Nephrocalcinosis and prematurity: importance of urate and oxalate excretion.

Nephrocalcinosis was described in preterm infants by several authors who tried to determine its association with hypercalciuria and furosemide therapy. We evaluated these potential mechanisms along with other lithogenic factors not previously studied in 10 premature babies. Hypercalciuria was an inconsistent finding like in other reports; elevated uric acid excretion and hyperoxaluria were observed in 5 and 6 cases, respectively. The aminocid excretion was normal in all infants. Our data suggest that in addition to hypercalciuria, other lithogenic factors may play a role in the pathophysiology of nephrocalcinosis of premature infants.

[Sertoli-Leydig tumors in children. 2 case reports].

Sertoli-Leydig tumors stem from the mesenchyma and sexual cords of the embryonic gonad. Two cases are reported. One manifested as symptoms of virilization in a 12 year old girl. The other patient developed adnexal torsion at the age of five years. Pelvic ultrasonography visualized the tumor in both cases. Increased production of ovarian androgens suggested the diagnosis in the first case. Histological studies disclosed intermediate differentiation in the first case and tubular differentiation in the second. These tumors usually exhibit low-grade malignancy and unilateral salpingo-oophorectomy ensures recovery in most instances.

[Severe colectasy in rheumatoid purpura. Probable role of water soluble contrast products].

BACKGROUND: About 60-70% of cases of anaphylactoid purpura suffer from abdominal colic, which may be quite severe. Because severe abdominal pain may be difficult to differentiate from a surgical abdomen, repeated examination and radiological studies are often necessary. CASE REPORT: A 4 year 7 month-old girl suffered from abdominal pain, vomiting and the skin rash characteristic of anaphylactoid purpura. Because of worsening of the abdominal manifestations, a Gastrografin enema was given. The result was normal but 48 hours later, the child suffered from acute abdominal colics plus symptoms of shock. X-ray examination revealed a large distension of colon and persistence of the contrast material. Exploratory laparotomy failed to find any perforation or necrosis. The colon was drained and the child was given parenteral nutrition and antibiotics. Blood culture showed Klebsiella pneumoniae. An upper gastrointestinal roentgenogram using meglumine ioxitalamate was performed 19 days later because of recurrence of the abdominal pain. This showed a submucosal hemorrhage in the first small bowel loop. Increased abdominal distension occurred 3 days later and the results of X-rays were similar to those seen the Gastrografin enema. A second enema using meglumine ioxitalamate was performed 3 days later. It showed sigmoid constriction but a second laparotomy failed to confirm this obstruction. The child died a few hours later despite ileostomy and antibiotics. CONCLUSIONS: The severe colectasy seen in this case of anaphylactoid purpura indicates that caution is required in performing enemas in acute digestive complications of this disease. Such X-ray studies should not be repeated and all water-soluble contrast material should be voided as soon as possible.

[Post-traumatic lung pneumatocele. A case in an infant].

Traumatic lung pseudocyst is a lung injury due to closed chest trauma. The authors report one case in a 20 month-old boy. This cavitary lesion is underestimated since associated lesions often mask the pneumatocele. Its frequency is higher in children and young adults. Diagnosis can be assessed early when chest film shows the characteristic cavitary lesion. Computed tomography may be helpful in the recognition of paramediastinal traumatic lung pseudocyst. Since the lung pseudocyst usually involves, no aggressive therapy is justified.

[Value of MRI in various forms of idiopathic growth hormone deficiency].

Magnetic resonance imaging (MRI) of the sellar and juxta sellar area was performed in 40 children with idiopathic hypopituitarism. MRI was normal in the 17 patients with partial growth hormone (GH) deficiency and in the 5 patients with GH neurosecretory dysfunction. Among the 18 patients with complete GH deficiency, the 10 with pituitary stalk transection presented more often with perinatal abnormalities, had the most severe GH deficiencies and more frequently multiple hormone deficiencies, in particular TSH deficiency. Pituitary gland size studies showed that the small size of the residual glandular tissue was responsible for these particularities. If MRI is of no great interest for functional GH deficiency, in half of the primary complete GH deficiencies, it enables to visualize a pituitary stalk transection with regeneration of neurohypophysis.

[Mediastinal extension of retropharyngeal abscess].

Retropharyngeal abscesses in children are unusual and the involvement of the mediastinum is exceptional since the era of the antibiotics. It can be explained by the anatomic continuity between the retropharyngeal space and the retrooesophageal space. We report a case where the CT Scanner allows a complete evaluation of the disease in the mediastinum and a good follow-up.

[Cerebral cysticercosis: imaging, serology, treatment and evolution. Apropos of 2 cases].

The authors present two cases of neurocysticercosis diagnosed both by serology and medical imaging (computed tomographic scan and magnetic resonance imaging). The first patient had a mixed form with localization of the parasites in the different anatomic parts. He developed suddenly a severe encephalitis which growth has been quickly fatal in spite of a prompt praziquantel and corticosteroids treatment. The second patient had a parenchymal form, and the same treatment was very effective.

[Congenital tracheal stenosis associated with aberrant left pulmonary artery. Apropos of a severe form with neonatal disclosure].

The case report is presented of a newborn infant afflicted with extensive congenital tracheal stenosis associated with anomalous left pulmonary artery, proving rapidly fatal. The possibility of tracheal anomaly must be envisaged in the face of any neonatal respiratory distress unexplained by pulmonary parenchymatous pathology. Diagnosis of this complex malformation relies on endoscopy and oesophagogram and tracheo-bronchogram contrasts.

[Hypochromic anemia and small bowel diverticula].

The authors appreciate the possible responsibility of small intestine diverticulosis in three patients with iron deficiency anemia. In the first case, anemia was associated with colo-duodenal fistula. In the second and third cases, because of the negativity of gastric and large bowel investigations, a barium opacification was done, which showed diverticulosis. This was considered as probably responsible for the anemia. After a review of the literature, the authors conclude that small intestine diverticulosis may lead to iron deficiency anemia through chronic blood loss.

[Gastric bezoar in diabetes mellitus. 3 cases (author's transl)].

Diabetic gastroparesis is the gastric manifestation of diabetic autonomic neuropathy and may result, on rare occasions, in the formation of a bezoar. This was the case in 3 patients (two women aged 36 and 66 and a 19-year-old man) whose insulin-dependent diabetes was complicated with neuropathy. All patients had marked glycaemic instability apparently related to digestive function. In one patient, a stable normoglycaemic state was obtained by continuous intravenous administration of carbohydrates until the bezoar had disappeared. In the other two patients, who had a long history of neglected digestive disorders, the bezoars provided intractable. In diabetics with dyspeptic symptoms, and particularly when neuropathy is present, it would be advisable to investigate for gastroparesis in order to prevent the development of a bezoar by dietetic and therapeutic measures.
