[Pediatric management of early-onset neonatal sepsis: guidelines adherence in Lille's perinatal care network].

UNLABELLED: Neonatal early-onset infection has been recognized as a significant cause of perinatal mortality and morbidity, in spite of recent improvement in perinatal care. In France, Anaes guidelines were published in 2002. OBJECTIVES: To evaluate paediatricians' knowledge about these official guidelines in Lille's perinatal network. MATERIALS AND METHODS: Every network's paediatrician, working in a hospital maternity or in a neonatology unit received a questionnaire, to evaluate his knowledge about early onset sepsis diagnosis, indication and type of per partum antibiotherapy, and immediate neonatal care, according to perinatal factors. RESULTS: Response rate was 55%. Thirty-five per cent of paediatricians underestimated gastric sample's diagnostic role and 41% prescribed a triple antibiotherapy. Duration of antibiotic treatment was too long in 56% of cases. Biologic tests were prescribed in excess in 71% of cases when a streptoccocal B mother's colonization was present. Per partum antibiotherapy was appropriated in only 29% of responses. CONCLUSION: This study underlines imperfect official guidelines' knowledge, leading to ecological, bacterial, and economical consequences.

[Birthweight curves: a review of the literature].

There are many birthweight reference curves in the literature. This study reviews birthweight curves that are commonly used in France as well as references that provide birthweight by sex. The 19 curves selected for review were published between 1971 and 2001 using samples of between 300 to 3 million newborns in Europe, Australia and North America. Our objective was to summarize their main characteristics and to provide guidelines for analysis by explaining which factors are responsible for differences between curves, and discussing the methods used to construct them. The characteristics of the population must be taken into account and study samples must be recent and representative. The sample size must be sufficient to ensure stable estimates for premature infants. It is preferable to use different curves for boys and girls because sex is a key determinant of birth weight. The choice of appropriate birth weight reference remains difficult in some countries, in France particularly, and the use of individual customised birth weight curves provides an interesting alternative.

Value of human papillomavirus testing after conization by loop electrosurgical excision for high-grade squamous intraepithelial lesions.

OBJECTIVE: The aim of the study was to evaluate human papillomavirus (HPV) testing during the follow-up of patients after conization by loop electrosurgical excision for high-grade squamous intraepithelial lesion. METHODS: A prospective study was conducted on 205 patients who underwent conization for high-grade squamous intraepithelial lesion (CIN 2 or 3). Loop electrosurgical excision procedure (LEEP) was used in all cases. High-risk HPV testing was realized by the Hybrid Capture II system before and 3 months after conization. RESULTS: Of the 205 patients, 193 (94.1%) were positive for the HPV test before conization. Seventy-one were HPV positive after conization (34.6%). The margins were positive in 36.1%. Residual disease was observed in 27 cases (13.2%). Four patients (2%) developed a recurrence after a mean follow-up of 18.1 months (+/-12). There was no correlation between pretreatment HPV testing and the residual disease or recurrence. Patients with positive margins were significantly more likely to have residual disease than those with negative margins (P < 0.0001). Residual disease was more likely to occur when the posttreatment HPV test was positive (P < 10(-7)). All recurrences were observed in patients with a positive posttreatment HPV test (P < 0.05). Residual disease and recurrence were correctly predicted with a sensitivity of 81 and 100%, respectively, and a negative predictive value of 96 and 100%. CONCLUSION: Posttreatment HPV testing could be useful in the follow-up of patients after conization. In case of negative posttreatment HPV testing, the frequency of follow-up could be reduced, particularly in those patients with free margins.

[Premature discontinuation of breastfeeding].

UNLABELLED: This study was performed to describe the factors associated with a premature cessation of breastfeeding. BACKGROUND AND METHODS: Three hundred and forty nine women with a plan for prolonged breastfeeding over two months were interviewed during their hospital stay to define sociological and medical variables. One month after discharge, mothers were contacted by a physician for making out evolution of the breastfeeding. RESULTS: Eighty-eight (28%) women were no more breastfeeding their infant and were compared to the other 224 (64%) mothers. The main sociological factors influencing the cessation of breastfeeding were low education level (odds ratio 4.2 [95% CI 2.2-8.3]), and primiparity (odds ratio 2.1 [95% CI 1.1-3.9]). Pacifier use was associated with a lower duration of breastfeeding (odds ratio 2.5 [95% CI 1.4-4.5]). Medical predictive factors were infant weight loss (odds ratio 4.3 [95% CI 1.7-10.6]), respect of intervals between feeding times (odds ratio 2.1 [95% CI 1.1-4.4]), delay before the first feeding (odds ratio 1.9 [95% CI 1.0-3.6]). On the other hand formula supplementation was not related with cessation of breastfeeding. DISCUSSION AND CONCLUSION: As the success of breastfeeding depends on maternal motivation, it seems essential to take into account mothers' plan to study breastfeeding duration. The knowledge of predictive factors of the cessation of breastfeeding should allow to identify mothers who need particularly the attention of healthcare professionals.

[Mechanical properties of biological or synthetic implants used to treat genital prolapse and stress incontinence in women : what is the ideal material?].

INTRODUCTION: Many surgical techniques proposed for genital prolapse or stress incontinence use prosthetic material to reinforce native tissues. Most of the synthetic meshes used have been designed for hernia repair. MATERIAL AND METHOD: We study the biomechanical properties of human Alloderm or animals tissues like Pelvicol and of synthetic resorbable and permanent meshes. We report the results from the literature. We report the results of a personal study of the biomechanical properties of synthetic meshes. RESULTS: The literature on biomechanical properties of biological or synthetic meshes and their evolution after implantation is sparse. Biogyn ITY or Prolène are the only meshes without spatial orientation. Their resistance to rupture and mechanical properties are variable and seem poor for Biogyn W8 et Mersuture. DISCUSSION: Reviewing the literature we discuss the ideal properties for synthetic meshes used for cure of genital prolapse.

[Biomechanics of stress distribution and resistance of biological tissues: why use prostheses for the treatment of genital prolapse?].

BACKGROUND: Solidity and elasticity are the two main biomechanical properties of pelvic tissues involved in surgical cure of genital prolapse-prevertebral, pectinate, and sacrospinal ligaments, tendinous arcs of the pelvic fascia, vaginal tissue. We report data in the literature and personal studies concerning these autologous biological tissues. MATERIAL AND METHODS: The resistance of pelvic tissues was tested on 29 cadavers. Measurements were also made on two 2-cm samples of vaginal tissue obtained during vaginal route surgery for prolapse cure in 20 menopaused women. Stress tests were conducted to determine resistance and level of rupture. RESULTS: There was a wide variability in ligament resistance, ranging from a minimum of 22 Newtons to a maximum to the order of 200 Newtons. Results varied greatly from one woman to another and also between the two sides in the same woman. The prevertebral ligament exhibited the greatest resistance. The pectinate ligament was significantly more resistant than the sacrospinal ligaments and the tendinous arcs of the pelvic fascia. There was a significant relationship between the subjective assessment of ligament quality and objective measurements of resistance. For vaginal tissues, resistance varied greatly from 12 Newtons to a maximum to the order of 76 Newtons. Flexion values ranged from 14 to 130 Newtons. CONCLUSION: Our findings illustrate pelvic tissue failure observed in patients with genital prolapse. Individual maximal resistance of the pelvic ligaments is vary variable, between ligaments and between subjects, and even between sides in a given subject. Pelvic ligaments used for cure of genital prolapse are moderately resistant with wide interindividual variability. The mechanical properties of vaginal tissue are also very variable, illustrating why these tissues may exhibit a certain resistance against dissociation when exposed to loading but much less resistance when exposed to traction by a surgical suture. These findings suggest a revision of classical surgical procedures.

[Comparison of fertility rates after ectopic pregnancy in Auvergne and Lille regions].

OBJECTIVES: Fertility rates after ectopic pregnancy (EP) were compared between the Auvergne region (central France) and the Lille region (northern France) in order to determine whether fertility factors are similar. We also wanted to determine whether regional differences persist if known fertility factors are taken into account. MATERIAL AND METHODS: Data from the Auvergne EP register 1992-1998 (1.285 women, among whom 552 tried to become pregnant again) and from Lille register 1994-1997 (678 women, among whom 343 tried to become pregnant again) were analyzed. Fertility was characterized by the time to a new pregnancy and its outcome (EP recurrence, intrauterine pregnancy (IUP)). Censored data methodology was used. RESULTS: Despite differences in general fertility characteristics between regions, fertility factors were the same with similar relative risks for age, history of infertility, previous tubal damage, use of an intrauterine device at the time of EP and EP treatment. When these factors were taken into account, fertility rate after EP remained significantly better in Auvergne than in the Lille region. CONCLUSION: This common analysis of data from two registers confirms, from a quantitative point of view, the role of the main factors associated with fertility after EP. However, other factors such as EP management or the surgical technique may intervene.

Neurological assessment at five years of age in infants born preterm.

AIM: To evaluate the agreement between Touwen's neurological examination and a derived simplified one, created to be applied at the age of 5 y to infants born preterm. METHODS: 185 children born at a gestational age (GA) of <33 wk and/or with a birthweight <1501 g, free of cerebral palsy, underwent Touwen's neurological examination at a mean age of 5 y and 8 mo (5-6.5 y). One-hundred and seventy had a full examination and were included into the study. They were born at a mean GA of 30 wk (range 24-35 wk) with a mean birthweight of 1250 g (range 600-2690 g). A simplified examination, based on the clinical experience of two of the authors, was created a priori. The data were reviewed retrospectively and the concordance between the two forms was assessed. The reviewers were blinded to the original categorization from the long form. RESULTS: On the basis of the original Touwen's neurological examination, the 170 children were classified into 122 with a normal neurological examination, 41 with grade 1 minor neurological dysfunction (MND) and 7 with grade 2 MND, giving 28% of the cohort with MND. The concordance between the two forms was excellent, with an agreement in 169 out of 170 subjects. None of the infants with the most complex form of MND was misclassified. CONCLUSION: The simplicity of this examination could allow its diffusion and its use in follow-up programmes. It could improve the quality of routinely collected follow-up data of preterm neonates.

[Fertility after ectopic pregnancy: the population-based register of the urban area around Lille, Northern France].

The aims of this work were the evaluation of the reproductive outcome after ectopic pregnancy, and the assessment of the role of infertility risk factors and treatment's strategy. All patients in the population-based register of the urban area around Lille, Northern France, were followed-up. 345 women treated between April 1994 and March 1997, who were trying to become pregnant were interviewed by telephone every 6 months and then every year. The cumulative pregnancy rates were calculated by the Kaplan-Meier estimation. Associations between infertility risk factors and intrauterine pregnancy were tested by the logrank test, and by a Cox model for multivariate analysis. The mean duration of follow-up was 22 months, and 228 (66%) women had obtained a new pregnancy at the time of the analysis. 23 (10%) of the first pregnancies were recurrences. For women for whom EP occurred with an IUCD (17 patients), the 1 year intrauterine pregnancy (IUP) reached 67%. For the others, the 1 year IUP rate was 56%, and reached 67% after 2 years. After adjusting factors associated with fertility with a Cox regression, 3 factors seemed to lower reproductive performances: age > 35 years, previous history of infertility, and anterior tubal damage. More than half the women treated for EP obtained spontaneously a normally progressive pregnancy after 1 year. Reproductive performances are associated with characteristics of the patients, but do not depend on radical or conservative treatment.

Survival analysis of fertility after ectopic pregnancy.

OBJECTIVE: To evaluate the reproductive outcome after ectopic pregnancy and to assess the contribution of risk factors to future fertility. DESIGN: Prospective follow-up in a population-based sample. SETTING: Register of ectopic pregnancies established in an urban area around Lille, France. PATIENT(S): Three hundred and twenty-eight women treated between April 1994 and March 1997 who had not been using an IUCD at the time of the ectopic pregnancy and were trying to become pregnant. INTERVENTION(S): Interviews by telephone every 6 months for 2 years and once yearly thereafter. MAIN OUTCOME MEASURE(S): Cumulative pregnancy rate. RESULT(S): Two hundred fifteen (65.5%) women became pregnant after a mean of 5 months. One hundred eighty-two (84.7%) pregnancies were intrauterine; 22 (10.2%) were recurrent ectopic pregnancies; and in 11 women (5.1%), it was too early to define implantation. The cumulative intrauterine pregnancy rate was 56% at 1 year and 67% at 2 years. After applying Cox regression, three factors associated with fertility seemed to decrease reproductive performance: age > 35 years, history of infertility, and anterior tubal damage . CONCLUSION(S): More than half of the women treated for ectopic pregnancy spontaneously conceived and had a normally progressive pregnancy at 1 year. Fertility depends more on established patient characteristics than characteristics of ectopic pregnancy itself or treatment thereof.

Patient satisfaction with management of ectopic pregnancy.

OBJECTIVES: This study was designed (1) to identify characteristics of patients and their treatment and aspects of care that may be associated with satisfaction, and (2) to investigate the way these factors contribute to the degree of satisfaction expressed after treatment of ectopic pregnancy. Our ultimate goal in so doing was to determine how to improve ectopic pregnancy management. STUDY DESIGN: We used data from a register of ectopic pregnancy established in an urban area (Communauté Urbaine de Lille) in northern France. Two months after ectopic pregnancy, a self-administered satisfaction questionnaire was completed by 192 women treated between January 1995 and July 1996. We searched which factors were associated with satisfaction by a logistic regression model. RESULTS: Greater satisfaction was significantly associated with average education. Women were more satisfied when pregnancy was an expected event, and when fallopian tube was unruptured. Patients satisfied with the way they were received, the comfort, the willingness to listen and provide explanations, were more satisfied overall. Physical pain, anxiety for later fertility, and need of psychological support were associated with lesser degrees of satisfaction. When adjusted for these factors, the following factors remained significant: level of education; conditions of conception; pain; reception and willingness to listen. CONCLUSION: This study should increase awareness amongst health care professionals of the importance of a compassionate approach when dealing with patients following an ectopic pregnancy, since patients value this aspect of care.

[Rehydration modalities for acute diarrhea in hospitalized infants. Impact of a permanent short-stay pediatric observation unit].

OBJECTIVE: To analyze hospital treatment of infants with acute gastroenteritis, especially the variations in modes of rehydration, hospitalization rates and lengths of stay, related to paediatrician permanency with a short-stay observation unit at their disposal. METHODS: This prospective multicentric study during two rotavirus epidemic periods included 1,187 infants (mean age 10 +/- 6 months) with acute gastroenteritis resulting or not in dehydration. During the first period, 12 volunteer hospitals were included. During the second period, we compared six (among the 12) hospitals where infants were admitted in emergency care or pediatric units without pediatric 24-h permanency, with a pediatric emergency care unit having pediatric permanency with a short-stay observation unit. RESULTS: During the first period, 32% were dehydrated and 6% had hypovolemic shock on admission. The hospitalization rate was 81%. Only 19% of dehydrated infants had exclusively oral rehydration, and 19% of normohydrated infants had intravenous rehydration. During the second period, the hospitalization rate was 42% in the pediatric emergency care unit (28% in the short-stay unit and 14% in hospitalization units) versus 77% in the six hospitals. In the pediatric emergency care unit as compared to the six hospitals, there were a reduction of 65% of hospitalization days, 58% of intravenous accesses, and 66% of laboratory tests. CONCLUSION: Our findings suggest that a paediatric permanency with a short-stay observation unit at their disposal improves quality of treatment and is cost-effective.

[Abdominal colposacroplexy for the treatment of vaginal vault prolapse with or without urinary stress incontinence].

OBJECTIVE: The aim of this study was to assess the factors of success in abdominal colposacropexy (CSP) procedures. PATIENTS AND METHODS: We performed 271 consecutive CSP between 1986 and 1997 (mean age: 48.8 years +/- 11.1). We reviewed 217 patients (80.1%). Mean duration of follow-up was 5.5 years (1-136 months). We performed: 18 CSP with Goretex mesh, 3 with resorbable mesh and 196 with Mersilene; 179 CSP with posterior colporraphy and 38 without; 208 CSP with culdoplasty (Moschowitz's procedure) and 9 without; 182 CSP with anterior and posterior meshes, 26 with posterior mesh only and 9 with anterior mesh only. RESULTS: 97.7% (212/217) of patients were cured for prolapse. 58% (125/217) had urinary stress incontinence totally cured and 82% (178/217) had urinary stress incontinence improved. Rejected grafts were 16.7% (3/18) with Goretex mesh and 1.1% with Mersilene mesh (p = 0.004). Recurrent prolapses were 1.1% (2/196) with CSP with posterior colporrhaphy and 7.9% (3/38) in CSP without (p = 0.009; OR = 0.14, CI = 0.02-0.86); 4/208 with CSP with culdoplasty and 1/9 with CSP without (p = 0.04; OR = 0.17, CI = 0.02-1.58). Recurrent stress incontinence was observed in 4/9 cases with CSP with anterior mesh only and 28/182 with CSP with anterior and posterior meshes (p = 0.03; OR = 0.34, CI = 0.12-0.97). CONCLUSION: CSP must use anterior and posterior Mersilene mesh. The CSP must be systematically combined with posterior colporraphy and culdoplasty (Moschcowitz's procedure).

[Mechanical resistance of pelvic ligaments used for incontinence or prolapse surgery].

OBJECTIVE: Underline the objective strength of the pelvic ligaments. PATIENTS AND METHODS: Twenty nine human female pelvis cadavers, whose storage conditions differed, were used in our study. In each cadaver we dissected all the ligaments used in pelvic surgery. A subjective clinical evaluation of the ligament properties was performed by visual observation as well as by finger palpation. Ligaments were classified into three groups in terms of thickness and apparent strength following finger palpation, high, doubtful and low apparent quality ligaments. Then a suture taking the entire ligament switched the ligaments and a force was applied on the vagina axis until tearing. The device used for strength measurement during traction was a Samson type force gauge, which was developed for the purpose of our study. Results were given in Newtons. RESULTS: We found a great variability in the values obtained at tearing with maximal values at 200 newtons and minimal at 22. Individually measured, ligament strength varied between individuals, and for a same patient between the type of ligaments and the side. The pre-vertebral ligament was on average the strongest. For bilateral ligaments, there was no difference between the left and right side. The iliopectineal ligament was statistically significantly stronger than sacrospinous and arcus tendineus of pelvic fascia. There was a correlation between subjective evaluation and objective strength measurements. DISCUSSION AND CONCLUSION: We performed the only study of the strength of pelvic ligaments at tearing. These are, however, routinely used in the cure of prolapse and urinary incontinence. Our results show that there is a great variability in strength between individuals, and for a same patient between the types of ligaments and side. These observations could explain some of the surgical intervention failures and demonstrate the importance of per-operative strength evaluation. Per-operative subjective evaluation on strength is related to objective measurements and could be used to determine the type of ligaments to be used for surgical assembly suspension.
