Chronic pancreatitis in north-east Italy: a clinical and pathological study.

The clinical and pathological features of 132 patients of North-Eastern Italy with proven chronic pancreatitis (presence of radiological pancreatic calcification and/or surgical and histological data) have been studied. The disease appeared to be associated with chronic and regular alcoholic habits in most cases. Histo-pathological examinations showed calcifying pancreatitis also in patients without radiological pancreatic calcification. Chronic pancreatitis in North-Eastern Italy seems to be similar to that described in France, except for a high frequency of associated gallstones.

[Changes in the epiphysis and epiphyseal plate in systemic and genetically-induced diseases].

Systemic disorders of the skeleton have either a primary or endogenous, mostly heritable cause like osteochondrodysplasias or they are secondarily or exogenously caused by pathologic influence of the metabolic or endocrine system. The functional unit of growth plate and epiphysis is frequently the target organ of these pathologic influences. The pathogenetic effect on the skeleton is manifested as a disturbance of growth and as an epiphyseal deformity and structural change. Here one must take in consideration the fact that these pathological influences affect not only the epi-and apophyses of tubular bones but also those of the skull, the pelvis and the spine. Because of that and because of the unequal quantitative effect of growth at different growth plates and the chronological different developmental state of the epiphyses, very variable disorders are the result. Disturbances of growth lead to proportional or disproportional short or tall stature. Epiphyseal deformations may have the consequence of contractures and early osteoarthritis. Joint replacement at the hips and knees is inescapable and requires, because of the diminutive skeletal proportions, a special custom -made prosthetic device. Slight epiphyseal deformities, like cone shaped epiphyses, result not so much in functional disturbance but may rather be used as a diagnostic clue. By means of the most typical and important systemic skeletal disorders the pathogenetic principles are discussed.

[The role of a bone cement filling in intralesional tumor treatment].

The article reports on the results of treatment of cystic and juxta-articular benign primary bone tumors and tumor-like lesions with a temporary bone cement plug. The mean implantation time of the cement plug was 18 months, following which the cement plug was replaced with an autologous spongiosa filling. Cases were followed up for an average of 58 months. In the patient group as a whole, a relapse rate of 5.8% was recorded (0% for Giant Cell Tumors alone). It was possible to describe the clinically functional results as "good" and "very good" in 95% of the cases. It is clear from these results that temporary plugging of large juxta-articular bone lesions is an excellent treatment method. The article goes on to describe complications in detail. Results are given in detailed tables.

[Changes in the juvenile pelvis in congenital systemic diseases of the skeleton (author's transl)].

Constitutional disorders of the skeleton show a characteristic appearance in the radiogram of the juvenile pelvis. These radiologic changes are presented in tables and are discussed demonstrating individual cases. Because of the limited reactive possibility of the skeleton not every disorder shows a typical feature of differential diagnosis and radiologic pattern. To differentiate from the congenital dysplasia of the hip and Legg-Calvé-Perthes disease the distribution of the skeletal disorders and their course therefore have to be known. Accordingly the therapeutic consequences have to be drawn. In this purpose the radiologic diagnostics of the juvenile pelvis are of significant value.

[Radiological and histomorphological correlation of bone tumors and tumor-like diseases (author's transl)].

In the diagnosis of bone tumors and tumor-like lesions the radiographic examination plays a dominant role in the phase before performing a biopsy, with the support of this examination important facts are available for the further diagnostic procedure, for planning the biopsy and also for getting the preliminary guiding principles how the therapy has to be performed. In order to interpret a radiograph of a diseased bone optimally it is necessary to know the histomorphological background of an X-ray feature. This more particularly as the reaction of bone to a pathological process is limited to only 2 radiographic images: osteolysis or/and osteosclerosis. On the basis of the histological pattern and the dynamic micromorphological process the picture of osteolysis and osteosclerosis is analysed. The periosteal reaction is also considered in this manner. After the representation of these general principles a detailed tabulated synopsis is given about the correlation between the histological and roentgenological findings of all benign and malignant bone tumors and tumor-like conditions.
