Local and systemic reactions to puncture injuries by the sea urchin spine and the date palm thorn.
Puncture wounds were cuased in 9 patients by sea urchin spines and 1 patient by a date palm thorn. All 10 patients developed local inflammatory reactions, which were moderate to severe in 7. Two patients exhibited severe systemic illnesses, and a marked synovitis was present in 2 others. Several of the patients presented as difficult diagnostic problems with signs and symptoms suggestive of an arthritic disease. Six required surgical removal of the foreign body.

MR findings in peroneal tendonopathy.
OBJECTIVE: The purpose of this presentation is to familiarize the reader with the often subtle findings of peroneal tendon derangement on MRI examination. MATERIALS AND METHODS: We present four patients who were studied with MR (1.5 T) prior to surgical exploration of the peroneal tunnel. All patients had lateral ankle symptoms for > 6 months. Normal MR anatomy and an example of the peroneus quartus tendon are also shown. RESULTS: Surgery revealed two peroneus brevis and three peroneus longus lesions. A "multipartite" appearance of the peroneus tendon on axial MR images, especially when paired with a "flame-shaped" appearance on more distal axial images, indicates a longitudinal tendon split. Both patients with peroneus brevis tendon splits had insufficient superior peroneal retinacula. CONCLUSION: Partial ruptures of the peroneal tendons characteristically take the form of longitudinal tendon splits. Axial MR images through the ankle and hindfoot can help distinguish peroneal tendonitis from longitudinal tendon splits.
Management of the arthritic forefoot.
Rheumatoid diseases frequently involve the forefoot producing severe hallux valgus and dislocated metatarsophalangeal (MTP) joints. One must stress the importance of continual evaluation of the foot in these patients. The relationship of midfoot and hindfoot deformities, especially pronation deformities, to the forefoot is essential. Nonoperative treatment can be highly effective and consists of skin and nail care and proper shoes which enclose and support early deformities. Operative care now spans the gamut from excising all or part of the MTP joints, to silicone implant arthroplasty of the MTP joints.
Strength of fixation constructs for basilar osteotomies of the first metatarsal.
Twenty-four pairs of fresh-frozen human feet had a proximal osteotomy of the first metatarsal that was fixed using either screws, staples, or K wires. Each metatarsal was excised and the specimen was loaded to failure in a cantilever beam configuration by applying a superiorly directed force to the metatarsal head using an MTS servohydraulic test machine. Specimens with a crescentic osteotomy that were fixed using a single screw demonstrated higher mean failure moments than pairs that were fixed with four staples or two K wires; staples were the weakest construct. All specimens fixed with staples failed by bending of the staples without bony fracture; all K wire constructs but one failed by wire bending. Chevron and crescentic osteotomies fixed with a single screw demonstrated equal bending strengths; the bending strength of an oblique osteotomy fixed with two screws was 82% greater than for a crescentic osteotomy fixed with a single screw. Basilar osteotomies of the first metatarsal are useful in correcting metatarsus primus varus often associated with hallux valgus pathology. Fixation strength is an important consideration since weightbearing forces on the head of the first metatarsal acting at a distance from the osteotomy site subject the construct to a dorsiflexion bending moment, as simulated in our tests. Our results show that screw fixation is the strongest method for stabilizing a basilar osteotomy. Based upon the relatively low bending strengths of the staple and K wire constructs, we would not recommend these forms of fixation.(ABSTRACT TRUNCATED AT 250 WORDS)

A prospective comparative clinical analysis of the first-generation knee replacements: polycentric vs. geometric knee arthroplasty.
A prospective study of 119 polycentric and 92 geometric knee replacements was performed to determine and compare the clinical effectiveness of these two prostheses. All kneex were followed for a minimum of 2 years and a mean time of 3 1/2 years (2--6 years). Data were collected using a specially designed proforma for subsequent computer analysis. Failure occurred in 11% of the polycentric and in 16% of the geometric knees. Males (8 of 47) and patients with osteoarthritic knees (22 of 68) failed most frequently. Both prostheses provided excellent relief of pain, the same degree of flexion and improvement in flexion contracture. However, walking, function, alignment, stability, muscle strength and patellar mobility varied as to the degree of improvement and the type of prosthesis. Present results with prosthetic knee replacement using a completely new operative technique can be used as a basis for comparison with other contemporary and future arthroplasty designs.
