Influence of shock waves on fracture healing.

During the last decades the influence of physical factors on fracture healing has been widely described. With the use of shock waves for the treatment of urolithiasis, a new mechanical medium has been introduced into medicine. For the first time the influence of shock waves on fracture healing was studied in rats. With fractioned shock-wave treatment (5 times 100 shock waves at 14 or 18 kV) an enhancement in healing could be achieved.

[Treatment of type II B open fracture--therapeutic regimen and results].

A consecutive series of 119 type III B compound fractures in 103 patients was treated at the University of Louisville from May 1983 to May 1989. All patients had timely irrigation of their wounds, serial wound debridements, external skeletal stabilization and parenteral systemic antibiotics (penicillin, cefazolin, tobramycin). 96 open fractures were managed with the supplemental local use of tobramycin-PMMA-beads. There were 13.5% wound infections and 10.1% osteomyelitis observed. The additional local antibiotic therapy was of significant (p less than 0.001, p less than 0.025) benefit to lower both infectious complications. The amputation rate was overall 5%, 9.7% for the lower leg. Four patients died due to multiple trauma.

[What modifies the outcome of abdominal trauma? An analysis of 558 patients].

A consecutive series of 558 patients with abdominal trauma were treated at "Bergmannsheil" Trauma Center in Bochum between 1974 and 1989. The most common patient group was traffic casualties with an age of up to 30 years a total of 218 (31.1%) patients underwent surgery, in which can the diagnosis was made only by surgeons. Isolated injuries were predominantly in the spleen (33%), liver (29%), mesenterium (16.1%), bowel (14.2%), and bladder and urethra (10.1%). Eighty-three of the patients who were operated on had multiple intra-abdominal injuries; 208 (37.3%) patients were included with polytrauma. Despite the increasing number of patients with multiple injuries mortality decreased from 27.1% between 1974 and 1979 to 19.7% between 1980 and 1989. This emphasizes the progress that has been made in diagnostic methods, surgical intensive care and the improved education of trauma surgeons.

[Antibiotic-containing collagen sponge in therapy of osteitis].

In 1988, we treated 47 patients with acute or chronic osteitis by means of a new industrial collagenous sponge containing gentamicin (Sulmycin Implant) as an adjuvant measure. The local surgical management was standard: total excision of all avascular or infected soft tissue and bone, stabilization of pseudarthrosis, the improvement of local circulation and the condition of the soft tissue by means of local or free flaps before autologous bone grafting and antibiotics administered according to the test results. In cases of instability AO external fixation was used. One or two Sulmycin Implants (10 x 10 cm) containing 130 or 260 mg gentamicin were placed in the remaining small cavities or in the transplanted autologous spongy bone. Gentamicin was measured in the wound exsudate for 3-5 days; during this time we found bactericidal concentration in these exsudates. The wounds healed in 44 patients, but we had a persistent fistula in 3 patients (6.3%). We see collagen-gentamicin sponges as being very useful in the treatment of osteitis. It seems to reduce delayed bone transplantation in infected areas.

[Continuous decompression. A new method in the treatment of juvenile bone cysts].

Venous obstruction in the bone is the true cause of a simple bone cyst. Scaglietti combined decompression with the injection of a corticosteroid into the cyst cavity, while Chigira used a multiple drill-hole method. We perforate the cyst wall and decompress continuously with cannulated screws for 12 months. Consolidation of a cyst cavity has been uniformly observed 9-11 months after drilling. Six patients have been treated, with excellent results and without any recurrences in the follow-up period of 24 months. Since the introduction of this simple method, resection and grafting of bone cysts in young people has been not necessary.

[Myxoid schwannoma of the forearm. A case report].

Tumors arising from or located within a peripheral nerve are not common and recognition and treatment are difficult. We report a patient with a myxoid swannoma surrounding the axon of ulnar nerve. This 50-year old woman had pain and tenderness. Careful clinical and technical investigation--including CT and angiography--could not give a definite diagnosis preoperatively. The patient was cured by simple enucleation without damage to the axon bundles. At surgery the appearance of the tumor was typical. An oval, encapsulated mass measuring up to 5.5 by 9 centimeters in diameter was seen. Only biopsy and pathologic examination can exclude malignancy. After follow-up of 48 months no local recurrence of the tumor was found.

Evaluation of average life span of epithelial and stromal cells of human prostate by superoxide dismutase activity.

Little is known about the cell kinetics on which development of benign prostatic hyperplasia is based. This prompted us to study the superoxide dismutase (SOD) activity, which is known 1) to correlate with the life span of cells and 2) to decrease with advancing age of cells. Therefore, SOD was measured in epithelium and stroma of the human prostate from patients of various ages (20-86 years) and compared with the activity in the postmitotic skeletal muscle. It was found that the highest mean specific SOD activity is present in skeletal muscle (4.0 mU.mg protein-1), followed by the stroma (2.1 mU.mg protein-1) and epithelium (1.4 mU.mg protein-1). Similar results were obtained when SOD activity was expressed per DNA (5.03, 1.73, and 0.16 mU.micrograms DNA-1, respectively). Comparing the slope of the age-dependent regression lines, it was demonstrated that the slope of the stroma is much closer to the slope of the postmitotic skeletal muscle than the slope of the epithelium. From the data, it was calculated that the average life span of stromal cells is probably longer than 30 years and of epithelial cells longer than 2 years. Hence in human prostatic tissue the average cell death rate might be rather low.

[Concept of functional therapy exemplified by fresh closed humeral fracture].

The method of functional treatment of closed humerus fractures has gained an established position of success. A good outcome has proved to depend not only on correct indication, but it also depends decisively on physiotherapeutic exercises. The challenge to the physiotherapist has grown, and a standardised concept is now required. The authors' own approach is described in some detail. Excellent success was obtained from 80 in 82 cases, with average periods of treatment being shortened by something between two and four weeks.

[Fitness for automobile driving of physically handicapped patients].

Some 30.5 million Germans hold a driving license. For the person with a physical handicap, too, active traffic participation is of special importance, in that it constitutes an essential link in his rehabilitation. Road traffic authorities, in view of traffic safety, are held to consider both, the handicapped individual's interest in obtaining a license and society's claim to safety. Numerous studies have established the fact that disabled drivers, in particular those with impairment of the limbs, are neither more accident-prone nor otherwise more conspicious in road traffic than the non-disabled. The view that the driver with a physical disability poses particular risks, has been invalidated by extensive factual data and thorough literature review. Diseases of major relevance for driving ability are touched on, the range of adaptive equipment is exemplified. Physicians in accident surgery primarily have to deal with trauma to the spinal cord and limbs. The majority of these conditions permit good compensation by technical devices, enabling safe driving of a private car. Awareness of legal/administrative aspects of licensing, as well as of the adaptive potential of technical aids are of great importance to both physician and client.

[Is surgical treatment of fresh, closed humeral fractures justifiable as a routine procedure?].

Excellent results were found to be achievable from conservative functional treatment of upper arm fractures. A critical appraisal is made in this paper of the authors' own cases and of literature published over the past 25 years for comparison of the above results with surgical therapy. The conservative functional approach was found to be the optional method to cope with closed upper arm fractures, since it provides all the advantages of surgical therapy, yet, without the risk implied in surgery. Publications on improved results from surgical therapy cannot be accepted as reasons for a general expansion of indications.

[Functional treatment of fractures of the upper and lower extremities. Results of 3 years' experience].

The use of plastic boxes in functional treatment of bone fractures has satisfactorily helped to speed up non-invasive reintegration of patients to their normal environment. The time of treatment can thus be considerably reduced. Correct indication, short-interval follow-up checks in the initial phase, and the patient's cooperative attitude are essential to good success. Brace treatment of upper arm fractures, ulnar fractures, and closed tibial fractures have been three of many indications suggested by Sarmiento which have proved to yield good results at "Bergmannsheil Bochum".
