Use of antimesothelial cell antibody and computer assisted quantitative analysis for distinguishing between reactive and neoplastic serosal tissues.

Nine malignant mesotheliomas and 12 specimens of benign reactive mesothelial hyperplasia were examined using a specific antimesothelial cell antibody. Immunostaining intensity was subsequently estimated by means of image analysis. The mean and standard deviation of integrated optical density in the malignant mesotheliomas differed significantly from those in the mesothelial hyperplasias. In all mesotheliomas but one the integrated optical density was greater than in reactive mesothelial hyperplasia. No significant difference in optical density was observed between the two groups; the standard deviation was significantly higher in the reactive lesions. This technique may be adopted to complement the traditional morphological assessment of primary lesions of the serosal cavities.

Phenoxy herbicides and soft-tissue sarcomas in female rice weeders. A population-based case-referent study.

A population-based case-referent study was conducted in an area of northern Italy where rice growing is the predominant agricultural activity and phenoxy herbicides have been used since 1950. Manual rice weeding was formerly performed by a seasonal female working population; in the early 1950s these women were concurrently exposed to chemical herbicides. Sixty-eight persons representing incident and histologically revised cases (31 women) and 158 population referents (73 women) were interviewed. The cases were histologically confirmed independently by two blinded pathologists, and exposure to phenoxy herbicides was assessed by two blinded pesticide researchers. An age-adjusted odds ratio of 0.91 was found for the living men (with suspect exposures; no man diagnosed as a case had been exposed with certainty to phenoxy herbicides). Among the living women the relative risk was 2.7 (90% confidence interval 0.59-12.37), and it further increased when attention was restricted to women exposed in the whole 1950-1955 period and to younger age groups.

Differentiation towards cartilage and bone in a primary tumour of pleura. Further evidence in support of the concept of mesodermoma.

A case of primary pleural malignant tumour is reported and its histological features described including histogenetic factors. The histological pattern was predominantly and typically biphasic (both epithelial and sarcomatous) with foci of sarcomatous cartilaginous differentiation and osteoid formation reproducing an embryonal pattern of enchondral ossification. This provides supportive evidence for the use of the term 'mesodermoma' to identify primary tumours of serous membranes and to the concept of identification of mesothelium with embryonal mesoderm.

New marker for mesothelioma: an immunoperoxidase study.

An antibody was raised against a protein isolated from the cytoplasm of mesothelioma cells. It was subsequently used in an immunoperoxidase procedure on formalin fixed, paraffin embedded tissue sections. A representative sample of benign and malignant tumours from all the systems of the human body was examined. All the tumours derived from coelomic surfaces (mesotheliomas of pleura, peritoneum, and ovary, and adenomatoid tumour of epididymis) reacted with the antibody. No other tumour tested in this study expressed the protein. These findings indicate that the antibody may be useful in the identification of mesothelioma cells in both histological and cytological diagnostic routine practice when morphological interpretation is in doubt.

The role of concurrent determinations of pleural fluid and tissue carcinoembryonic antigen in the distinction of malignant mesothelioma from metastatic pleural malignancies.

A combined determination of pleural fluid and tissue carcinoembryonic antigen (PF-CEA and T-CEA) by radioimmunoassay and immunoperoxidase staining technique respectively was performed in patients with malignant mesotheliomas (12), metastatic pleural carcinomas (17) and benign pleural diseases (seven). All PF-CEA-positive (greater than 39 ng/ml) cases were T-CEA-positive metastatic carcinomas. In contrast, 4/30 PF-CEA-negative (less than 39 ng/ml) cases were T-CEA-positive metastatic carcinomas (three cases) and idiopathic pleuritis (one case). These results suggest that CEA, though present in the tumour, is not always released in measurable amounts in effusions. Hence T-CEA content should be determined in the PF-CEA-negative cases when an early and definite diagnosis of tumour type is required to enable correct management of these patients. These ancillary tests aim at enhancing the level of confidence of the routine morphological diagnosis of serous surface malignancies in living patients using minimal intervention instead of resorting to open chest surgery.

Isolation and characterization of a possible new protein marker for differential diagnosis between metastatic carcinoma and mesothelioma cells in serous effusions: preliminary findings.

A distinctive diagnostic staining pattern of the cytoplasm of reactive and neoplastic mesothelial cells in serous effusions was assumed to be caused by a specific compound. This was isolated by polyacrylamide gel electrophoresis and corresponded to a protein with an apparent molecular weight around 200,000. Isoelectric point was found to be 8.7. This protein contained high proportions of glutamic acid, serine, and glycine.

Carcinoembryonic antigen in pleural effusions. Diagnostic value in malignant mesothelioma.

The usefulness of the determination of carcinoembryonic antigen (CEA) in pleural effusion was assessed as an aid to the diagnosis of malignant mesothelioma. The concentration of CEA was determined by radioimmunoassay (RIA) in pleural fluid of 213 adult patients, of which 140 had malignant pleural disease and 73 had nonmalignant pleural disease. Pleural fluid CEA (PF CEA) was lower than 12 ng/ml in all 15 mesotheliomas. The statistical probability of a mesothelioma associated with PF CEA greater than 15 ng/ml was found to be zero. The CEA assay in pleural effusion proved to be a valuable adjunct to other diagnostic procedures in differentiating the malignant mesothelioma from metastatic serosal spread.

Ovarian mesothelial tumors and herbicides: a case-control study.

The ovarian cancer risk from herbicide exposure has been studied in a hospital-based, case-control study after some of these compounds have been shown to be carcinogenic for animals. This study includes 60 cases of primary mesothelial ovarian tumors and 127 controls with non-ovarian malignancies drawn from the same file and matched by year of diagnosis, age and residence. A positive association (relative risk 4.38) has been found between herbicide exposure and ovarian mesotheliomas.

Mesodermomas: a new embryological approach to primary tumours of coelomic surfaces.

It appears that the so-called mesothelium lining the serous cavities is in fact mesoderm, with an epithelial-like arrangement which persists in the post-embryological stage. Cellular variability of primary tumours of serous surfaces can be explained on this developmental evidence. The term mesodermoma seems to be justified to define neoplasms arising from undifferentiated and multipotential mesoderm which, in neoplastic disease, exhibits a wide range of differential activity and gives rise to tumours with prevailing myoblastic, angioblastic, lymphoblastic, chondroblastic, osteoblastic, fibroblastic or epithelial-like features.

Acid and alkaline phosphatase in macrophages of Tenebrio molitor larvae stimulated with asbestos.

The authors studied the activity of acid and alkaline phosphatase in macrophages of Tenebrio molitor larvae stimulated with various types of asbestos: A and B chrysotile, crocidolite, amosite and anthophylite. The activity of the two enzymes increased, as did those of beta-galactosidase and N-acetyl-beta-D-glucosaminidase, two previously assayed enzymes. The increase indicates the toxic action of various types of asbestos and correlates to variations in the mortality curves. The increases of the enzymatic activity and the macrophage response vary with the type of asbestos.

[Endemic goiter and inflammatory lesions of the hypothalamus].

Inflammation of the hypothalamus, consisting of perivasal parvicellular sleeves between the paraventricular nucleus rostrally, the mamillary nuclei caudally, 3rd ventricle wall below the sulcus, medially and o sagittal plane taken a little beyond the column of the fornix laterally--i.e. in the space shown to include the hypophysothyrotropic structures by experimental physiopathology--was observed in a woman with endemic goitre. Reference is made to a similar finding in goitrous dogs from enzootic areas of Piedmon. Epidemiological and clinical considerations are put forwary for the view that endemic goitre is responsible for primary cerebral damage, perhaps of encephalitic origin.
