Additional case of Neuhäuser megalocornea and mental retardation syndrome with congenital hypotonia.

We report on a 3-1/2-year-old Spanish girl with the Neuhäuser megalocornea and mental retardation syndrome. A review shows that megalocornea, mental retardation, and, presumably, hypotonia, are the major manifestations for diagnosis. The facial appearance of the typical cases is characterised by frontal bossing, broad nasal bridge, mild hypertelorism, long upper lip, and small mandible.

Synthesis of biosynthetic inhibitors of the sex pheromone of Spodoptera littoralis. Part II: Acetylenic and cyclopropane fatty acids.

The synthesis of new acetylenic and cyclopropane fatty acids, as potential inhibitors of the beta-oxidation step in the proposed biosynthesis of the sex pheromone of the Egyptian armyworm Spodoptera littoralis, is reported. The biological activity of the compounds has been determined by in vitro and in vivo bioassays, and among all the compounds tested, dichlorocyclopropane acid has shown the highest inhibition activity displayed so far.

Application of glow curve analysis methods to improve TLD-100 dose reassessment performance.

This paper presents a method of improving the TLD-100 dose reassessment performance. This method consists of applying numerical analysis techniques for evaluating the TLD-100 phototransferred thermoluminescence (PTTL) glow curve. From this analysis, a simple procedure for estimating the ultraviolet background components usually present in phototransferred thermoluminescence (TL) signals has been established. This procedure has been implemented in a computer program which performs the automatic evaluation of the glow curves and extracts the dose information contained in the PTTL curves. The use of this computer-aided evaluational method has enabled the extension of the working range of estimated absorbed dose down to 0.2-0.5 mGy with very adequate operational quality for doses even below the conventionally admitted lower reestimation limit (approximately 2 mGy). Because TL readout is a destructive process, the ability to reestimate doses can be important in any kind of dosimetric activity, such as operational dosimetry programs. The other commonly used dosimeter, film, uses a nondestructive readout and, therefore, presents some advantages over TLD when dose reassessment is necessary. With the reported improvements in the TLD-100 dose reassessment performance, the full range of absorbed doses covered by film dosimetry can now be reliably reassessed using TLD-100 dosimeters.

Behavior in agricultural soils of a recombinant Pseudomonas bacterium that simultaneously degrades alkyl- and haloaromatics.

Pseudomonas sp. FR1 (pFRC20P) is a recombinant bacterium able simultaneously to degrade alkyl- and haloaromatics due to the xylXYZL and xylS genes from the TOL plasmid borne on the bacterial chromosome, and to the gene encoding for a gamma-lactone isomerase from Alcaligenes eutrophus on pFRC20P. The survival of this strain in sterile soils was shown to depend on the physicochemical properties of the soil. The recombinant information was stable in bacteria introduced in soils and conferred selective advantage to the host bacterium when the soils were amended with low amounts of p-methylbenzoate. However, relatively high amounts of this chemical significantly reduced survival of the bacterium. Survival was more seriously affected by chlorobenzoates than by methylbenzoates. Evolution of 14CO2 from p-methyl-14C-benzoate in soils confirmed that Pseudomonas sp. FR1 (pFRC20P) functions in a complex system in accordance with its design. Survival of Pseudomonas sp. FR1 (pFRC20P) was better at 4 degrees C and 25 degrees C than at 37 degrees C. On plates, the pFRC20P plasmid was mobilized by helper plasmids to P. putida at a frequency of about 10(-6) transconjugants per recipient. However, no transfer of recombinant DNA integrated on the chromosome was found, nor was lateral transfer of the recombinant DNA borne by Pseudomonas sp. FR1 (pFRC20P) detected in soils.

[Efficacy of auditing in improving a Control Program of Pregnancy].

OBJECTIVE: To compare the results obtained in two successive audits to observe the effectiveness of this control method. DESIGN: The care process followed by pregnant women attended in 1986-87 is compared with that in 1988. LOCATION: Cartuja Health Centre (Granada). PARTICIPANTS: Data referring to 20 markers with defined quality criteria are obtained by auditing clinical protocols. METHODS AND MAIN RESULTS: The qualitative markers were compared to the comparison test of two proportions for separate samples and the quantitative markers with the of student for separate samples and equal variants. The quality index improved in 9 markers and worsened in 1; the remainder did not change. The results prove the effectiveness of quality control to improve care given in a Health Centre.

[Spanish Pediatric Association. Madrid. Human immunodeficiency virus infection. Spanish multicenter study].

The results of an epidemiological survey in Spain organized by the "Spanish Pediatric Association" of children born to HIV+ mothers from 1981-1989 are presented. The objectives were, a) to document in the different parts of Spain the number of HIV+ children born to HIV+ mothers, and b) to identify the risk factors involved in the HIV infection. Only in public hospitals, a total of 1938 HIV+ children born to HIV+ mothers were identified. A systematic yearly increase of HIV+ newborns was evident since 1985. Madrid, Catalonia, Valencia, Basq and Andalucia communities incorporated 85% of all cases. 93% of mothers were IVDU and 51% of them were married. Prematurity occurred in 27% of cases and low birth weight in 33% of NB's. The rate of transmission in children over 18 months of age was 25.6% Breast feeding could be a risk factor in the transmission of HIV infection observed in children over 18 months of age. No other maternal risk factors responsible for the HIV transmission were identified. 12% of children died with AIDS. Clinical classification and evolution of HIV infection did not significantly differ from similar studies in other parts of the world. In spite of only public hospitals being involved, Spain has more identified HIV+ children than Italy, Switzerland, Federal Republic of Germany, United Kingdom, Sweden and Belgium combined. The pediatric centres with large number of cases require interdisciplinary pediatric specialized units to cope with the increase demand of medical services, now inadequate and insufficient. To fight drug use and HIV infection in Spain, health education and family support, are indispensable in trying to arrest the continuous spread the HIV infection.

A study of the electrokinetic and stability properties of nitrofurantoin suspensions. I: Electrokinetics.

An experimental investigation is described of the electrokinetic properties of nitrofurantoin. The experiments are based on measurements of electrophoretic mobility in suspensions of this drug and the calculation of its zeta potential (zeta) following two different procedures. A simple mechanism is proposed for explaining the sign of the surface charge of nitrofurantoin and its increase with the pH of the medium. The effect on zeta of changes in the concentration of NaCl, CaCl2, or AlCl3 is studied for different pH values of the suspensions. The behavior of zeta in the presence of NaCl shows evidence of Cl- adsorption on the particles, especially at low electrolyte concentrations. The AlCl3 salt was capable of reversing the sign of the surface charge, changing it to positive when a given, pH-dependent concentration was added to the suspension. The effect of this salt on zeta for different pH values was probably due to the hydrolysis of the cation Al3+. A significant increase of the negative surface charge was observed when a small amount (0.1%) of a synthetic polymer, Carbopol 934, was present in NaCl or AlCl3 systems. This appears to indicate that the large polymer molecules adsorb on nitrofurantoin particles as part of their stabilizing mechanism.

A study of the electrokinetic and stability properties of nitrofurantoin suspensions. II: Flocculation and redispersion properties as compared with theoretical interaction energy curves.

The stability and redispersion properties of nitrofurantoin dispersions are experimentally studied using simple but reproducible techniques. Solutions of different electrolyte concentrations and pH values are employed as dispersing media. The pH appears to be a determinant factor in the properties studied. Thus, with all the electrolytes considered (NaCl, CaCl2, and AlCl3, almost optimum redispersion was achieved when the pH was maintained close to neutrality. However, the important effect of AlCl3 as compared with the other electrolytes is also clearly demonstrated. Especially interesting are the results obtained when 0.1% Carbopol is added to the suspensions. The important effect of this polymer on the surface charge of nitrofurantoin, mainly at pH 7, manifests in excellent redispersion of the suspensions with either of the electrolytes employed. Using simple expressions that approximate the equations proposed in the DLVO theory of interaction between colloidal particles, the variation of the total interaction energy between nitrofurantoin particles is calculated as a function of interparticle distance. The results are compared with the experimental determinations of the sedimentation and redispersion properties of nitrofurantoin. Although some discrepancies between theory and experiments are found, the results indicate a reasonable agreement between the general features of both.

The effect of diameter and length of hydroxylapatite-coated dental implants on ultimate pullout force in dog alveolar bone.

This study compared the effect of diameter and length on the pullout force required to extract hydroxylapatite-coated implants from dog alveolar bone. After 15 weeks for integration, implants of 3.0, 3.3, and 4.0 mm diameter and 4, 8, and 15 mm length were pulled. The results showed that the ultimate pullout force correlated strongly to implant length, but not diameter. Cortical bone contributed more to implant retention than cancellous bone.

Renal haemodynamic effects of short term cyclosporin A administration in patients with insulin-dependent diabetes mellitus.

To investigate the time relationships involved in cyclosporin-induced nephrotoxicity we studied changes in blood pressure, renal haemodynamics and sodium excretion in 22 adult patients with insulin-dependent diabetes mellitus treated with cyclosporin (CsA) for 4 +/- 2 days, compared to 22 insulin-dependent diabetic patients receiving conventional insulin therapy, who were matched for age and duration of diabetes. To further clarify the pathogenic role of the renin-angiotensin system, insulin-dependent diabetic patients receiving CsA were studied before and after sublingual administration of 75 mg captopril. An average of 4 days CsA treatment markedly increased blood pressure and renal vascular resistance, but did not alter glomerular filtration rate, renal plasma flow, sodium urinary excretion, or body-weight. The marked renal vasoconstriction without early changes in GFR suggests that the late decrease in GFR may involve other factors in addition to renal hypoperfusion. Acute inhibition of angiotension II formation was still able to decrease blood pressure and renal vascular resistance, although not to normal control values. These results indicate that a physiological concentration of angiotensin II may potentialise but may not be the sole factor involved in the vasopressor effect of CsA.

Electrophoretic study of nitrofurantoin in aqueous suspensions. Effect of the addition of a polymeric thickener.

The electrophoretic mobility of nitrofurantoin (based on the microelectrophoresis method) has been studied in dilute dispersions of the drug. Specifically, the effect of NaCl, CaCl2 and AlCl3, pH, and a thickener, Carbopol 934, was determined. The electrophoretic mobility (mu) increases in absolute terms when the pH is raised between 3 and 9, although mu remains negative in this pH range. The variation of absolute mobility ([mu[) with NaCl concentration shows a pronounced maximum for a concentration of about 10(-4) M. However, when the concentration of CaCl2 in the medium is increased, [mu[ decreases steadily. The effect of AlCl3 concentration on the mobility is markedly influenced by the pH of the dispersing medium although a general trend is observed for mu to become more positive with increasing concentration of the salt. Finally, Carbopol 934 appears to impart an extra negative charge to the nitrofurantoin surface, since higher negative mobilities were measured in the presence of the polymer.

Blood pressure changes induced by ciclosporin A in type I diabetes.

The determinants of ciclosporin A-induced blood pressure changes were analyzed initially and after 3 months in 30 recent type I diabetics submitted to chronic treatment with ciclosporin as single immunosuppressive drug. Prevalence of hypertension was 17% and relatively low as compared to those reported in organ transplantation. Ciclosporin induced a slight decrease in glomerular filtration rate and renal plasma flow, with unchanged filtration fraction, a mild average increase in blood pressure and a more pronounced increase in renal vascular resistance. There was a trend toward decrease in absolute urinary sodium excretion whereas fractional excretion of sodium was unchanged. Presence of familial history of essential hypertension was characterized by a slightly insignificantly lower renal plasma flow and did not affect the renal effect of ciclosporin except that the renal plasma flow was significantly lower in the groups of patients genetically predisposed.

[Orbital abscess as a complication of orbital cellulitis. Diagnostic value of C.A.T].

We present a case of "orbital abscess"--uncommon and severe complication of "orbital cellulitis"--in a 2 year-old female, and we consider about the etiology and pathogenesis of this process. The treatment of "orbital cellulitis" requires adequate parenteral antibiotic therapy and judicious surgical intervention, which should be planned with the results of computerized tomography in mind.

GABAB receptor activation inhibits Ca2+-activated potassium channels in synaptosomes: involvement of G-proteins.

86Rb-efflux assay from preloaded synaptosomes of rat cerebral cortex was developed to study the effect of GABAB receptor agonist baclofen on Ca2+-activated K+-channels. Depolarization (100 mM K) of 86Rb-loaded synaptosomes in physiological buffer increased Ca2+-activated 86Rb-efflux by 400%. The 86Rb-efflux was blocked by quinine sulphate, tetraethylammonium and La3+ indicating the involvement of Ca2+-activated K+-channels. (-)Baclofen inhibited Ca2+-activated 86Rb-efflux in a stereospecific manner. The inhibitory effect of (-)baclofen was mediated by GABAB receptor activation, since it was blocked by GABAB antagonist phaclofen, but not by bicuculline. Further, pertussis toxin also blocked the ability of baclofen or depolarizing action to affect Ca2+-activated K+-channels. These results suggest that baclofen inhibits Ca2+-activated K+-channels in synaptosomes and these channels are regulated by G-proteins. This assay may provide an ideal in vitro model to study GABAB receptor pharmacology.

New 1,4-dihydropyridine derivatives combining calcium antagonism and alpha-adrenolytic properties.

A series of twelve 1,4-dihydropyridine derivatives incorporating an alpha-adrenergic moiety in one of the ester chains was synthesized. The compounds were evaluated for their calcium antagonist activities by the inhibition of [3H]nitrendipine binding and, in vitro, on pig coronary artery. Their alpha 1- and alpha 2-adrenolytic effects were assessed from their inhibition of [3H]prazosin and [3H]yohimbine binding and, in vitro, on rat aorta and guinea pig vas deferens. Compounds 6 and 9-11 displayed strong calcium antagonist activities, identical with that of nicardipine. The moderate alpha-adrenolytic properties observed were attributed to the presence of alpha-adrenergic moieties. The four chiral derivatives 6a (R,R), 6b (S,S), 6c (S,R), and 6d (R,S) with an N-methyl-N-(benzodioxanylmethyl)amino group on the ester chain were prepared and tested as done previously. Some structure-activity relationships are discussed.

Effect of nicardipine on renal sodium handling in normotensive insulin-dependent diabetics and controls.

In 12 newly diagnosed normotensive insulin-dependent diabetics and eight controls, we compared the effects of acute nicardipine administration on renal haemodynamics and segmental tubular movement of sodium assessed as by lithium clearances. Both in the controls and the insulin-dependent diabetics, nicardipine induced similar haemodynamic changes, including a mild decrease in mean arterial pressure and renal vascular resistance, whereas the glomerular filtration, renal plasma flow and filtration fraction were unchanged. Despite a fall in blood pressure, nicardipine exerted a marked natriuretic effect in both groups, which appears to have been primarily due to a decrease in distal reabsorption and, to a lesser extent, in proximal reabsorption.
