Nine cases of myelodysplastic syndrome with a deletion of the long arm of chromosome #11 (11q-) showed ringed sideroblasts, and three of which had an acquired sideroblastic anemia according to the criteria of the FAB classification. In contrast, among four cases of myelodysplastic syndromes with translocation of extra material to the long arm of chromosome #11 (11q+), only one showed bone marrow sideroblasts. These results strongly indicate that an 11q- chromosome is a marker of iron overload in myelodysplastic syndromes. Within the cases of 11q- associated with sideroblastosis, two cytogenetically different anomalies (i.e., terminal or interstitial deletions) were delineated.

We present four patients with acute nonlymphocytic leukemia, one M2 and three M4 French-American-British (FAB) types and one patient with refractory anemia with excess of blasts in transformation who at diagnosis had trisomy of chromosome 4 as the primary karyotypic anomaly. This chromosome anomaly probably defines a previously undescribed subset of acute nonlymphoid leukemias. Hematologic characteristics commonly found in these patients were dysplastic features of all bone marrow lineages, suggesting that trisomy 4-associated disorders involve the early myeloid hematopoietic stem cell.
The association of pulmonary alveolar proteinosis with hematologic malignancy is well known. The prognosis, in this peculiar setting, is poor. Yet, few patients have been treated in due time. We report a case of pulmonary alveolar proteinosis associated with chronic granulocytic leukemia. The evolution was fatal in spite of intensive treatment.
The aim of this study was to detect the cardiotoxicity of Adriamycin (ADM) by the evolution of the systolic time intervals (STI). The PEP/LVET ratio represents an easy and reproducible index of myocardial function. The more important this increase, the greater the risk of developing heart failure. A significant correlation exists between the variation of this ratio and the total administered dose, but the correlation coefficient is low. A heart failure may appear for doses of ADM under 500 mg/m2 but it is preceded by an increase of the index. In the absence of a significant modification, the generally admitted maximum dose of 550 mg/m2 may be exceeded. In case of a ratio increase in excess of 0.08 it will be necessary to balance the potential benefits of treatment with the hazards of cardiac failure. The PEP/LVET ratio allows proceeding with the cytostatic treatment in increased security by selecting the patients at high risk for cardiac failure.
A phase II protocol associating vindesine and prednisone was used in 47 hematological patients of whom 40 were evaluable. The best results were achieved in relapses of childhood's ALL, with a high proportion of complete remissions and in blastic crises of CML. The efficacy appeared less in relapses of ALL in adults and of malignant lymphoma. The other cases seemed not to respond favorably. The hematological toxicity consisted of leucopenia generally of short duration, but sometimes severe. The neurological toxicity was little and led to reduction of the dosage in only two adults with preexisting neuropathy.

Eleven cases are reported of subacute thyroiditis with histopathological study; there were 9 females and 2 males. Bacteriological studies were inconclusive. Different stages of pathological involvement were observed at the same time in all patients. The clinical course followed the classical pattern in most cases: hyperthyroid-like, hypothyroid-like phase and recovery. Blood TSH assessment before and after TRH stimulation revealed an early phase of depression unresponsive to TRH, followed by high levels with marked stimulation; during the first phase, radioiodine uptake was low, but was enhanced by exogenous TSH administration; accordingly the low uptake seems to be due to low TSH levels and not to complete destruction of the thyroid gland. Failure of TSH levels to rise after TRH stimulation is typical of this stage of the disease. Although the final outcome is not yet predictable in some patients, definitive myxoedema appears to be probable in two cases.
Of 514 patients hospitalized for miscellaneous hematologic diseases, 31 had severe anal lesions (5 per cent); these complications were most commonly observed in agranulocytosis, acute myeloid leukemia, and medullar aplasia. They included infiltration of the perianal area, ulceration, and abscesses. In 20 per cent of the 31 patients, the anal lesion was the first manifestation of the hematologic disease. In all instances, the prognosis of the condition was closely related to the type and severity of the underlying hematologic disease. Surgical therapy, which was applied to the majority of the abscesses, was followed in all instances by rapid symptomatic improvement and was never associated with local or general complications.
Between 1987-1988 we treated, in a cooperative study, 13 patients in blast crisis of CGL according to Koller and Miller's regimen. We observed no return to the chronic phase of the disease, while some patients suffered major drug-induced side effects. Six patients in accelerated phase were treated with the same combination therapy. All of them responded with a return to the chronic phase for a median of 4.5 months. In our hands the combination of plicamycin and hydroxyurea was ineffective in patients with CGL in blast crisis. This regimen could deserve further evaluation in patients with CGL in acceleration.
