The authors make an epidemiologic analysis of the first cholera cases notified by the Congo in 1978 and 1979. Contrary to the explosive cholera, experiences by Burundi and Zaïre during the same period, the cholera, in the Congo, had a very subtle propagation, with the well-know "iceberg" phenomena. Human contacts were very important in the transmission of the disease. The cases encountered were almost exclusively of the family type. Healthy carriers represent a threat to the other countries.

The Ch 1 + 2 fraction has been marked by means of culture of Vibrio cholerae Ogawa HK1 on a synthetic medium containing leucine H3. The antigen distribution has been then studied before and after vaccination with the same non-marked antigen in normal and axenic mice, at the same time by the demonstration of radioactivity and radioimmunofluorescence in intestine, spleen, liver, kidney and thymus. Whichever the administration route, intestine and spleen are first stimulated, then more intensively with the second injection or ingestion. When administrated per os the fraction crosses the intestinal barrier and is fixed on the main lymphoid organs: intestine, spleen, thymus. After ingestion or injection into axenic mice, spleen is the first organ stimulated.
The effects of different combinations of quaternary ammonium compounds (Sanitized, Sanigard) on faecal and soil organisms demonstrate the usefulness of these products (dilution 1/100,000). A contact time of 30 seconds to 4 minutes is sufficient for bacteriocidal activity. Materials treated with these compounds retain their bacteriocidal activity after 10 washes with boiling water and for over a month with normal usage. The disinfection of a door handle contaminated with Vibrio cholerae or Salmonella typhi destroys these bacteriae after 30 seconds and prevents re-contamination for a month. Therefore, they are very useful in the control of diseases caused by bad hygiene.
Authors report on epidemiologic, bacteriology and therapeutic data related to 1991 cholera outbreak in Benin in the general context of the 7th world pandemic. 7474 cases were notified from all over the country. Vibrio cholerae 01, El Tor biotype, was identified in many patients stools and in the surroundings. Control measures implemented in this situation are described: early parenteral and mainly oral rehydration, antibiotic treatments for patients and contacts, systematic home control around cases, wells disinfection and general hygiene promotion, mobilization of the Civil Service Authorities and population education. Authors conclude that strict epidemiologic monitoring is necessary in epidemic circumstances as well as during endemo-sporadic period.
