[Impact of a PACS/RIS-integrated speech recognition system on radiology reporting time and report availability].

PURPOSE: Quantification of the impact of a PACS/RIS-integrated speech recognition system (SRS) on the time expenditure for radiology reporting and on hospital-wide report availability (RA) in a university institution. MATERIAL AND METHODS: In a prospective pilot study, the following parameters were assessed for 669 radiographic examinations (CR): 1. time requirement per report dictation (TED: dictation time (s)/number of images [examination] x number of words [report]) with either a combination of PACS/tape-based dictation (TD: analog dictation device/mini-cassette/transcription) or PACS/RIS/speech recognition system (RR: remote recognition/transcription and OR: online recognition/self-correction by radiologist), respectively, and 2. the Report Turnaround Time (RTT) as the time interval from the entry of the first image into the PACS to the available RIS/HIS report. Two equal time periods were chosen retrospectively from the RIS database: 11/2002 - 2/2003 (only TD) and 11/2003 - 2/2004 (only RR or OR with speech recognition system [SRS]). The mid-term (> or = 24 h, 24 h intervals) and short-term (< 24 h, 1 h intervals) RA after examination completion were calculated for all modalities and for CR, CT, MR and XA/DS separately. The relative increase in the mid-term RA (RIMRA: related to total number of examinations in each time period) and increase in the short-term RA (ISRA: ratio of available reports during the 1st to 24th hour) were calculated. RESULTS: Prospectively, there was a significant difference between TD/RR/OR (n = 151/257/261) regarding mean TED (0.44/0.54/0.62 s [per word and image]) and mean RTT (10.47/6.65/1.27 h), respectively. Retrospectively, 37 898/39 680 reports were computed from the RIS database for the time periods of 11/2002 - 2/2003 and 11/2003 - 2/2004. For CR/CT there was a shift of the short-term RA to the first 6 hours after examination completion (mean cumulative RA 20 % higher) with a more than three-fold increase in the total number of available reports within 24 hours (all modalities). The RIMRA for CR/CT/MR was 3.1/5.8/4.0 in the first 24 hours, and 2.0 for XA/DS in the second 24-hour interval. CONCLUSION: In comparison to tape-based dictation, an SRS results in a significantly higher primary time expenditure and a modified report dictation workflow. In a university institution, a PACS/RIS-integrated SRS achieves a marked improvement in both short- and mid-term RA which eventually results in an improvement in patient care.

[Bavarian mammography screening program].

In Bavaria since the 1st April 2003 we have been conducting a high quality mammography-screening carried out in individual practises (BMS). We have used the European and the S 3 guidelines. The best diagnosis is an early diagnosis of the breast carcinoma to save human life. Because of this and the high mortality rate due to this disease it is essential to have a mammogram screening program. There is no single one ideal way of constructing a screening program, it is always based on compromise within the particular health care-systems. Arising problems cannot be avoided, it is only possible when all parties work closely together that the BMS works properly.

Effectiveness of 4 pulpotomy techniques--randomized controlled trial.

Pulpotomy is the accepted therapy for the management of cariously exposed pulps in symptom-free primary molars; however, evidence is lacking about the most appropriate technique. The aim of this study was to compare the relative effectiveness of the Er:YAG laser, calcium hydroxide, and ferric sulfate techniques with that of dilute formocresol in retaining such molars symptom-free. Two hundred primary molars in 107 healthy children were included and randomly allocated to one of the techniques. The treated teeth were blindly re-evaluated after 6, 12, 18, and 24 months. Descriptive data analysis and logistic regression analysis, accounting for each patient's effect by a generalized estimating equation (GEE), were used. After 24 months, the following total and clinical success rates were determined (%): formocresol 85 (96), laser 78 (93), calcium hydroxide 53 (87), and ferric sulfate 86 (100). Only calcium hydroxide performed significantly worse than formocresol (p = 0.001, odds ratio = 5.6, 95% confidence interval 2.0-15.5). In conclusion, calcium hydroxide is less appropriate for pulpotomies than is formocresol.

Risk factors for undervaccination against measles in a large sample of preschool children from rural Bavaria.

BACKGROUND: Due to insufficient vaccination rates, measles outbreaks still pose a threat to public health in Germany. The purpose of this study was to identify risk factors for undervaccination against measles. PATIENTS AND METHODS: Datasets from two cross-sectional surveys were analyzed by logistic regression models. The surveys had been done prior to and after an intervention aiming to raise measles, mumps, and rubella vaccination rates among children entering primary school in five Bavarian counties. Subjects were 9,582 children aged 6 to 7 years and their parents. RESULTS: Main independent predictors of non-immunization were doctors advising against vaccination (OR 84.8; 95% CI 19.9-360.4), doctors abstaining from advice (OR 6.3; 95% CI 4.8-8.4), and parental reservations due to alternative health beliefs (OR 27.9; 95% CI 17.6-44.1). CONCLUSION: Doctors advising against measles vaccination and doctors not giving any advice proved to be important risk factors for childhood undervaccination against measles. Future strategies to increase measles vaccination rates in Germany should focus more on family doctors.

Atopic disease and its determinants -- a focus on the potential role of childhood infection.

BACKGROUND: Atopic diseases develop on a genetic background and are modulated by environmental factors among which some infectious diseases are thought to have a protective influence. OBJECTIVE: The aim of this study was to determine the influence of infectious diseases in younger ages, bacterial and viral, on atopic diseases and sensitization to aero- and food-allergens in adults. METHODS: A population-based sample of 4262 subjects aged 25-74 years were interviewed concerning their history of infectious disease within the first 18 years of life. Information about allergic disease, including atopic eczema, allergic rhinitis (AR), and asthma was obtained. A blood sample was drawn and analysed for allergen-specific IgE antibodies against food- and aero-allergens. RESULTS: Multiple logistic regression analyses identified viral infection to be associated with AR (adjusted odds ratio (OR) = 1.39; 95% confidence interval (95% CI): 1.13-1.72) and sensitization to aeroallergens (OR = 1.21; 95% CI: 1.05-1.41). Bacterial disease was a negative predictor for atopy development in the subgroup of patients sensitized to nutritional allergens with concomitant atopic eczema (OR = 0.34; 95% CI: 0.11-0.99), AR (OR = 0.67; 95% CI: 0.42-1.07), or asthma (OR = 0.41; 95% CI: 0.19-0.87). Influences of viral and bacterial infection on AR differed with regard to family history of atopic disease. CONCLUSION: In our study population, history of viral infection was consistently positively associated with AR. Our data suggests that bacterial infections might be preventive for specific subgroups of atopy.

Perceived risk of bovine spongiform encephalopathy and dietary behavior.

The German BSE crisis in early 2001 can be considered as a natural experiment with strong behavioral consequences. The present study investigated psychological and other factors associated with reduced meat consumption compared to the first months of the previous year. As expected, all types of meat, with the exception of poultry and game, were eaten less often. The effect was strongest in beef, where almost half of the sample reported reduced meat consumption. As predicted by the health belief model, perceived threat was associated with subjective vulnerability. It was not, however, strongly associated with perceived seriousness of BSE, probably due to the ubiquitous public discussion of the topic. Reduced beef consumption has three to four times more frequent in the event of subjective threat.

Reproducibility of coronary calcium quantification in repeat examinations with retrospectively ECG-gated multisection spiral CT.

High reproducibility is a key requirement for coronary calcium scoring in follow-up examinations. We investigated the inter-examination reproducibility of calcium scoring with retrospectively ECG-gated multisection spiral CT (MSCT). Fifty patients were examined twice with MSCT. Slices were reconstructed with retrospective ECG gating in the diastolic phase with 3-mm slice width and up to 125-ms temporal resolution. We calculated the Agatston score, calcium volume with and without isotropic interpolation, and calcium mass, and derived the mean and median variability. We investigated the change of variability with use of 3-mm non-overlapping and overlapping increments (2, 1.5, 1 mm). Use of overlapping increment results in considerably reduced interscan variability. We observed a minimum mean variability of 12% and a minimum median variability of 9% for the Agatston score. For volume and mass quantification we obtained a minimum mean variability of 7.5% and a minimum median variability of 5%. Multisection spiral CT enables coronary calcium quantification with high reproducibility in follow-up examinations mainly founded on image data with reduced partial-volume errors due to overlapping increment.

[Health-related lifestyles and personality traits in fathers of children of minor age. Results of a telephone inquiry].

UNLABELLED: 70 fathers were interviewed by telephone, who had at least one child below 18 years of age and lived with or lived apart from their children because of separation or divorce. The questionnaire comprised information on health resources, health-relevant behaviours, sociodemographic measures, and personality traits as described by the 12 dimensions of the Freiburger Persönlichkeitsinventar (FPI). AIM OF THE STUDY: We intended to describe life situation, health-relevant behaviours and personality of separated fathers and to explore potential differences to fathers living in a normal family setting. METHODS: Cross-sectional survey with computer-aided telephone interviews (CATI). RESULTS AND CONCLUSIONS: Age of participants, number and age of children and many lifestyle parameters were similar in both groups. Separated fathers, however, showed a trend towards riskier lifestyles. There were no differences in health situation, but separated fathers did differ in quality of sleep and medication. Separated fathers also had significantly higher scores of three dimensions of personality (psychosomatic disorders, depressivity, emotional lability) and lower scores of the dimension masculinity. Divorce and separation are life events putting an extreme pychosocial strain on fathers. Psychosomatic disorders and depressivity as measured by the FPI show a distinct risk for fathers' health. A psychometric instrument like the FPI seems to be able to identify target groups for individual health prevention strategies.

[Evalutation of health monitoring in Bavaria by computer-assisted telephone interviews (CATI) in comparison to the German National Health Examination Survey conducted in 1998 by the Robert Koch Institute].

Between June 1999 and May 2000, 2051 residents of Bavaria were interviewed on health and health-related behaviour. The objective of the study was to assess the representativeness and validity of data acquired by computer-assisted telephone interviewing (CATI) as compared to results of the German National Health Examination Survey 1998. The following variables showed excellent agreement: subjective health status, lifetime disease prevalences, such as myocardial infarction, stroke, cancer, asthma, diabetes, hypertension, hypercholesterolaemia, utilisation of medical services, and smoking habits (prevalence of present and former smoking, average cigarette consumption per day). Minor differences were found with respect to the frequency of consulting naturopathic doctors. Self-reported anthropometric data from the present study differed considerably from the results of the objective measurements during the German Health Survey. We conclude that, in terms of data quality, CATI compares well with traditional methods such as face-to-face interviewing or mailed questionnaires. In view of several practical advantages of the method, we recommend CATI as a future basic methodology of a German health surveillance system consisting of different coordinated health data collections.

Separation from children as a specific risk factor to fathers' health and lifestyles.

OBJECTIVES: The study was to examine whether fathers living apart from their children following divorce or separation ("fathers without children") differ in their health-related lifestyles and attitudes, and in their health status, from fathers in intact family settings ("fathers with children"). METHODS: Data was acquired by means of a self-administered questionnaire within an exploratory cross-sectional survey. RESULTS: Fathers without children differed in their lifestyle patterns, parameters of satisfaction, health, and health related orientations from fathers with children. Negative lifestyles could be observed in fathers who had a low income and saw their children only rarely. CONCLUSIONS: Separation from their children is a major life crisis for fathers. Subgroups could be identified who had significant health risks. Due to study design, conclusions on causation are not possible. Longitudinal studies are necessary to yield more detailed impact for prevention.

Standardization of non-aggregated data: theory and practice.

Direct standardization of rates is a classical method in epidemiology by which the relationship between confounders and variables of analysis is balanced between the samples or populations to be compared. While the method is appropriate if single variables of aggregated data sets are to be compared, it is limited with respect to handling many variables of different measurement scale, especially in primary data situations. Therefore the method of direct standardization is adapted to the analysis of non-aggregated data. Computation of not only rates but other statistics of central tendency and dispersion as well as confidence intervals hence becomes feasible. In addition it is possible to re-adjust the primary data weights in order to perform stratified analyses. A procedure to do so, based solely on the available original weights without the need for analyzing the standard population is proposed along with an extensive SAS macro. The utility allows for the rapid and standardized analysis and tabulation of large multi-variable data sets. Both the statistical and technical properties of the macro are discussed and its usage is explained as well as exemplified.

[Population-based health monitoring via computer-assisted telephone interviews in Bavaria].

From June 1999 until May 2000, 2051 computer-assisted telephone interviews concerning health and health related behaviours were assessed in Bavarian households. The aim of the study was the evaluation of the validity and representativity of this data by a comparison of selected variables from the Bavarian Mikrozensus-survey 1999. The distribution patterns of the marital status, the size of the households, the gainful employment, the household-netto-income, the school qualification and of the smoking status correspond well with those of the Mikrozensus 1999. As often found in questionnaire based surveys, a disproportionately high rate of participation of highly educated persons was observed. This led to an effect only in terms of a too small rate of persons with a low household-netto-income. Within the study, two different designs were compared. The commitment of the telephone numbers to a gender for an equal quotation of men and women led to an under-representation of single-households and therefore also of unmarried persons, but it had no effect on the other characteristics. The results of this study show, that the method of computer assisted-telephone interviews is a valid and cheap basis for the establishment of an exhaustive health surveillance system in Germany.

Coronary artery calcium measurement: agreement of multirow detector and electron beam CT.

OBJECTIVE: The purpose of our study was to establish the most suitable algorithm to compare coronary artery calcium measurements performed with electron beam CT and multirow detector CT for the assessment of coronary artery disease. SUBJECTS AND METHODS: Coronary artery screening was performed in 100 patients with both electron beam and multirow detector CT. The images were transferred to a dedicated workstation for determination of the calcium score, volume, mass, density, and number of lesions. In addition to the traditional threshold of 130 H, the score of multirow detector CT studies was reevaluated at a threshold of 90 H. Fifty-nine of the patients underwent conventional coronary catheterization. Receiver operating characteristic curve analysis of the different scoring algorithms for detection of significant coronary artery stenosis was performed. RESULTS: The correlation between electron beam CT and multirow detector CT was high for every quantification algorithm. Determination of the score and the number of lesions with multirow detector CT revealed a systematic error of the measurement compared with electron beam CT. The areas under the curve in the receiver operating characteristic curve analyses for electron beam and multirow detector CT were similar for the score, volume, and mass, whereas they were lower for the density. No significant difference was found for the areas under the curve between scores using a 130-H and those using a 90-H threshold. CONCLUSION: Volume and mass indexes are superior to the traditional score, density, and number of lesions for comparing the results of electron beam and multirow detector CT and for determining significant coronary artery disease.

Multislice helical CT of focal and diffuse lung disease: comprehensive diagnosis with reconstruction of contiguous and high-resolution CT sections from a single thin-collimation scan.

OBJECTIVE: We tested breath-held 1-mm multislice helical CT for obtaining both contiguous and high-resolution CT sections of the chest from a single set of raw data. SUBJECTS AND METHODS: Seventy patients with suspected focal and diffuse lung disease were allocated into two groups for comparison. The first group (n = 35) underwent multislice helical CT of the chest with 1-mm collimation and a pitch of 6. From the raw data, 5-mm contiguous and 1.25-mm high-resolution CT sections were reconstructed. The second group (n = 35) underwent conventional single-slice helical CT and high-resolution CT. High-resolution CT sections and 5-mm scans were rated for overall image quality, spatial resolution, subjective signal-to-noise ratio, diagnostic value, depiction of bronchi and parenchyma, and motion and streak artifacts. The 5-mm scans were also rated for contrast resolution and depiction of the heart and vessels. Radiation dose was calculated. RESULTS: We rated 5-mm multislice helical CT superior to 5-mm single-slice helical CT, having a significantly higher total score (p = 0.0001). No significant difference (p = 0.986) was found between multislice and single-slice high-resolution CT sections. Radiation dose was 5.55 mSv for multislice helical CT and 5.50 mSv for single-slice helical CT. CONCLUSION: Contiguous chest scans of superior quality and high-resolution CT sections of equal image quality compared with single-slice helical CT can be obtained using multislice helical CT. Therefore, a comprehensive diagnosis is feasible in patients with suspected focal and diffuse lung disease by obtaining a single scan.

[Incidence of operations in Germany via telephone survey in 4 selected cities].

Considerable regional variation of surgical procedure rates has been reported for a number of countries. Influential variables that have been discussed are patient-related, physician-related and health care system-specific factors. A representative computer-assisted telephone survey was conducted in four selected regions for a study sponsored by the German Federal Ministry of Health. These regions were the townships of Aachen and Hamm in North Rhine-Westphalia and Chemnitz and Görlitz in Saxony. During the telephone survey 1897 persons were contacted and 1041 interviews completed. Multiple logistic regression showed a surgical procedure frequency which was by 40% higher in Aachen than in Hamm. During further analysis the regional localisation was replaced by region-specific structural health care information variables. During this analysis the regional frequency of medical specialists was significant at a 5% level (odds ratio: 1.07; 95% confidence interval: 1.02-1.12). This result can be interpreted as an increase in surgical procedures by 7% for each additional specialist per 10,000 persons. However, no definite conclusion can be reached based on the available data. Intensifying the presented survey-based health services research has the potential to identify regional over- or undersupply of medical services, to objectify and accompany informational, administrative or political action and thereby to support equity in access and health care in the sense of an optimized allocation of resources.

Quantitative MRI in CADASIL: correlation with disability and cognitive performance.

OBJECTIVE: To study correlations between total lesion load on brain MRI and clinical features, and to evaluate the influence of demographic variables on quantitative MRI variables in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). BACKGROUND: CADASIL is a hereditary form of small-vessel disease caused by mutations within the Notch3 gene. MRI abnormalities have been found both in asymptomatic and symptomatic CADASIL individuals. METHODS: Quantitative measurements on cerebral MRI were performed in 64 CADASIL individuals. MRI lesions were quantified using a semi-automated segmentation technique based on local thresholds. RESULTS: MRI total lesion volume correlated significantly with disability (Rankin Scale) on both T1- and proton density (PD)-weighted images. There was a significant inverse correlation between total lesion volume and overall cognitive performance as determined by the Mini-Mental State Examination. Age but not sex was correlated with lesion load both on T1- and PD-weighted images. There was no detectable influence of the Notch3 genotype on quantitative MRI variables. CONCLUSIONS: This study demonstrates correlations between MRI lesion volume and clinical characteristics in CADASIL. Longitudinal studies are now warranted to investigate whether quantitative MRI could be used as an adjunct outcome measure in future therapeutic trials in CADASIL.

Automated exploration of two-level interrelations of differently-scaled variables.

The SAS macro EXPLORE is conceived as an exploratory data analysis tool. Its output is an extension to the traditional correlation matrix, allowing the assessment of both categorical and numerical data 'at one look'. Depending on the type of data, appropriate statistics and graphs are produced. An overview of the features of the program and their implementation is given. Program invocation and execution are explained. Potential misuses are discussed.
