Combined hemodynamic effects of dobutamine and IV nitroglycerin in congestive heart failure.

The combined and separate hemodynamic effects of dobutamine and IV nitroglycerin were compared in 12 patients with chronic congestive heart failure (nine with ischemic heart disease, two with idiopathic congestive cardiomyopathy, and one with valvular heart disease). Dobutamine (7.1 micrograms/kg/min) increased cardiac index from 2.4 +/- 0.4 to 3.4 +/- 0.9 L/min/m2 (P < 0.001) and decreased pulmonary wedge pressure from 28 +/- 5 to 16 +/- 4 mm Hg (P < 0.001) while nitroglycerin (127 micrograms/min) alone increased cardia index to 2.8 L/min/m2 (P < 0.001) and decreased wedge pressure to 14.3 mm Hg (P < 0.001). With both drugs, cardiac index increased to 3.5 +/- 0.6 L/min/m2; (NS compared to dobutamine alone) wedge pressure decreased to 11 +/- 4L/min/m2 (P < 0.05 compared to dobutamine alone). Those beneficial hemodynamic effects occurred without a significant change in the double product of heart rate and blood pressure, and were associated with an improvement in the transmyocardial gradient. Thus, the greatly enhanced ventricular performance with dobutamine + nitroglycerin was associated with a better relationship between myocardial oxygen demand and supply.

[Is the process of localized lysosomal exocytosis responsible for the cytolytic action of killer T-lymphocytes?].

In this paper, we formulate the hypothesis that in the process of target cell lysis a lysosomal enzyme regurgitation, performed by killer cells at the level of the target effector junction, accounts for the target lesion which precedes the lysis (lethal hit). This process of exocytosis, similar to the one described previously in polymorphonuclear neutrophils is supported by cytological studies performed directly on identified killers isolated by micromanipulation. Light and electron microscopy observations confirm a previous report which describes the effector cells rich in lysosomal bodies. In addition, when a killer cell is associated with a target cell to form a conjugate, lysosomes are concentrated near the cell junction and, after incubation at 37 degrees C, acid phosphatases may be detected at the junction. Lysosomal enzyme exocytosis explains why target lysis needs an effector target binding to occur and also the other conditions required for any exocytosis process such as Ca++ in the medium, integrity of the microtubular apparatus, a low level of cyclic AMP and energy dependancy.

An enzyme electrode for acetylcholine.

A new enzyme electrode is described to measure continuously acetylcholine concentration. A coating containing active acetylcholinesterase is produced on a pH-glass electrode. The mean thickness of the coating is 50 micrometer. Optimal operational conditions with respect to buffer concentration, ionic strength, linearity, stability, sensitivity, pH of the bulk solution, and response time are studied and discussed. The use of acetylcholinesterase-containing membranes as sensors could offer several novel advantages.

The cervical polyp: a new diagnostic and therapeutic approach with CO2 hysteroscopy.

CO2 hysteroscopy for diagnosis of cervical polyps was performed in 36 patients. The polyps could be located under direct vision and their attachments were found to be deeper than what is seen in regular inspection. A relatively high number of endometrial polyps were found. It is advised that the polyps be surgically removed by cutting the stem at its attachment followed by a curettage.

Sexual function, pregnancy and delivery in spinal cord injured women.

Complete spinal cord injury results in abolition of motor, sensory and autonomic functions. Among the autonomic functions, the sexual system is seriously disturbed. The spinal cord injury with its many complications offers a challenge to the rehabilitation team, and the sexual problem is one of its most important aspects. For the female paraplegic, rehabilitation of sexual functions is an essential part in the building of a positive new self-image. In this review, we try to summarize the gynecological and obstetrical aspects of spinal cord injured women. Although the innervation of the reproductive system is damaged, this system continues to function and the menstrual cycle resumes its normal course after a while. Ovulation takes place regularly and pregnancy is possible. Today the general opinion is that the pregnant paralyzed woman can deliver vaginally and not by cesarean section as was believed before. We emphasize the importance of special medical care before, during and after delivery. The cooperation between physiatrist and gynecologist is mandatory.

[Immunoglobulins (IgG, secretory-IgA) and function of parotid gland in children with cystic fibrosis, with bronchial asthma, and with chronic bronchitis (author's transl)].

The immunoglobulins IgG and IgA (Secretory-IgA) were determined in connection with the parameters of secretion in parotid saliva. Children with cystic fibrosis, with bronchial asthma and with chronic bronchitis were compared. No significant differences were found for the groups of cystic fibrosis, of bronchial asthma, and of chronic bronchitis in comparison to a control group in relation to flow rate and protein content in parotid saliva. The comparison of the groups of diseases showed striking differences only with regard to the output of the gland after stimulation. The output of the gland per minute for IgG was decreased for patients with bronchial asthma as well as for patients with cystic fibrosis. No differences were found for the patients with chronic bronchitis and for the control group. Also the output of the gland for secretory-IgA was decreased after stimulation in the groups of disease: bronchial asthma and cystic fibrosis. The concentration of immunoglobulins in serum no correlation with the concentration of immunoglobulins in saliva neither for IgA nor for IgG. A high content of IgG in serum of patients with cystic fibrosis was observed. The mean values of IgA in serum of patients with chronic bronchitis were found to be significantly decreased.

[Acid phosphatase activity of cytolytic T cells during cytolysis].

The localization of Golgi areas, particularly lysosomal bodies detected by their acid phosphatase activities, has been determined in the cytotoxic lymphocytes conjugated to their target cell. The study points out the preferential situation of these organelles close to the contact area between the two cells during the target cell lysis.

Survival in marriage in the paraplegic couple: psychological study.

In the study of sexual functions and marital relations in 16 couples involving a normal female and a paraplegic male, married following the injury, it was found that a major source of dissatisfaction on the part of the wives was their inability to become pregnant and bear children. Sexual relations were surprisingly of secondary importance. Common in all 16 couples was the almost total lack of knowledge on the part of the woman about the man's sexual capabilities and the effect of his injury on fertility. Premarital counselling seems to be of primary importance in assuring the survival of such marriages. It may be useful to suggest they have premarital experience. Later, counselling should include the importance and advantages of artificial insemination donor treatment as a solution to infertility.

Spinal cord injuries: male infertility aspects.

In a study of 16 married paraplegic patients, it was found that without electrostimulations, erections occurred in eight patients and ejaculation in three of them. Rectal electroejaculation performed in all the patients for the purpose of collecting sperm showed poor results. Sperm analysis revealed severe distubrances in spermatogenesis and spermiogenesis, the etiology of which was not clear since hormonal and sperm metabolic substrates were found to be in the normal range.
