Experience with fungal infections affecting the heart in necropsies at an Indian hospital.
Twenty-one cases of fungal infection of the heart were encountered among 4000 necropsies performed in a South Indian hospital over a period of 20 years. All 5 cases of endocarditis due to infection with Aspergillus followed cardiac surgery, while the 4 cases of endocarditis due to Candida affected normal heart valves. Cardiac micro-abscesses secondary to fungal infections elsewhere were caused by Aspergillus in 5 cases and Candida in 4. A fibrosing granulomatous Aspergillus infection extended from the lungs to involve the base of the heart in 3 patients.
Changing pattern of primary glomerulonephritis in a south Indian hospital.
Data were reviewed on 2805 patients with primary glomerulonephritis admitted to a south Indian hospital in 1972-1987. A decrease in the proportion of endocapillary proliferative, crescentic and mesangial proliferative types was noted. There was also a decrease in group A beta-haemolytic streptococcal isolates among the 24,657 throat swab cultures made during the same period. This suggests a decreasing prevalence of nephritogenic streptococcal infections in this tropical region.
How useful is immunofluorescence in tropical renal pathology?
A study of 841 renal biopsies from a south Indian hospital showed that immunofluorescence significantly helped in making a diagnosis in 74 cases. This contribution was most useful in biopsies from patients with nephritic or haematuric presentations; in those whose biopsies showed mesangial proliferative, focal proliferative, or crescentic glomerulonephritis; and in biopsies without changes detectable by light microscopy from patients with systemic lupus erythematosus.
