[Giant ovarian cyst and pregnancy (a case report)].

We report a case of giant ovarian cyst in pregnancy of 23 weeks of amenorrhea revealed by digestive and respiratory compression. As the absence of ultrasonographic and biologic malignancy criterias, several punctures of the cyst were done permitting the evolution of pregnancy until the preterm period. An annexectomy done in the immediate post-partum permitted to avoid the frequent mechanical accidents during this period. From this case, we discuss clinical and therapeutic problems of this association.

Secretion of parasite-specific immunoglobulin G by purified blood B lymphocytes from immune individuals after in vitro stimulation with recombinant Plasmodium falciparum merozoite surface protein-119 antigen.

The C-terminal 19 000 MW fragment of merozoite surface protein-1 (MSP119) is one of the most promising candidate antigens for a malaria vaccine. Baculovirus recombinant Plasmodium falciparum MSP119 has been used to define conditions for the in vitro production of specific antibodies by purified human blood B cells in a culture system where T-cell signals were provided by the engagement of CD40 molecules and exogenous cytokines. MSP119 preferentially induced surface immunoglobulin G (IgG) -positive (sgamma+) B lymphocytes from P. falciparum-immune donors to differentiate and produce antigen-specific IgG. In contrast, naïve B cells or cells from non-immune donors could not be induced to secrete parasite-specific IgG in vitro. Although IgG secretion was obtained in the absence of exogenous cytokines, it was dependent on B-cell-derived interleukin-10 (IL-10) and/or B-cell factor(s) under the control of IL-10, since IgG levels were significantly decreased in the presence of neutralizing anti-IL-10 antibodies. These results demonstrate at the cellular level that a single malaria vaccine candidate polypeptide can direct parasite-specific antibody production mediated by the secretion of potentiating factors.

Immune responses to Plasmodium falciparum-merozoite surface protein 1 (MSP1) antigen, II. Induction of parasite-specific immunoglobulin G in unsensitized human B cells after in vitro T-cell priming with MSP119.

A baculovirus recombinant antigen corresponding to the C-terminal 19 000 MW fragment of Plasmodium falciparum merozoite surface protein 1 (MSP119), has been used to prime T cells from individuals with no previous exposure to malaria, to provide help for the induction of a parasite specific antibody response in vitro. Although MSP119 alone could induce a small but detectable T-cell response, which included interleukin-4 (IL-4) secretion, this response was significantly increased by the presence of IL-2. In addition, IL-4 was shown to synergize with IL-2 for the induction of antigen-specific T-cell responses. If interferon-gamma (IFN-gamma), IL-12, or neutralizing anti-IL-4 antibody was present at the time of priming, the T-cell responses were abolished. Parasite-specific immunoglobulin G (IgG) could be detected after secondary restimulation with MSP119, IL-10 and anti-CD40 monoclonal antibody in cultures containing MSP119 primed T cells, autologous B cells, IL-2 and IL-4. No antibody was secreted in the absence of primed T cells in this B-cell culture assay. These data show that recombinant MSP119, a leading malaria vaccine candidate, can prime non-immune human lymphocytes under defined in vitro experimental conditions, which include regulatory cytokines and/or other costimulatory molecules. This is a complementary approach for exploring immunogenic mechanisms of potential vaccine candidates such as P. falciparum antigens in humans.

Different Plasmodium falciparum recombinant MSP1(19) antigens differ in their capacities to stimulate in vitro peripheral blood T lymphocytes in individuals from various endemic areas.

This study reports on T-cell proliferative responses to the 19-kDa C-terminal domain of the Plasmodium falciparum merozoite surface protein (MSP1(19)). Three different recombinant proteins were used: an Escherichia coli product expressing the first EGF-like domain and Saccharomyces cerevisiae and baculovirus/insect-cell-produced proteins containing both EGF-like domains, the latter protein being produced with or without N-glycosylation. Cell donors were P. falciparum-immune adults with no recent history of clinical malaria and recruited from three Senegalese settings with different epidemiological parasite transmission. Each mononuclear-blood-cell preparation was stimulated with a range of concentrations of the three proteins. Most subjects' mononuclear cells were reactive to at least one protein, but significant differences in lymphoproliferation were seen between the settings and within individual cultures depending on the protein source and concentration. Importantly, lymphoproliferation indices correlated inversely with the intensity of P. falciparum malaria transmission. When purified T lymphocytes were cultured in the presence of MSP1(19) plus autologous monocytes, B lymphocytes or a proposed CD1+ dendritic-cell population as costimulatory cells, significant differences were observed depending on the individual's previous exposure to parasites. This study shows that the stimulation of lymphocyte proliferation in vitro with MSP1(19) depends on several factors, including epidemiological conditions and protein preparations.

[Antibodies specific to Plasmodium falciparum antigens in immune individuals: III. Seasonal course of the response to a major antigen of the asexual blood forms in two sites of different malaria exposure].

Specific IgG1 and IgG3 antibody responses to a major Plasmodium falciparum blood stage antigen i.e. MSP1 have been measured in plasma obtained from immune individuals living in areas with different endemicities and sampled at different periods corresponding to different levels of parasite transmission. The study shows a significant imbalance between IgG1 and IgG3 antibody responses in Dielmo vs. Ndiop, and a differential regulation of IgG1 and IgG3 responses both in subclasses and in titers (low: 1/200, and high: 1/2,000) depending upon intensity of parasite exposure.

[Fibrodysplasia ossificans progressiva or Munchmeyer disease apropos of 2 cases].

Fibrodysplasia ossificans progressiva or myositis ossificans progessiva or still Munchmeyer disease is a genetic ailment with dominant autosomic transmission. It includes a high rate of change and doesn't appear any race. The authors gave an account of two remarks about it on a causal and late discovery upon two young black Africans. The first observation is a nine year-old-boy, without any similar family previous history and who has been hospitalized in maxillo-facial milieu for a mandible osteitis staphiloccocus. It showed some muscular ossifications of paravertebral, cervical, dorsal and lumbar nature--from a spontaneous appearance and evolving progressively since the age of three months. The radiographic results displayed some specific bones disorders which enable to retain the diagnosis above. The second observation is the case of a twenty four-year-old-woman who has previously benefitted from a surgical exploration a non-inflammatory muscular tumefication at the right arm which occurred at the age of twenty one. Three years later, she took surgery for the restriction of the oral gap and a right hand side lumbar paravertebral tumefaction which was thus restricting the mobility of the rachis. The radiological results have found some specific bone disorder++ at the hands and the feet which enable to link the paravertebral ossifications to their fibrodysplasic origin. Munchmeyer disease remains a affection of easy radio-clinical diagnosis. The essential point is to precociously think of it before the specific osseous anomalies occurring at the level of the feet and the hands. The functional or even vital prognosis remains closely linked to both the important and the topography of conjunctivo-muscular ossifications.

[Aflatoxins in food: tests of decontamination of peanut cakes by ionizing treatment].

The efficacy of ionising treatment for decontaminating peanut cakes was tested. The influence of cakes water content and the effect of ionisation dose rate were studied. The results obtained after a reverse phase liquid chromatographic determination of B1, B2, G1 and G2 aflatoxins have revealed an important contamination of the peanut cakes (up to 1000 ppb of total aflatoxin's contents). After ionising treatment at 25 kGy, the aflatoxins degradation in peanut cake's was less important in dried samples (about 5-10% at 0.55 water activity: aw) than in the humid ones (40-60% degradation at 0.95 water activity). At this dose, any indicative difference of the degradation rate of aflatoxins, with regard to the ionising process was observed. The efficacy of ionising treatment for decontaminating peanut cakes could probably be improved, however the economic interest of such process as alternative of the treatment with ammonia is questionable.

[Pesticide bioaccumulation: measurement and levels of organochlorine residues in products of vegetable origin].

Because of their lipophilic properties and their persistence, organochlorine pesticides residues can be accumulated in human body by consuming regularly plants products. This study consisted of the monitoring of the contamination level of pesticides residues from various plants products, in order to assess the long- term intoxication risk to which Senegalese consumer is exposed ant identify throughout these plants some indicators of atmospheric pollution. The chromatographic analysis of medicinal plants, fruits and vegetables extracts showed that they could be contaminated by organochlorine residues. About ten of these pesticides residues were found confirming that the analysed plants were really contaminated. The residues level in medicinal plants (from 0.1 to 45 micrograms/kg) were widely below the maximum allowed concentrations and were also lower than those in fruits and vegetables (from 0.1 microgram/kg to 460 micrograms/kg). Heptachlorine residues contents of tomatoes and citrus fruits were higher than FAO/WHO norms, so that, a regular monitoring of the plants products is recommended for better protection of consumers.

[Measurement and levels of aflatoxins in small-scale pressed peanut oil prepared in the Diourbel and Kaolack regions of Senegal].

Aflatoxins are mycotoxins produced by some strains of fungus (Aspergillus) which develops in peanuts seeds. Peanuts oil and past are very used up in Senegal, then the aflatoxins poisoning risk are very actual. This study relates to the determination of contamination levels by aflatoxins from peanut oil food prepared by small scale production in areas of Kaolack and Diourbel. High Performance Liquid Chromatography (HPLC) analysis of the different samples showed that 80% of them were contaminated in the areas of Kaolack and Diourbel. Aflatoxin B1, B2, G1 and G2 has been detected with a profile of contamination almost identical in the both areas. Aflatoxin B1 was prevalent and has been found in over 85% of samples. Mean contents of this mycotoxin (the most dangerous toxin) is about 40 ppb, value widely superior to allowable specifications.

[Surveillance of a woman treated for breast cancer].

The intensive follow up of breast cancer patients is not safer than a minimalist policy for breast cancer surveillance. The survival rate is not modified by the use of expensive exams. The follow up program is anxiogenous, delay the reinsertion of the patients and useless because in 75% of cases the patient herself discovered the relapse. A good clinical examination, including gynaecological examination and a mammography are sufficient for a good follow up practice. The guidelines for the follow up of breast cancer patients include self examination of the breast monthly, clinical examination twice a year and an annual mammography during five years and an annual clinical examination and a mammography after. The radiotherapist, the surgical or medical oncologist should be involved with the general practitioner for this follow up.

[Well water pollution in the Khombole district: research on the contamination by organochlorine pesticide residues and organic substances (feces)].

The study realized in the district of Khombole (SENEGAL) has permit to estimate the contamination levels of wells waters used by the populations. The research and the dosage of the organichlorine pesticide residues, nitrites and nitrates and microbiologic analysis have been done on 19 wells chosen after a drawning of lots. The organochlorine pesticide residues which have been found prove that the wells are permanently exposed to these chemical substances which don't constitute nevertheless a major risk for the populations health. The results of our research proved also that there is a real risk of intoxication with the nitrogen oxides. In effects more than 50% of the wells have revealed nitrates contents up to the indicative value (25 mg/l). As for the nitrates, with a few exceptions (5/18), the contents are superior to the authorized norm (0.1 mg/l). By another way the bacteriologic analysis has revealed in the one hand a DBT (Total Bacterian Count) up to 10,000 germs/l for all the wells, and in the other hand the presence of Escherichia coli, Enterococcus faecalis that confirm a faecal contamination.

[Emergency obstetrical hysterectomy].

Hysterectomy on pregnant uterus is a mutilating procedure used by obstetrician only when he haven't choice in case of major complications of pregnancy or delivery. This retrospective study worked on 35 cases of obstetrical hysterectomy performed on 22 months period (January 94-October 95). The frequency of the procedure among all the in hospital deliveries was 0.45%. The mean age of the patients was 31 years, and the mean parity was 7. 77.5% of the women were referred from over surrounding hospitals where natural deliveries had failed. We performed the hysterectomy because of uncontrolled haemorrhage in 71.7%, and infection in 25.8%. Sub-total hysterectomy was performed in 82.8%. Maternal mortality was 20%. Hysterectomy is very mutilating, but is a life rescueing procedure behind a lifethreatening complication of pregnancy or delivery. The best strategy to prevent the necessity of hysterectomy, is to develop the early obstetrical reference program that unfortunately is not yet in practice in our health care system.

[Monitoring women on tamoxifen ].

Worldwide, 7 million women are taking tamoxifen for breast cancer. The beneficial effects of tamoxifen on patient survival have been clearly demonstrated. Tamoxifen has few adverse effects and severe complications are very uncommon. An higher risk of endometrial cancer at the 20 mg/d dosage has been discussed and if it does exist, is minimal (RR x 2 instead of 1.3). Patient monitoring is basically clinical with an annual examination of the lower limbs and genital organs and an ophthalmological examination. Further explorations are needed in cases with specific symptoms, especially metrorrhage. There has been no proof that systematic endometrial cytology or ultrasound explorations are useful.

Plasmodium falciparum- and merozoite surface protein 1-specific antibody isotype balance in immune Senegalese adults.

This study shows markedly different isotype distributions of antibodies to asexual blood stages of Plasmodium falciparum and to merozoite surface protein 1 in clinically immune Senegalese adults depending on the study site. The relationships between immunoglobulin M (IgM) and IgG and between IgG3 and IgG1 antibodies differed in settings where transmission is perennial compared to settings where it is seasonal. This suggests a role for antibody class and/or subclass production and utilization in the regulation of protective immunity to such antigens.

[Specific antibodies against Plasmodium falciparum antigens in immune subjects: I. Comparison of detection/titration methods].

We report a comparative study of specific antibody levels to the blood stages of P. falciparum in individuals living in two different areas with different transmission levels of malaria. We have compared 2 techniques for the detection/titration of antibodies i.e. ELISA and IFI, ELISA being particularly suitable for immuno-epidemiological related studies. A schizont lysate from P. falciparum infected red blood cells from FUP/CB Marburg strain as antigen proved usefullness, as compared with an antigen extracted from a local strain recently adapted to in vitro culture. Using these techniques, high specific antibody responses were found in the villagers' sera and mean levels of antibodies increased with age. Levels of specific IgG were comparable between those two locations, in spite of a ten fold higher level of transmission between the two villages. In contrast, a significantly higher level of IgM in adults living in holoendemic.

[Retroplacental hematoma at the Dakar University Hospital Center].

Abruption placenta has been a principal cause of maternal mortality by hemorragy at Dakar University Teaching Hospital. The incidence is 2.44%. It related with multiparity and age. Etiologics factors such as toxemy (7.01%) and HTA (5.26%) were founded, lack of prenatal care was notified. The complete form is observed in 71.3% and bad hemodynamic conditions in 61%. The death rate is 8.4%: morbidity is dominate by anemia, renal failure and coagulation defect. Perinatal mortality is 87.07%. Cesareans were performed on only 2.25%. The amelioration of the prognostic is linked with depistage of women with high risk of abruption placenta, a good prenatal survey, an early diagnosis and quick evacuation of the uterus.
