[Intraoperative lactic acidosis, can it be treated? Clinico-experimental, prospective, sequential study].

OBJECTIVE: To verify the efficacy and absence of risk attributable to therapy with alkaline solutions for correction of lactic acidosis and to demonstrate the usefulness of L-carnitine in converting lactate into pyruvate in conditions of good blood oxygenation. DESIGN: Prospective study on a consecutive series of patients subdivided into three groups following the use of: alkalinizing therapy (group I), L-carnitine (group II), or saline solution (group III). Groups 1 and 2 were further subdivided into subgroups "a" and "b" according to the type of alkalinizing agent and of L-carnitine somministration. SETTING: Teaching Hospital-Torrette di Ancona. PATIENTS: 65 patients submitted to major vascular surgery with aortic clamping in the time period between January 1992 and August 1993. INTERVENTIONS: During aortic clamping patients of: group I received 2 mEq:kg of bicarbonate or tromentamolo according to the specific subgroup. Group II received a bolus of 2 g of L-carnitine, patients of group IIb received further 2 g of carnitine in continuous perfusion until the end of surgery. Group III received no pharmacological intervention. MEASUREMENTS: HR BP, arterial blood gases and lactic acid levels were measured at 12 pre-determined times. RESULTS: Only a neutralizing effect of alkalinizing therapy was observed, whereas the lactic acid measurements demonstrated no significant differences between the different groups. CONCLUSIONS: These results confirm the data of other Authors concerning the good compliance of alkalinizing therapy, however, there was demonstrated no clear evidence of its effective usefulness. No metabolic stimulation due to L-canitina could be demonstrated in our experimental conditions.

Environmental aspects of pesticide use in Italian agriculture.

Pesticides used in agriculture accumulate in organisms and reach man through complex metabolic pathways. Accurate analysis of pesticides presence in each step of the food chain is necessary because of the high number of these substances, their potential toxicity and the presence of amplified toxic effects due to synergic interactions. In the same way it is necessary to evaluate the environmental effects of the whole pesticides cycle, from industrial production until final disposal of used containers and residuals. Furthermore recent introduction of new categories of chemicals which are not strictly classifiable as pesticides (photosynthetic activity stimulators, aesthetic aspect enhancers) and not yet sufficiently studied in their metabolic behaviour, increase the problem complexity. Complete studies about these complex problems and about the effects on ecosystems have not yet been carried out in Italy, due to ambiguous laws and regulations, inadequate data collection methods and decisional influence of the chemical industry on agriculture policy.

[Heterologous lyophilized collagen in the secondary healing of pilonidal fistulae].

The authors evaluate heterologous lyophilized collagen efficacy and tolerability in second intention healing of pilonidal sinus fistulae. This therapeutical approach seems to be characterized by a lower relapse incidence. The use of heterologous lyophilized collagen relevantly reduces the prolonged healing time required by such method.

[Foreign bodies and injuries of the rectum].

Personal experience of 33 cases of foreign rectal bodies introduced transanally with or without associated lesions is reported. After examining the principles of diagnosis, stress is laid on therapy and personal experience and it is suggested that more energy should be applied to extracting the foreign body transanally even using more or less deep anaesthesia, so avoiding the need for abdominal surgery.

[Perinatal fetal distress: correlations between Apgar score at birth and assay of CK-BB in umbilical cord blood].

32 newborn infant's umbilical cord blood CK-BB activity was tested at birth. 14 of them had a superior to 7 Apgar score at first and fifth minutes, whereas 18 newborn infant Apgar score was inferior to 7. The CK-BB activity was significantly higher in newborn infants having a low Apgar score at birth. A low Apgar score at first and fifth minutes together with a high CK-BB activity must be considered a nervous trouble signal and a warning in order to look after children's psychomotor skills more strictly and individual neurological damage.

[The kidney in shock. Changes of some hemodynamic and bio-humoral parameters. II].

In our experiments on 5 pigs of weight between 40-50 kg we've noticed changes of the parameters occurring precociously during the initial stage of the shock. Particularly we observed a sudden reduction of diuresis reaching total anuria in 15 min and persistence of this anuria for the entire time of the shock (120 min). Immediately after introduction of the removed blood the arterial pressure and the diuresis return to normal value. Concerning the blood flow in renal artery it was observed a gradual decreasing to 10-30 ml/min (normal value = 120-180 ml/min). After the restoration of the blood volume we did'nt notice a clean resumption of the flow although arterial pressure reached normal levels (flow value = 70% of the basal value). The hematic level of K+ undergoes a sudden increase that reaches. Maximal level compatible with animal's life. Within 45-60 min from the beginning of the shock and than decrease with restarting of the diuresis. Creatinine reaches 2 mg/ml in 60 min and remain constant during the following 120 min. The value of the plasma renin were precociously raised, reaching values particularly elevated after 90-120 min from the beginning of the shock and decreasing rapidly with the introduction of the removed blood (parallel behaviour of the arterial pressure).

[Spontaneous chylopericardium. A case report (author's transl)].

The AA report an isolated case of chylopericardium in an 11 years old female as anomalous communication between the thoracic duct and pericardium. The origin of this clinically asymptomatic effusion was observed after lynphography, where, two months later a simple surgical drainage was applied without success. A positive result was obtained by a new surgical procedure which consisted of a pleuro pericardial window. The AA. emphisise on a very rare case of spontaneous pericardial effusion not accompanied by lynphatic stasis in other districts (only 32 observations in Literature). An accurate lynphographic study is obligatory once the nature of the liquid has been accertained in order to avoid a certain recurrence when a simple drainage is applied.
