[Treatment of parkinsonism with L-dopa and peripheral decarboxylase inhibitor].

In 38 patients with Parkinson's syndrome Madopar preparation was used (L-dopa with peripheral decarboxylase inhibitor) in 33 cases as the main drug and in 5 cases as an addition to L-dopa. In the group of 33 patients 39 could complete the treatment, one patient died suddenly, three had the treatment withdrawn in view of side effects. The effectiveness of Madopar was assessed by means of five-rate scoring systems NUDS and ART. Clinical improvement was found in 22 cases (about 67%). The improvement included mainly bradykinesia and rigidity, while tremor was only slightly improved. Side effects developed in about 40% of patients and were slight and transient (apart from 3 cases). The main contraindications seem to be psychotic disturbances. In the group of 5 cases treated with Madopar as an additional drug in low doses improved the result of long-term treatment with L-dopa.

[Selective mechanical dilatation of brain arteries: intracranial angioplasty].

Intracranial vascular stenosis of haemodynamic significance is underappreciated both clinically and diagnostically. It is thought to be the cause of cerebral ischaemia in 8% of patients. Now it is possible to perform dilatation of the intracranial arterial stenosis by modern intraarterial angioplasty. Two cases are presented of successful dilatation of middle cerebral artery trunk stenosis. Diagnostic problems, follow up and pharmacological treatment as well as technique of intravascular treatment is described.

[Embolization of the branches of the external carotid artery].

Authors present their own experience with the embolization of the external carotid artery branches. The main indications to embolization included well vascularized and hemorrhagic brain tumors in facial part of the skull, mainly meningiomas, juvenile angiofibromas, and angioneuromyomas. Embolization of external carotid artery was performed in 15 patients. Complete impatience of blood vessel supplying the tumor was achieved in 12 cases, but it was incomplete in 3 cases. Single serious complication in the form of hemiplegia was noted. There were also mild complications in 5 patients, which did not require any intervention.

[Hydrocephalus as a sequel of hemorrhage from ruptured cerebral aneurysm in light of CT investigations].

In the years 1985-1987 CT studies were carried out in 182 patients after haemorrhage from ruptured intracranial aneurysm. The study was carried out before surgery or the patients were not operated on for various reasons. Hydrocephalus was found in 33 cases, that is 18.2%. The patients were divided into subgroups depending on the time of CT after haemorrhage and on the state according the Hunt-Hess classification. Since only a small per cent of cases fell into the I and II categories of the classification, one may state that posthaemorrhagic hydrocephalus is connected usually with worse condition of the patient. Early hydrocephalus was relatively rare. In only a part of the cases CT was done on the first day after haemorrhage and it was difficult to say whether the presence and amount of blood in the basal cisterns and over the convexity contributed significantly to the development of hydrocephalus. However, in half the cases with intraventricular bleeding hydrocephalus was present.

[Spasm in angiographic image and the development of the symptoms of ischemic stroke in patients after subarachnoid hemorrhage from supratentorial ruptured aneurysm].

The results of angiographic examinations are presented in relation to the development of ischaemic stroke in 219 patients. In the selection of the patients cases were ruled out with coexistent intracranial haematomas, and the patients treated surgically were ruled out in whom the course of the operation suggested a possible influence of additional factors which might have changed the condition of the patient independently of the main disease. Depending on the results of angiography three groups of patients were isolated with different likelihood of ischaemic stroke development: patients without vasospasm or with segmental vasospasm-stroke developed in 15%-9% of cases; patients with unilateral or bilateral diffuse vasospasm of low or medium intensity-stroke in 34%; patients with high-grade diffuse intensity-stroke in 73% of cases. The frequency of stroke in these groups is analysed in relation to the condition of the patient at the time of angiographic examination.

[The risk of complications of cerebral angiography in patients with cerebral aneurysms in relation to the time of its use].

The authors analysed the effect of cerebral angiography on the condition of the patients after haemorrhage from an intracranial aneurysm. The studied material comprised 242 patients in whom cerebral angiography was carried out at various times after the haemorrhage by the method of direct percutaneous needle insertion into the carotid artery. Complications of angiography included: worsening of the general condition or neurological condition during the angiography and within 24 hours after it. Such complications were found in 14 patients (5.8%). In only 3 cases they appeared during the angiography. No cases of repeated haemorrhage from the aneurysm were observed. The greatest risk of complications was after 7-9 days from the haemorrhage which was connected with the greatest intensity of arteriospasm at that time. All carotid angiographies were done using a contrast medium hypersmolar and hypertonic in relation to the blood and cerebrospinal fluid (Uropolinum-Polfa 60%).

[Location of cerebral aneurysms and arteriospasm after subarachnoid hemorrhage].

On the basis of an analysis of 314 patients with aneurysms situated supratentorially the authors tried to establish a possible correlation between the location of the aneurysm and the frequency, intensity and extent of this spasm. Similarly as in the reports of other authors it was not possible to find a correlation between the location of the aneurysm and arteriospasm. Only the group of 28 patients with multiple aneurysms showed the highest percent of most intense and most extensive arteriospasm, which was particularly evident within the first three days after haemorrhage. In the period from the 4th to the 6th days after the last subarachnoid haemorrhage the lowest proportion of angiograms with arteriospasm was noted in patients with aneurysms of the anterior communicating artery. In this group diffuse bilateral arteriospasm was significantly more frequent than in the remaining groups.

[Preoperative selective embolization of intracranial meningiomas].

The authors report preliminary experiences with preoperative embolization of intracranial meningiomas describing the technique using a specially prepared gelfoam as embolizing material. Embolization was done in 3 cases, in 2 of them transient facial nerve paresis appeared as complication. The effectiveness of embolization was assessed by means of selective control intraoperative angiography and histological examination. In the conclusions the authors stress the effectiveness of preoperative embolization for reducing intraoperative bleeding and indicate the usefulness of the procedure in selected cases of other brain tumours.

[Spasm of the cerebral arteries in angiographic studies in relation to the duration of bleeding from the aneurysm].

The purpose of the study was determination of the incidence of arteriospasm in the brain in relation to the time after the last bleeding to the time of angiography. The authors analysed 209 cases of bleeding in which one-time angiography of the carotid arteries was done. In 45% of cases arteriospasm was found. The lowest incidence of arteriospasm was observed 1-3 days after the last bleeding, and highest on days 7-9 after this bleeding.

[Conservative treatment of selected cases of chronic paracerebral hematoma].

The authors report an analysis of the course of 16 cases treated conservatively for paracerebral haematomas. The diagnosis was based on angiographic findings of computerized tomography (CT). Haematoma resorption was confirmed by CT. The follow-up was from 3 months to 6 years. The patients were in a general good condition without or with only transient signs of focal central nervous system damage, the thickness of the haematomas was never over 15 mm. In all cases the haematomas were resorbed, 15 patients feel well and lead an active life, in one case signs of organic brain injury developed connected with dilatation of the ventricular system and cortico-subcortical atrophy. In the light of the observed material and a literature review criteria are suggested which could indicate expected good effects of conservative treatment in such cases.

[A case of angioma of the spinal cord].

A case of angioma situated in the lumbosacral spinal cord is described in a man aged 35 years. During 10 months of conservative treatment and rehabilitation therapy the condition of the patient deteriorated, paralysis of lower extremities developed. The angioma was diagnosed during myelography. The patient was treated surgically, the angioma was removed, and after the operation and rehabilitation therapy an improvement of the neurological state was obtained.

[Secondary post-traumatic intracerebral hematomas].

Two cases are reported of post-traumatic intracerebral haematoma development several days after trauma, as demonstrated by serial computerized tomograms of the brain. In both cases the haematoma was removed surgically. The pathological mechanisms of such haematomas is analysed calling attention to the possibility of secondary bleeding appearance in foci of contusion of the brain. The importance is stressed of computerized tomography in the diagnosis of these post-traumatic complications.
