[Bourneville's tuberous sclerosis and genetic counseling. Study of 36 families].

From their own experience, the authors grapple with the difficulties of the genetic counselling in the tuberous sclerosis. They dwell on the necessity of undertaking a very full assessment in the parents of the apparently isolated cases. It includes skin examination, fundoscopy, cranial CT scanning, renal imaging and echocardiography. After these investigations which may need to be done to make or to exclude the diagnosis, it will be possible to quantify new mutations. Nevertheless the exceptional description of pedigrees with incomplete penetrance cannot be forgotten for the genetic counselling. It is likely however that most of these problems will not be resolved until accurate genetic markers are obtained.

The nature of overinclusive thinking in schizophrenia.

Schizophrenics were compared with neurotic controls on four tests of categorical thinking in an attempt to discover the nature of overinclusive thinking. The tests provided both verbal and nonverbal measures of conceptual loosening, the proportion of out-of-category items, distortion of the internal structure of a category and any tendency towards overcategorization. The results showed that schizophrenics were relatively worse on nonverbal than on verbal categories with, in particular, a greater degree of conceptual loosening and a tendency towards overcategorization. The most striking abnormality emerged during observation of the actual sorting strategy itself. Schizophrenics tackled the task in a way which suggests that they lack the ability to form a Gestalt of the task.

Sustained division of protoplast-derived cells from primary leaves of Pinus pinaster, factors affecting growth and change in nuclear DNA content.

Leaf protoplasts were isolated from apical and in vitro-induced axillary buds of Pinus pinaster Ait. seedlings. First divisions were seen after 8-10 days of culture in a 650 mOsm kg H(2)O(-1) medium in which glutamine was the sole nitrogen source. Colony formation was achieved in 6-7 weeks in a modified protoplast culture medium in which a reduction in the concentrations of both calcium and carbon was essential for sustained divisions. To maintain cell suspension growth, it was necessary to subculture every three weeks to a 170 mOsm kg H(2)O(-1) medium. Lowering the C/N ratio did not support better growth. Phenolic compounds were detected in stationary phase cultures. Analysis by HPLC indicated that the cinnamate pathway was involved in their synthesis. After 3 and 7 months of culture, 65 and 74%, respectively, of protoplast-derived cells had a nuclear DNA content comparable to that of leaf protoplasts.

[Is osteosynthesis justified for spinal injuries in children?].

The legitimacy of osteosynthesis for the treatment of spinal injuries in children is analyzed as a function of results in 10 cases, 7 affecting the cervical, 2 the thoracic and one the lumbar region, six of the children being under 10 years of age. Five of these children had serious neurologic disorders, including 4 with high tetraplegia. Technical difficulties of osteosynthesis were not made easier by the poorly adapted nature of the material for infantile statures. Indications for this surgery were not only those cases not improved by orthopedic therapy: instability of lesions, the child's condition and deterioration in the neurologic state can also require initial treatment by osteosynthesis. The principal complication is postoperative cervical kyphosis, but this can be prevented by early active reeducation, the stability of the fixation dispensing with the need for external contention.

Dementia due to meningioma: outcome after surgical removal.

The outcome of operations in nine patients with meningiomas who presented with dementia was studied. Two patients died within 6 months of their operations. Six out of the remaining seven patients improved; two of these patients had normal intellectual function confirmed by psychometry. Our results show that recovery can be expected in most patients who present with dementia due to a meningioma.

Prevalence of otitis media in selected populations on Pohnpei: a preliminary study.

There are no previously published studies on the prevalence of otitis media in Micronesia. As part of an Otitis Media Workshop to train health care workers from the Federated States of Micronesia in the identification and treatment of otitis media, 5-point prevalence studies were conducted to explore the prevalence of otitis media in infants, Head Start, elementary and secondary age students. The results showed an especially high prevalence among the visualized tympanic membranes of the well infants, with 46-47% of the infants showing evidence of otitis media. Of the 1338 ears visualized, 985 or 74% were within the range of normal. Two-hundred and sixteen, or 16% showed evidence of extensive past or mild present disease, with the remaining 10% showing active disease. There were a total of 31 perforations among the 779 persons examined between the ages of two months and twenty-five years, representing 4% of the ears examined. The perforation rate for this population was 39.79 per 1000. The results are discussed as well as their implications for developing areas with limited resources for primary, secondary and tertiary health care.

[Otopalatodigital syndrome. Apropos of 3 female cases].

The oto-palato-digital syndrome about which much has been written on the male subject is analysed for a case of a mother and her two daughters which exhibited the main characteristics of the affliction with the exceptions of deafness and cleft-palate. These two manifestations, quoted in the name of the syndrome will be tackled later like its mode of transmission which still remains obscure.
