[A case of tumor of the pineal body treated with gamma rays of Co-60].

A 21-year-old patients was admitted to hospital with symptoms suggesting a tumour in the area of the epiphysis. Computer tomography confirmed the initial diagnosis. Since signs were present of increased intracranial pressure a Pudenz valve was implanted and in the second stage of treatment radiotherapy with radioactive cobalt was applied. A very good result was obtained with control CT failing to demonstrate presence of the tumour and hydrocephalus.

[Effect of Coretal Polfa (Oxprenolol) on parkinsonian tremor and benign essential tremor].

Coretal -- an agent blocking the beta-adrenergic receptors -- was given to 12 patients with Parkinson's syndrome with evident tremor and to 4 patients with benign essential tremor. In the group of Parkinson's syndrome the effectiveness of this treatment was evaluated by the blind method. The whole period of observation was 6 weeks, during 3 weeks the patients received Coretal 60-120 mg daily, during the next 3 weeks they were given placebo. The intensity of tremor was assessed by means of a scoring system. Improvement was achieved in 7 patients with Parkinson's syndrome and in 3 out of 4 cases of idiopathic tremor. Complete disappearance of tremor was never observed. Transient side effects were observed in 2 causes. The authors think that Coretal may be used with good result in treatment of parkinsonian tremor and benign essential tremor.

[Clinical pattern of epilepsy with abortive temporal lobe attacks].

The purpose of the present work was to evaluate the clinical course of epilepsy with infrequent partial attacks with complex manifestations derived from the temporal lobe. Thirty patients aged 14 to 64 years treated on an outpatient basis and discovered during epidemiological investigations were followed-up. In half the cases only attacks without generalization occurred, in the remaining cases isolated generalized seizures appeared during many years of disease duration. In both groups a decrease was observed in the frequency of seizures in patients treated systematically or irregularly, as well as in those who had never been treated. Despite a long duration of the disease (above 6 years in 24 cases) the patients had normal mental level and good social adaptation.

[Study of children of mothers treated with anticonvulsants during pregnancy].

Forty children of mothers with epilepsy treated with anticonvulsants during pregnancy were investigated. Medical documentation and history data permitted to gain information concerning the course of the disease and treatment during pregnancy, the course of pregnancy and labour and the neonatal condition of the child. The children had medical, psychological and EEG investigations. It was found that in the group studied disturbances in the course of pregnancy occurred much more frequently than in the general population (pregnancy disturbances in 20.5%, premature labour -- 25.6%, complicated labour -- 15.4%). Numerous so-called minor malformation syndromes were found in these children (37.5%) and somatic development was deficient in 52.5% of cases. EEG abnormalities were present in 65.6% of children. No abnormalities were observed in mental development. It seems that these abnormalities were most frequent when the mothers had been treated with derivatives of phenytoin and phenobarbital. The observed group is, however, too small for reaching reliable conclusions.

[Importance of catheterization of inferior petrous sinuses in surgical treatment of Cushing's in children and adolescents].

The authors present 12 cases of Cushing disease in children and adolescents. In childhood the course of this disease differs from that in adulthood. The predominating initial signs in children are growth stunting and arrest of sexual maturation. Radiological methods (sella X-ray, CT, NMR) are normal initially in most cases and the diagnosis is confirmed exclusively by hormonal investigations. ACTH determination of venous blood samples obtained by catheterization of the inferior petrous sinuses is essential not only for the diagnosis of pituitary adenoma but also for choice of proper surgical treatment. The optimal method is selective removal of the adenoma. In case of impossibility of this removal (anterior lobe hyperplasia) hemiphysectomy is suggested (with removal of the lateral part of the anterior lobe on the side of greater secretion of ACTH into the inferior petrous sinus. This method is possible only if before the operation catheterization of these sinuses was done and a significant difference of ACTH level was found between the right and the left sinus. This is the reason why this test is essential for selection of proper treatment. Anterior lobe hypophysectomy, bilateral adrenalectomy, X-ray hypophysis irradiation are unacceptable for children because of their maiming character making impossible further somatic development and sexual maturation of children.

[Posttraumatic carotid-cavernous fistula: diagnosis and treatment--exemplified by a patient with a fistula following a blunt head injury].

Carotid-cavernous fistula is a rare posttraumatic complication. The authors described the patient in which a blunt cranial injury caused the carotid-cavernous fistula. In diagnosing the fistula Doppler examination proved very helpful. It showed the vein-arterial flow in the ophthalmic artery on the fistula side. The arteriography of the common carotid artery fully confirmed the diagnosis. Successful treatment was achieved by means of percutaneous endovascular balloonization of the fistula ring.

[Evaluation of perceptive activity in persons with low pressure hydrocephalus treated surgically. Preliminary report].

Preliminary assessment of perceptive activity was carried out in patients with low-pressure hydrocephalus diagnosed clinically. The purpose of the study was establishing of the valuable tests for the neuropsychological diagnosis making possible finding of perceptive processes most frequently disturbed in low-pressure hydrocephalus. The studied group comprised 53 patients operated on for this hydrocephalus. In only 9 cases the condition of the patient made possible a detailed study of the perceptive processes. In this group the tests were carried out three times: before, immediately and after a longer time after insertion of ventriculoperitoneal shunt. The results showed differences in the level of general perceptive functioning in the whole group. In the subgroup studied in detail no general perception deterioration was found, and the usual disturbances were noted in the memorization of new information and in visuospatial orientation. Late after the operation the greatest improvement was observed in the most disturbed functions.

[The use of iodinated contrast agents in neuroradiology].

The authors describe iodinated contrast materials, which are commonly used in neuroradiological applications. The adverse reactions after their, administration are discussed. The indications for safer, non-ionic contrast media are presented. The protocol of prophylactic treatment of high risk patients is also formulated. We present the contrast material protocol used at our institution for neuroradiology.
