Heparin inhibits PGE1-sensitive adenylate cyclase and antagonizes PGE1 antiaggregating effect in human platelets.

PGE1-sensitive adenylate cyclase in human platelet lysates in inhibited (50%) by heparin (10 microgram/ml). Inhibition is of a mixed type when analyzed by double-reciprocal analysis. It is also shown that heparin (2.5 to 10 microgram/ml) effectively antagonizes the antiaggregating effect of PGE1 on ADP-induced platelet aggregation. The possibility that heparin may exert its platelet-aggregating activity through inhibition of PGE1-sensitive AC in human platelets is discussed.

Granulocyte transfusion therapy: a clinical trial in patients with acute leukemia and sepsis.

The effect of granulocyte transfusions on the course of infection in patients under treatment for acute leukemia was evaluated by comparing 19 febrile episodes in 15 patients receiving antibiotics alone with 18 febrile episodes in 13 patients receiving antibiotics in combination with granulocyte transfusions from ABO-matched donors. Both groups had a similar age, sex distribution and duration of disease prior to the febrile episode. About two-thirds of the patients in both groups had acute myeloblastic leukemia. 94% of the patients in the transfused group and 74% of the control group survived the febrile episode. In patients with positive blood cultures all transfused patients survived as compared to only 57% in the control group (p=0.05). In patients with persistent bone marrow failure 92% of the transfused patients survived as compared to 73% in the control group. Granulocyte transfusions had no effect on the outcome of febrile episodes in patients with negative blood cultures or early recovery of marrow function. These data appear to support the contention that granulocyte transfusions are beneficial in patients with blood culture-proved sepsis with persistent neutropenia.

Abnormal platelet functions in beta thalassaemia.

Haemorrhagic tendency, including frequent epistaxis and easy bruising, has been observed in patients with beta thalassaemia major. Disturbances in the coagulation system were also described in this condition, which probably resulted from liver damage associated with this disease. There was, however, no quantitative or qualitative correlation between the haemorrhagic manifestations on the one hand and the abnormalities in the clotting mechanism on the other. Platelet functions were studied in 15 patients with beta thalassaemia major and in 5 with thalassaemia minor. In most of the thalassaemia major patients and in some with thalassaemia minor, diminished platelet aggregation to ADP, collagen, ristocetin and epinephrine was found. These anomalies could not be corrected by the resuspension of the thalassaemic platelets in normal plasma.

Hemorrhagic tendency in beta-thalassemia major.

A mild hemorrhagic tendency was observed in a group of beta-thalassemia major patients. This included easy bruising and frequent epistaxis. A consistent platelet anomaly manifested by diminished platelet aggregation to ADP, collagen, ristocetin and epinephrine was found in these patients, and could be responsible in part for the hemorrhagic phenomena.

Aberrant congenital dyserythropoietic anemia with negative acidified serum tests and features of thalassemia in a Kurdish family.

Three siblings of a Kurdish Jewish family with clinical and hematologic findings compatible with congenital dyserythropoietic anemia (CDA) are described. All patients presented with mild anemia, marked hyperbilirubinemia and splenomegaly. The bone marrow morphology and ultrastructure of the normoblasts was typical of CDA type II and there was strong agglutination of the patients' red blood cells by anti-i serum. These patients displayed two features that were not characteristic of CDA type II, namely, the acidified serum lysis test was negative on more than 10 occasions, and high levels of Hb A2 were observed in two siblings. In one of the siblings, abnormal globin-chain synthesis was found and alpha-chain production exceeded beta-chain production, as in beta-thalassemia minor. In the light of the above findings, our patients are perhaps best classified as having aberrant CDA with features of thalassemia.

Multiple myeloma terminating in lymphocytic leukemia with B-lymphocyte membrane markers.

The development of lymphocytic leukemia with a rapidly fatal clinical course is reported in a patient with kappa light-chain multiple myeloma treated with alkeran. The leukemic cells lacked the ultrastructural features of plasma cells but bore readily detectable B-cell markers and resembled lymphocytes under the light, transmission, and scanning electron microscopes. The leukemic phase is perhaps best defined as lymphocytic and probably represents a variant of plasma cell leukemia, in which the cells showed a degree of dedifferentiation from plasma cells to B lymphocytes. The possible relation between these 2 proliferative processes is discussed and the nature of leukemias developing in cases of plasma cell myeloma is briefly reviewed.

Correlation between thromboplastic activity and lecithin/sphingomyelin ratio in amniotic fluid preliminary report.

THe thromboplastic activity of amniotic fluid was correlated with the lecithin sphingomyelin (L/S) ratio in 59 pregnancies. It was shown that the thromboplastic activity and the L/S ratio in amniotic fluid had a coefficient of correlation of r = --0-73 (P less than 0-004). The thromboplastic activity was estimated by a modified method for prothrombin time, and a time of 113 seconds appeared to correspond to an L/S ratio of 2, in that it was the border line between maturity and immaturity of the fetal lungs. Thirty women were delivered within 24 hours of a sample of amniotic fluid being obtained. In the three patients whose babies developed the respiratory distress syndrome, the thromboplastic activity was estimated as being slower than 113 seconds.

Alveolar rhabdomyosarcoma presenting as subacute intravascular coagulation.

A patient with rhabdomyosarcoma is described in whom the presenting clinical and laboratory features were those of disseminated intravascular coagulation. The patient's rapid downhill course was primarily expressed by haemorrhagic tendency. An alveolar rhabdomyosarcoma, affecting many organs, including vascular and cardiac lumina, was found at necropsy and was considered to be the cause of the consumption coagulopathy.

Thromboplastic activity in amniotic fluid during pregnancy.

Thromboplastic activity of amniotic fluid (TAAF) was determined in 97 normal and pathologic pregnancies using a modification of Quick's one-stage method for prothrombin time. It was shown that amniotic fluid (AF) has thromboplastic activity. This activity was found to increase with the progression of pregnancy, showing a very high correlation coefficient (r = -0.86). In cases of pathologic pregnancies such as those associated with diabetes, toxemia, IUGR, and Rh incompatibility, the values of TAAF do not differ from normal pregnancies. However, in 7 of 10 cases of postmature pregnancies the TAAF was below 45 seconds while no preterm or term pregnancy showed TAAF of less than 45 seconds.

Relapsing haemolytic anaemia of pregnancy with negative antiglobulin reaction.

Four patients are described in whom seven episodes of haemolytic anaemia associated with pregnancy were observed during a follow-up period of 2 to 16 years. Normal haemoglobin levels were found both before and after pregnancy. Despite consistently negative antiglobulin tests, an autoimmune haemolytic mechanism is suggested by the rapid destruction of transfused blood, favourable response to corticosteroids and the development of transient haemolytic anaemia in three of the infants. In view of its unique clinical and laboratory features and relatively mild course, recognition of this condition as a distinct clinical entitity seems to be justified.

Erythrocytosis associated with multiple cutaneous leiomyomata: report of a case with demonstration of erythropoietic activity in the tumour.

A patient with leiomyomata of the skin and erythrocytosis is reported. After surgical removal of the larger skin tumours there was remission of the haematologic abnormalities and the erythrocytosis and the red cell mass decreased to normal levels. The raised serum erythropoietin levels were reduced but remained slightly elevated and less than those recorded prior to surgery. Tumour extracts contained significant erythropoietic activity. This seems to be the first documented case of erythrocytosis associated with skin leiomyomata.
