Incidental pituitary adenomas: a retrospective study.

A retrospective study was performed to study the incidence, morphology, and endocrine activity of incidental pituitary tumors in 500 consecutive autopsied subjects. Forty-two pituitary glands were found to have occult pituitary adenomas. These tumors were more frequently found in patients who died in the sixth or seventh decade of life. There was no obvious sex predominance. Of these patients 48% were obese, 34% were diabetic, and 57% were hypertensive. However, in only one case a pituitary lesion was clinically questioned but specific endocrine functions were not performed. The tumor size ranged from 1 to 15 mm, with 62% of tumors being less than 3 mm. In 34 cases the tumors were located along the lateral aspect of the gland, and 36% of these tumors stained positive for the presence of prolactin. Suspicious or abnormal areas were found in 11 of 17 cases in which skull x-rays were taken. In this article we discuss the factors that may affect the recovery incidence of occult pituitary adenomas. The biologic behavior of these microadenomas, especially those classified as prolactinomas, is unclear, but there clinical effects appears to be relatively benign.

Incidental pituitary adenomas.

Pituitary glands from 500 consecutive autopsies were reviewed and the findings correlated with clinical symptomatology. Occult pituitary adenomas were identified in 42 pituitary glands (8.5%). In only one case was a pituitary lesion clinically questioned, but specific hypophyseal function studies were not performed. These tumors occurred most frequently in the sixth and seventh decade of life, without obvious sex predominance. Of these patients, 48% were obese, 57% were hypertensive, and diabetes mellitus was noted in 11. The tumor size ranged from 1 to 15 mm, but was greater than 1 cm in only one case. In 34 cases, the tumor was located along the periphery of the gland. Even though the etiological and functional significance of these tumors is unclear, the clinical course appears to be relatively benign.

Giant craniopharyngioma evaluated by computerized axial tomography.

The advent of computerized axial tomography (CAT scan) has augmented the neurologic diagnostic armamentarium dramatically. The ability of the CAT scan to differentiate and accurately locate various intracranial densities in a noninvasive manner makes it a very valuable method of investigating craniopharyngiomas. Because they are often composed of cystic, solid, and calcified portions, craniopharyngiomas are very well demonstrated, with the same CAT scan revealing ventricular size as well. A recent and representative case is reported demonstrating the correlation between the CAT scan and the operative findings.

Significance of the lateral canthal tendon in craniofacial surgery.

Craniofacial approaches to the anterior skull base involve manipulation or removal of the supraorbital and frontozygomatic orbital margins. Necessarily, structures within the orbit are detached from the orbital margins. The lateral canthal tendon is attached to the inner aspect of the frontozygomatic process on the orbital osseous tubercle and is essential to the structural fixation of the lateral canthus as well as a check on the mobility of the lateral canthal angle of the eye itself. Our anatomic studies on the lateral canthal tendon as well as specific neurosurgical considerations in its mobilization are reviewed. The detachment of the lateral canthal tendon may result in blunting of the lateral canthal angle, a distraction of the eyelid away from the globe, or an asymmetric repositioning of the canthus as compared with the contralateral angle. It would appear that if the periosteum of the orbit is carefully dissected from the orbital rim and reapproximated following the procedure, the lateral canthal tendon insertion and function will not be disturbed unless the bony orbit margins are altered. In these instances, further ocular plastic surgery may be required.
