Lupus anticoagulant and transient severe bleeding tendency in a girl aged nine years.

A 9-year old girl admitted in our clinic for severe hemorrhagic syndrome was found to display a high level of lupus anticoagulant (LA) leading to an important prolongation of phospholipid-dependent coagulation. Positive antinuclear and anti DNA antibodies, as well as very low levels of complement C3 and C4 proteins confirmed the diagnosis of systemic lupus erythematosus. Therapy with cortisone and cyclophosphamide led to normalization of the clotting tests but could not arrest the development of renal and hepatic lesions. The patient is one of the few cases with presence of lupus anticoagulant associated with severe hemorrhagic diathesis, in opposition to the more frequently reported thrombotic tendency connected with antiphospholipid antibodies.

Clinical studies on plasma antithrombin III using an antiserum prepared in our laboratories.

Using an antiserum obtained in rabbits injected with antithrombin III purified in our laboratories, the plasma level of this inhibitor of coagulation was found to be significantly (p less than 0.001) decreased in 8 patients with decompensated cirrhosis of the liver (15.66 mg/dl +/- 2.22) and in 18 nephrotic patients (21.74 mg/dl +/- 1.75). When compared to values noted in 22 normal-weight normolipidemic control subjects (31.54 mg/dl +/- 0.81), the plasma level of antithrombin III was slightly (p less than 0.05) higher in the 18 hyperlipidemic subjects (36.11 mg/dl +/- 1.9). Owing to an extremely wide dispersion of individual values, the mean level of antithrombin III noted in 16 critically ill postoperative patients (28.16 mg/dl +/- 2.98) did not significantly differ from control values, although this protease inhibitor was obviously decreased in those postoperative patients who developed multiple organ failure or disseminated intravascular-coagulation. Among the 11 patients with recurrent venous thrombosis, two subjects with familial deficiency in antithrombin III were detected. The results provide evidence that the antiserum prepared in our laboratories is a useful tool for detecting acquired and inherited antithrombin III deficiencies.

C5b-9 deposition in children with glomerular diseases.

Kidney biopsies were obtained in 28 children with glomerular diseases and studied using indirect immunofluoreoscence and immunoperoxidase for the detection of IgG, IgA, IgM, Clq, C3c, C4, Fibrinogen and the neoantigens of the terminal C5b-9 complement complex. An affinity-purified rabbit IgG was used to recognize the neoantigens of the assembled terminal components of the complement system into the C5b-9 complex. The immunohistochemical studies were correlated to the clinical data and laboratory investigations. Fourteen of the 28 patients presented specific C5b-9 glomerular deposits; they were also present at the tubular sites in 8 patients and at the vascular sites in 12 patients. An unfavourable evolution was observed for patients presenting C5b-9 deposits in contrast to those without such deposits, even of the same histopathological pattern of glomerulonephritis. The presence of C5b-9 complex at the site of glomerular injury suggests a pathological involvement of the "in situ" complement activation and could be a marker in the prognosis of the glomerular diseases.

Computerized studies of influenza epidemiology in the Sibiu county between 1968 and 1986.

The report presents a statistical synthesis (a microcomputer Junior was used) concerning the incidence of various acute infections of the upper respiratory tract (AIURT), the stages of their evolution, the distribution of cases among the population groups, the seasonal incidence and the values of epidemiologic parameters. We used clinical and epidemiologic data on AIURT of viral etiology (especially influenza). The structure of a data base on the epidemiology of AIURT of viral etiology is also presented.

[Assessment of the epidemiological parameters of the most widespread viral diseases in the Socialist Republic of Romania and in Sibiu County for period 1967-1986].

Essential peculiarities of the kinetics of the morbidity evolution of most frequent virus diseases were explained using a determinant mathematical model. The main epidemiological parameters for the whole territory of the S.R. of Romania and of Sibiu County were evaluated for the 1967-1986 period.

Fibronectin in plasma and synovial fluid of patients with rheumatic diseases.

Fibronectin is a high molecular weight glycoprotein from plasma and other body fluids, connective tissue matrix and basement membranes. No significant differences in the mean values of plasma fibronectin were found in patients with rheumatic diseases compared to control subjects. The fibronectin in synovial fluids in these patients presented higher levels than in plasma. No other protein from the synovial fluid presented such a peculiar behaviour. The synovial fluid fibronectin/plasma fibronectin ratio is 2.49 in patients with rheumatoid arthritis, 1.56 in those with inflammatory nonrheumatoid arthritides and 1.60 in those with osteoarthritis. Statistically significant higher values of synovial fibronectin were found in patients with rheumatoid arthritis compared to those with osteoarthritis. No significant statistical correlations were found between the synovial fibronectin concentrations and the other clinical or biological parameters of the rheumatoid arthritis patients, but for synovial fluid C3. Immunohistochemical localization of fibronectin in the rheumatoid synovium showed more intense and extended specific deposits than in the control patients. These results suggest a local synthesis of fibronectin related to the chronic inflammatory process.

High levels of plasma von Willebrand factor in hyperthyroidism.

Both von Willebrand factor antigen and activity tested as ristocetin cofactor, were found to be increased in the 20 patients with hyperthyroidism, while changes detected on the 10 hypothyroid patients did not significantly differ from the mean values recorded in 19 healthy control subjects. It is considered that the high levels of endothelia-derived plasma von Willebrand factor in hyperthyroidism is the result of a long-standing stimulation of vascular endothelial cells by either a direct effect of thyroid hormones or by an excessive adrenergic activity. The present findings represent an important limitation in interpreting raised levels of plasma von Willebrand factor as a marker of endothelial injury or in terms of a thrombotic tendency.

Increased level of plasma factor VIII-related antigen in peripheral arterial disease.

When compared to age-matched control subjects factor VIII-related antigen was found to be significantly increased in patients with peripheral arterial diseases, while there were no significant differences concerning its plasma level between patients with thrombangitis obliterans and those with atherosclerosis of the lower limbs or between the various stages of the disease. Acute phase reactants such as plasma fibrinogen and alpha 1 antitrypsin were changed to a much lesser extent in the initial stages of peripheral arterial disease, and the afore mentioned antiprotease increased significantly only in stage IV. It is considered that the increased plasma level of factor VIII-related antigen occurring already in the initial stages of arterial disease, should be interpreted as an expression of widespread endothelial lesions and not as the mere result of an acute phase reaction. The relevance of changes affecting factor VIII-related antigen for the development of thrombotic complications could be assessed only in the context of other abnormalities of the hemostatic variables.

Plasma fibronectin in peripheral arterial disease.

When compared to values obtained in 24 age-matched normal weight normolipidemic control subjects, plasma fibronectin level was not significantly changed in the 28 patients with peripheral arterial disease, although endothelial cell-derived factor VIII related antigen was found to be obviously increased. On the other hand the 15 patients with obliterative atherosclerosis of the lower limbs who were hyperlipidemic (mainly hypertriglyceridemic) displayed a plasma fibronectin level (46.3 +/- 2.8 mg/dl) which was higher than the mean values recorded in both control subjects (36.2 +/- 1.66 mg/dl) and in normolipidemic patients with peripheral arterial disease (34.5 +/- 4.14 mg/dl). The results suggest that an increased level of plasma fibronectin should be connected rather to a peculiar pattern of hepatic protein synthesis occurring in overweight and hypertriglyceridemic subjects than to a release of this glycoprotein from an ongoing arterial lesion.

Quantitative determinations of immunoglobulins and complement components in human aortic atherosclerotic wall.

Saline and acid eluates of intima with only fatty streaks, fibrous plaque and intima surrounding the fibrous plaque were obtained from 42 human aortae. IgG, IgA, IgM, C1q, C3c, C4, C9, C3A, C-reactive protein, alpha-1-antitrypsin, alpha-2-macroglobulin, albumin, transferrin and fibrinogen were quantitatively determined in these eluates using the radial immunodiffusion technique. Saline extracted IgG and IgA were significantly higher in the fibrous plaque and adjacent tissue than in the fatty streak intima, where IgM was the highest. IgG and IgA were only present in acid eluates. The complement components were present in all saline eluates, but only traces of C3c and C1q in some acid eluates. Statistically significant differences were found for C1q and C9. C-reactive protein was present in 23 samples. The results could suggest an involvement of immune mechanisms in the progression of the atherosclerotic lesion.

Studies of human cytomegalovirus complement-fixing antigen.

Various techniques are described for the preparation of complement-fixing(CF) antigen from the cell-associated phase of two strains of human cytomegalovirus. Preliminary results regarding the preparation of CF antigen from extracellular virus are also presented. The qualities and shortcomings of each of the biological preparations obtained by the different procedures are outlined.
