes place within 48 hours. The second stage occurs between 72 and 96 hours when some of the parasites migrate to the esophagic valve and the rest of the free parasites are excreted in fecal drops as large, active promastigotes. The first drops excreted react positively to glucose or contain crystals of urate. The excess promastigotes of the second stage of development are eliminated in the last excretions and react positively to the Hemoscreen and Biuret tests for total proteins as also for glucose and account for 82% of the of drops excreted. The excretion of parasites by Lu. youngi is a normal stage in the growth of L. pifanoi into a vector.

[Extrapelvic endometriosis involving the perineum].

The ectopic endometriosis in the perineum is very rare (0.2% of ectopic endometriosis). We studied 16 cases of vaginoperineal endometriosis. Fifteen of them were localized in the site of an episiotomy scar and in one there was another focus of a severe endometriosis of multiple localizations. All the patients were submitted to anamnesis, physical, gynecologic and proctologic examination, surgical excition, histopathologic study of the lesion and control. All the patients are asymptomatic after treatment. To cure definitively the disease it is necessary to make a complete ablation of the endometrial lesion. Eventually, hormonal treatments could be use in case there were other endometrial localizations besides the vaginoperineal one.

[Superficial invasive squamous carcinoma of the vulva].

We present our experience at University of Chile Clinical Hospital, about superficial scamous vulvar carcinoma, during January, 1967 and april, 1992. We studied eight patients, mean age 57.1 years. Diagnostic was histopathologic in all cases. Three patients were submitted to simple total vulvectomy; other three were treated by total vulvectomy and bilateral inguinal lymphadenectomy (classic form of treatment). The other two cases, were submitted to simple in one case and partial vulvectomy in other, with lymphadenectomy by separate bilateral inguinal incisions. Histopathological study proved in situ scamous carcinoma near invasive focus at 50% of all cases. Follow up was between 2 and 25 years, mean 10.5 years. In one patient, carcinoma in situ relapsed at 2 years and 8 months after treatment, and then, at 8 years after primitive treatment. In other patient, and anal carcinoma was demonstrated 20 years after vulvectomy.

[Female genitoperitoneal tuberculosis an in force pathology].

We present the experience during 1987-1991 years, about Female Genital Tuberculosis, at the University of Chile Clinical Hospital. We present 16 cases with histologic and bacteriologic demonstration; in two cases also positive cultures for myco bacterium tuberculosis. It is made emphasis about personal characteristics of patients physical examination findings, laboratories procedures and technics of diagnostic, treatment and posterior control of patients. We comments utility of new procedures as laparoscopy, gynecologic sonography and A.D.A. analysis of ascitis.

[Multiple splenic abscesses. Report of 2 cases].

Splenic abscesses are infrequent and usually associated to sepsis and immunodepression. The diagnosis is commonly made in the necropsy. We report two cases of multiple splenic abscesses in type II diabetic patients that were diagnosed by ultrasound. Both patients were successfully managed with surgery (drainage of left subphrenic space in one and splenectomy in the other) and antimicrobial agents.

IgA reactivity in patients with intestinal or hepatic amebiasis.

The antigens of E. histolytica recognized by IgA antibodies in sera from patients with either amebic liver abscess or intestinal amebiasis, as well as saliva from the latter, were determined in immunoblots of whole native trophozoite proteins. Results show that sera of patients with amebic liver abscess reacted mainly with amebic proteins of more than 200 kDa, whereas those of individuals with intestinal amebiasis recognized a protein of 145 kDa. The saliva of the latter detected a protein of 210 kDa which is not always seen when using the sera of patients with intestinal or hepatic amebiasis.

[Ultrasonography-guided transvaginal puncture of ovarian cysts].

Presenting the experience in 18 cases of ovarian cystic transvaginal puncture guided by ultrasonography. The cystic selected for puncture were unilateral, regular contours, uniloculated without internal papillae, without ascites. This procedure was made in women on reproductive age with failure to medical treatment.

[Compliance with tuberculosis treatment in adults in Santiago, Chile].

A prospective study was conducted to estimate the current magnitude of adherence to short-course tuberculosis treatment, the degree of abandonment, the characteristics of treatment dropouts, and the causes of this abandonment. The study group was made up of tuberculosis patients over the age of 15 who received care at the Western and Southern Health Services of the Metropolitan Region of Santiago, Chile, between 1 October 1987 and 31 January 1988. The percentage abandoning treatment, calculated by the life table method, was 11.5. The profile of patients who dropped out of treatment was as follows: male, under 45 years of age, single, low level of education, no steady work, homeless, and alcoholic. In addition, an opinion survey on the variables associated with abandonment was conducted and it was concluded that the main ones were alcoholism and intolerance to tuberculosis drugs. Awareness of this profile makes it possible to take measures to prevent patients from abandoning treatment, as well as to educate and even hospitalize at the start of treatment those tuberculous patients exhibiting such a profile.

[Intraepithelial squamous cell carcinoma of the vulva, 1972-1989].

We report our experience about intraepithelial squamous carcinoma of the vulva, consisting of 20 cases with clinical and histopathological diagnosis, at the University of Chile Clinical Hospital. Between december 1972 and october 1989. The age of patients was between 30 and 78 years old, median age 57.6 years old. Clinical HPV infection was detected in three cases (15%) 17 patients were submitted to simple total, vulvectomy (94.5%) without operatory mortality. The follow-up was between 3 months and 11 years (mean follow-up 6.5 years). Recurrence was observed in 2 patients (11.1%) of the 18 submitted to treatment.

[Multifactorial analysis of patients with endometriosis in an institutional program of assisted reproduction].

It is a retrospective and transverse study with an analysis of results from patients submitted to intrauterine insemination (IIU), intratubal gamete transfer (GIFT) and in vitro fertilization and embryo transfer (FIVTE), at the Clinic of Assisted Reproduction, Instituto Nacional de Perinatología in Mexico City. From 134 cycles of patients submitted to IIU, 113 corresponded to women with diagnosis of sterility from non determined cause, and 21 to cases with endometriosis, and at least one permeable tube. From the first group, 22 pregnancies were obtained, for a monthly fertility rate of 19.4%; and from the second one, 12 pregnancies with a fertility rate of 56%. This, which is very high , may be explained by the relatively small number of cases in the second group, or else, by the influence of a previous medical treatment for endometriosis; the square Chi analysis shows a P less than 0.01 in the two groups. In the GIFT and FIVTE cases, from which 35 cycles were analyzed, per each procedure, there were 10 pregnancies, 9 for GIFT and one for FIVTE; the amount is very limited in order to obtain conclusions, but it seems that there is a proportional tendency in GIFT to less amount of pregnancies in women with endometriosis. The analysis of hormonal follow up did not show differences in the groups, and the averages may be useful to determine, early, the cancellation, or not, of an stimulation cycle. Finally, in FIVTE cases there is a diminution of fertilization index in women with endometriosis (53%), as compared with the cases without disease (80%).(ABSTRACT TRUNCATED AT 250 WORDS)

A case of intravascular leiomiomatosis is presented. This gynecologic diseases is rare, although it can be of risk. A case is described with a review of the literature.

[Verrucous carcinoma of the vulva].

We reported a case of verrucous carcinoma of the vulva, (VCV) presented at an 71 year old caucasian woman, who was admitted to University of Chile Clinical Hospital, December 11, 1987. She had complained of vulvar itching, pain and a tumor at left labium mayor during the last ten years. The tumor was removed, and then a simple vulvectomy with bilateral superficial lymphadenectomy through separate groin incisions was performed. All the 27 inguinal nodes were free of metastasis, and the vulvar skin presented a mixed dystrophy with light atypia (VIN I). She is alive and well one year after surgical treatment.

[Diagnosis of sexually transmitted diseases in Chilean pregnant adolescents].

We searched in 100 healthy pregnant women by isolation, the presence of Neisseria gonorrhoeae, Herpes simplex, Mycoplasma hominis, Ureaplasma urealyticum and Chlamydia trachomatis. Blood was also taken for examination of specific antibodies to these microorganisms. We studied only for antibodies titled Cytomegalovirus (CMV), Treponema pallidum and Human Immunodeficiency Virus, and Condyloma acuminatum by cervical cytology. In 85 adolescents we found 5 (6%) patients with C. trachomatis, four of these patients had another microorganism added, one with N. gonorrhoeae, M. hominis and U. Urealyticum, one with U. urealyticum and the last two with M. hominis and U. urealyticum, In relation to Mycoplasmas 69 (81%) out of 85 had Mycoplasmas, 4 (5%) had M. hominis, 46 (54%) U. urealyticum and 19 (22%) patients had both. The seropositivity to CMV was 96.25%. We didn't find any other microorganism. We concluded that the rate of STD in chilean pregnant adolescent women is high, especially with no traditional bacteria.

[Intramyoma intrauterine device].

This article describes a cases of "missing strings" of a Zipper IUD embeded in an intracavitary myoma. Two attempts with classical procedures failed to remove it and the diagnosis was done after a hysteroscopy complicated with uterine perforation. It is very difficult to assure if the myoma grew around the IUD or it was perforated at the time of its insertion. Histological evidences lead to the first option.
