The mechanical role of the digital fibrous sheath: application to reconstructive surgery of the flexor tendons.

The movements of flexion of the digital articular chain are dependent upon a vast functional complex comprising the long flexor tendons, their antagonist long extensor tendons, and the coupled action of the interosseous and lumbricales muscles. Based on theoretical biomechanical data backed up by experimental investigation, the authors emphasize the important role of the digital fibrous sheath in the mechanism of action of the long flexor tendon. Any proximal resection (even of minor degree) of the proximal pulley of the digit induces severe mechanical disturbances by enhancing flexion of the metacarpophalangeal joint to the detriment of the proximal interphalangeal joint. Conversely, distal resection of the pulley enhances flexion of the interphalangeal joint to the detriment of the metacarpophalangeal articulation, although the resulting abnormalities are less pronounced than those due to proximal resection. The fibrous pulley plays a role in tendinous orientation in the frontal plane and in digital orientation of the index finger where the angulation between the pulley and flexor tendon is most pronounced. In this latter case ulnar deviation of the second finger occurs on flexion. Reconstructive surgery of the long flexor tendons requires that the digital fibrous sheath, especially its proximal segment, be preserved. The metacarpophalangeal joint seems to be the most vulnerable to morphological modifications of the pulley, as stressed from the anatomical standpoint by the absence of insertion of the long flexor tendons on the proximal phalanx.

The triple classification of dry eye for practical clinical use.

PURPOSE: "Dry Eye is a condition produced by the inadequate interrelation between lacrimal film and ocular surface epithelium, and is caused by quantitative and qualitative deficits in one or both of them. It can be produced by one or combined etiologic causes, affecting one or several of the secretions of the glands serving the ocular surface, and producing secondary manifestations of different grades of severity". Clinicians need a practical classification to face diagnosis, prognosis and treatment. Dry eyes have many etiologies and pathogenesis, different affectation of the various dacryoglands and ocular surface epithelium, and diverse grades of severity. The specialists in xero-dacryology must know these three parameters to evaluate any case of dry eye, and to establish an adequate treatment. METHODS: To facilitate this, an open session in the 8th congress of the International Society of Dacryology and Dry Eye (Madrid, April, 2005) proposed modifying the Triple Classification of dry eye approved in the XIV congress of the European Society of Ophthalmology (Madrid, June, 2003). There was consensus of all conclusions. CONCLUSIONS: The following classification has been established: First, a classification of the etio-pathogenesis, distributed in ten groups: age-related, hormonal, pharmacologic, immunopathic, hyponutritional, dysgenic, infectious/inflammatory, traumatic, neurologic and tantalic. Second, a classification of the affected glands and tissues, which under the acronym of ALMEN includes the Aqueo-serousdeficient, Lipodeficient, Mucindeficient and Epitheliopatic dry eyes, and the Non dacryological affected exocrine glands (saliva, nasal secretion, tracheo-pharyngeal secretion, etc). And thirdly, a classification of severity, in three grades: Grade 1 or mild (symptoms without slitlamp signs), grade 2 or moderate (symptoms with reversible signs), and grade 3 or severe (symptoms with permanent signs).

[Neonatal dacryocystitis. A case report].

The authors report one case of neonatal dacryocystitis. They consider the diagnostic and therapeutic problems. One surgical intervention, with a large orifice of the lacrimal meatus, upon the catheter, with rhinoscopy, seems them the best solution, in this precise case.

[Macular manifestations of acute lymphoblastic leukemia. A case report].

The authors report a case of ocular involvement by acute lymphoblastic leukemia, in a young woman. The initial symptom was a bilateral diminution of visual acuity. The examination revealed a bilateral aspect of serous detachment of neuro-epithelium. A literature review of macular manifestations of leukosis is envisaged.

[Arterial vascularization and innervation of the lacrimal gland. Apropos of 100 dissections].

The authors report the different types of arterial vascularization and lacrimal gland innervation, noticed in 100 human orbits with arterial latex injected. The lacrimal artery may be unique, arising either from the ophtalmic artery, the most frequent case (vascularization of type I) or from the external carotid system (vascularization of type II): it is possible that 2 lacrimal arteries are simultaneously present, originating from the 2 carotid system (vascularization of type III). The lacrimal gland is innerved by the lacrimal nerve and by one or several nervous branches, originated from the maxillary nerve. 3 possibilities can be found: a zigomatic branch from the maxillary nerve makes a preglandular anastomosis with the lacrimal nerve; a real lacrimal loop, which may form either an intraglandular anastomosis or be completely independant of the lacrimal nerve. Sometimes, several zigomatic rami do exist. These different variations and their frequency are analyzed and referred to the literature.

[Macroscopic aspects of arterial vascularization of the orbit].

The authors report the result of 80 orbital dissections, after injection with latex of the arterial system. They insist upon the course of the ophthalmic artery, usually found, upon his possible variations: specially under-crossing of the optic nerve, absence of an extra-conical passage. After all, the authors describe the principal branches of the artery.
