Hyperkalaemia secondary to hypo-aldosteronism. A report of 2 cases differentiating hyperreninaemic from hyporeninaemic hypo-aldosteronism.

Hypo-aldosteronism has been considered an important pathophysiological mechanism in the development of hyperkalaemia. This may occur as a consequence of defective regulatory mechanisms in the plasma renin-angiotensin-aldosterone system. Two case reports of hyperkalaemia are described which illustrate the differentiation of hyperreninaemic from hyporeninaemic hypo-aldosteronism. Laboratory investigations, together with measurements of plasma renin activity and aldosterone concentrations during adrenocorticotrophic hormone stimulation and sodium depletion, demonstrated this difference. Appropriate therapy with fludrocortisone (Florinef; Squibb) and furosemide (Lasix; Hoechst) respectively returned the situation to normal.

Doctors have always been involved in politics, as individuals or on a national or international level. Some have deserted medicine in order to pursue a full-time political career. A selection of doctor-politicians who have appeared on stamps is presented.

Serum lipid levels in black diabetics at Baragwanath Hospital: our concepts of coronary heart disease probed.

Cholesterol, triglyceride and phospholipid levels were measured in the serum of fasting adult diabetics who regularly attended Baragwanath Hospital, Johannesburg. A total of 149 subjects were studied (69 males and 80 females). Correlation coefficients were determined between the lipid levels and between additional variables of age, obesity and duration of diabetes. Significant correlations were found between cholesterol and phospholipid levels in males and females and between cholesterol and triglyceride levels in males only. Of interest is the lack of correlation between the degree of obesity and any lipid level. A notable finding was that the cholesterol concentration was bimodally distributed in the males and possibly in the females. About half the subjects had high serum lipid levels, apparently for the duration of their diabetes. In addition, this population showed a large number of other factors associated with a significant risk of developing coronary heart disease (CHD); yet, this disease was exceedingly rare in the population. It is suggested that there is some undefined factor(s) which is powerfully protective against CHD in Blacks.

Medical philately: The pen and the scalpel -- the story of medicine and literature.

The medical profession has always been prominent in the cultural life of the community. Many doctors have been able to carry out their professional duties as well as actively to pursue their outside interests. Some have abandoned their medicine altogether -- they have been called 'medical truants'. Literature and medicine have always been closely associated, and a number of literary doctors who have appeared on stamps are presented in this article.

Epidemic listeriosis. Report of 14 cases detected in 9 months.

During the period August 1977 to April 1978, 14 Black patients in Johannesburg area had systemic infections caused by Listeria monocytogenes. Nine of these patients were neonates who presented with septicaemia (5 cases) or septicaemia and meningitis (4 cases) and 5 were adults, all of whom had meningitis. The mortality rate was 43% (6/14), with 4 neonatal and 2 adult deaths. All isolates of L. monocytogenes were type 4b. Only sporadic cases of human listeriosis have previously been reported in South Africa, and the cases reported here constitute the first epidemic in this country.
