Fetal pituitary transplants into the hypothalamic area of hypophysectomized rats.

The pituitary tissue, when transplanted to extracerebral sites other than the normal anatomic position, fails to perform its secretory function properly. Several works on experimental intracerebral pituitary transplantation have shown that the transplanted tissue has been able to survive histologically. In this study, fetal pituitary tissue was implanted into the anterior hypothalamic area of the hypophysectomized rat by stereotactic methods. Electron-microscopic examination and hormonal blood levels demonstrated that ectopic pituitary tissue was able to survive to establish the correct anatomic relationship and thus perform secretory functions when transplanted intracerebrally.

[Cisterna megamagna. Dimensions and relations to cerebral atrophy. Clinical and x-ray computed tomographic demonstration].

Polaroids of 9,400 consecutive unselected computed tomography (CT) scans are reviewed and 31 cases with Mega Cisterna Magna (MCM) are detected. The incidence is 0.33% (31/9,400). Clinical and radiological data relating to these 31 cases shows that none of the patients presented neither any clinical sign of a posterior fossa space occupying lesion nor any radiological image such as hydrocephalus or forward displacement of the fourth ventricle. The dimensions of the cistern ranged as follows: width between 22.2-92.5 mm, depth between 22.2-37.0 mm, height between 26.0-39.0 mm. The authors propose 20 mm as the limit value of width and depth of a normal cisterna magna and 26 mm for its maximal height, these dimensions being the most frequently encountered ones of these 31 patients. The correlation between the magnitude of the cistern (the sum of the width and depth of the cistern: w + d) and the severity of cerebral atrophy (the sum of the maximum and minimum widths of the anterior horns of the lateral ventricles which is the most reliable atrophy index defined by Hirashima: a + b) is illustrated in figure 5. The authors conclude that Mega Cisterna Magna which has a constant incidence in routine CT examinations is predominantly a consequence of cerebral atrophy and does not have any specific clinical pathological correlate and does not require any form of treatment.

Spinal meningiomas.

Between the years of 1970-1982 29 spinal meningioma cases were operated on; 22 cases (76%) were female and 7 (24%) were male. The youngest patient was 16, and the oldest was 68 years old. On neurological examination, patients had neurological deficits in varying degrees. Diagnosis were done by myelography in all cases. Laminectomy was performed in all cases. 34.4% of cases were situated in cervical region and 65.6% were in thoracic region. 14% of the tumours were located extradurally and 68% were intradural. Multiple spinal meningioma was seen in only one case. One case died after operation; the mortality rate is 4%. 22 cases came for follow-up examination, 86.5% of them were in excellent or good condition. Only three cases showed no improvement.

Histopathologic and microdensitometric analysis of craniotomy bone flaps preserved between abdominal fat and muscle.

In this study we have investigated the preservation of craniotomy bone flaps within the abdominal wall and the use of such bone flaps for cranioplasty in the early postoperative period. The craniotomy flaps were implanted into a "pocket" between abdominal fat and muscle of adult rabbits, and were later used as autologous grafts in cranioplasty. The flaps were analyzed by histopathologic and microdensitometric methods. In 36 of the flaps, new bone formation near the periosteum was observed. The bone flap-cranium relationship was supplied by a bony tissue. Microdensitometric analysis showed increased optic densities in the experimental group.

[Primary carcinoma of the choroid plexus. 2 case reports and review of the literature].

Primary Carcinomas of Choroid Plexus are seen rarely. In previous literature there were 37 cases which were reported according to Lewis and Russel-Rubinstein malignancy criterias. In this article, we describe 5 years old boy and 1.5 year old girl who suffered from cerebellar ataxia and intracranial pressure increase syndrome. In case 1, the tumor of the fourth ventricle was diagnosed by brachial angiography and ventriculography. In case 2, the diagnosis was established by CT scan. In case 1, the patient died during the postoperative course. In case 2, the postoperative time was unremarkable. A radiotherapy of 3 000 rads was applied to the posterior fossa. One year later, the patient had only mild ataxia.

[Intracranial ependymomas. Study of 81 cases and comparison with the literature].

For ten-years of period, between 1973-1982, 81 cases with intracranial ependymoma have been operated in H.U.T.F. Neurosurgery Department. Supratentorial localisation have been found in 24 cases (30%), and infratentorial in 57 (70%). 80% of the patients in supratentorial group and 91% in infratentorial group were under the age 20. In supratentorial group 79% were in paraventricular localisation, and 21% were intraventricular. In infratentorial group 70% of the cases were localised within the IV. ventricle, 20% in province of the tentorial roof, 5% in the lateral recessus and 5% into cerebellum. The 40 patients who were followed up were rated according to the localisation and the grades of the tumors, the type of the surgical intervention and chemotherapy with respect to the postoperative survival. As a result only the total resection of the tumor was found to be significant.

Spinal hydatid cysts.

Eleven cases of spinal hydatid cyst are reported. The patients were admitted with symptoms of compression of the spinal cord. The cysts were localized epidurally in 10 patients and intradurally but extramedullarily in one, and were confirmed histopatholgically after surgical intervention. The results are compared with those reported in the literature. Six of the patients were female and five were male. Their ages ranged from 10 to 65 years. All of the patients were treated surgically, with an incidence of cyst recurrence of 18% and no mortality. Mebendazole (Vermox) was given to two of the patients after their operations.

Orbital echinococcosis. Report of two cases studied by computerized tomography.

Two cases of orbital echinococcus cysts studied by computed tomography are presented. Incidence of orbital hydatidosis in Turkey is reported. Computed tomographic findings of orbital hydatid cysts are reviewed and summarized. Medial displacement of the orbital plate of the ethmoid bone is emphasized as a frequent finding in the reported cases.

Papilledema in Behçet's syndrome.

Six patients with Behçet's syndrome (five male, one female) had intracranial hypertension diagnosed by elevated CSF pressure in the presence of normal or small ventricles. All six patients had headaches and papilledema. Histories included oral ulcers in 5 patients, genital ulcers in 3, uveitis in 1, arthralgia in 4, and thrombophlebitis in 3. One patient had obstruction of the superior vena cava. A filling defect in the superior sagittal sinus was shown in two patients by carotid angiography, and cerebral circulation time was prolonged in one patient. Papilledema and intracranial hypertension as a result of cerebral vein thrombosis can be initial manifestations of Behçet's syndrome. This has not been a well-recognized feature of the syndrome.

Primary choroid plexus carcinoma in an infant. Report of a case evaluated by MRI.

Primary carcinoma of the choroid plexus is a very rare neoplasm. We report a 17-month old female patient who was evaluated by MRI. The tumor was gross totally removed and radiotherapy was given subsequently. CT and MR correlation and histopathological differential diagnosis criteria of such tumors are discussed with an emphasis on total tumor removal. New chemotherapy regimens are needed for better survival especially in cases of recurrences.

Craniosynostosis: a review of 143 surgically-treated cases.

In this study, 143 cases of craniosynostosis are presented. There were 109 males and 34 females. The major complaints were skull deformity (92 patients), proptosis (38 patients) and microcephalus (32 patients). Neurological examination revealed the presence of optic atrophy in 24 patients and papilledema in 20 patients. Seventy-four patients (53%) had three or more suture closures, with the sagittal suture being the most commonly involved (20% of patients). All patients underwent surgery. Suture removal was performed in 131 patients (91.7%), suture removal plus orbital decompression in 34 (23.8%), and linear craniectomy plus wrapping in 12 (8.3%). The reoperation rate was 6.2 percent. During the follow-up period, preoperative papilledema and proptosis improved in 88.2 and 78.9 percent of patients, respectively. Skull deformity disappeared in 46.9 percent of patients, but remained unchanged in 16.6 percent.
