[Peripheral arterial embolism. Considerations on 101 cases (121 emboli)].

The treatment of 101 patients with peripheral arterial embolism between 1970 and 1976 is discussed. Embolectomy was performed 98 times in 78 cases, antigoagulant and fibrinolytic management was employed in 19 cases, and primary amputation was necessary in the remaining four subjects. The limb was saved in 70% of those operated, while secondary amputation proved necessary in 14%. Overall mortality (after embolectomy and amputation) was 22%. Lastly, the therapeutic protocol used is discussed, together with the various factors that still prejudice the results of embolectomy.

[Personal experience with gastric resection in the treatment of gastroduodenal ulcer].

Billroth I or II resection was adopted in the treatment of 85 subjects with simple or complicated gastroduodenal ulcer. Operative mortality was 1.4% in surgery of choice and 18.7% where emergency management was required to combat perforation of serious haemorrhage. Follow-up after 1-2 yr showed that marked benefit had been obtained from the operation in 83%, while partial success was achieved in 15,5%. Postoperative peptic ulcer was observed in one case only. Comparison with a series of 120 patients subjected of choice or in emergency to truncal vagotomy and gastric drainage during the same period for duodenal ulcer and followed for 2-6yr (no operative deaths and complete success in over 91%) made it clear trat, while resection was attended by a smaller number of recurrences (1.5% as opposed to 2.9%), this advantages was obtained at the price of a greater operative risk and by no means negligible long-term sequelae.

[Personal experience with vagotomy in the treatment of duodenal ulcer].

Truncal vagotomy was employed in 120 cases of simple or complicated duodenal ulcer. Pyloroplasty was also performed in 98 cases, antroduodenostomy in 5, Billroth I or II in 12, and gastrojejunostomy in 5. No operative deaths were noted, even though the series included emergency situations and patients in poor condition. The incidence of complications and immediate sequelae was insignificant. Follow-up for 2-6 yr showed complete success in over 91%, with partial improvement in 3.8%. Poor results included 3 cases of recurrence, 1 of late scar stenosis on the pyloroplasty site, and 1 of intermittent gastroplegia.

Transformation of normal follicles into thyrotropin-refractory "cold" follicles in the aging mouse thyroid gland.

Autoradiographs of thyroid glands of aging mice demonstrate the gradual appearance of "cold" follicles which fail to iodinate the intraluminar iodoproteins even after intense exogeneous or endogeneous TSH stimulation. "Cold" follicles first appear at the age of 5 months. They may account for 80% of all thyroid follicles in 13-month-old mice. Morphologically, the "cold" follicles are characterized by a larger than normal colloid volume and a comparatively flat epithelium for any given follicle size. Old thyroids are twice as large as young ones. They contain twice as much normally iodinated thyroglobulin/mg wet weight. The total number of cells per gland remains constant throughout the life time. This is probably also true for the total number of follicles. The iodide pump, as judged by the tissue to serum ratio, remains normal and TSH-responsive. Yet, it is not established whether this is due to a higher activity of the pump in normal follicles or to preservation of inorganic iodide transport in "cold" follicles. Pinocytosis is defective in "cold" follicles and is poorly responsive to TSH stimulation. Furthermore, diffusion of iodocompounds is severely impaired in the colloid of "cold" follicles. It is suggested that the primary defect in the pathogenesis of "cold" follicles in old mice is the gradual failure of endocytosis to respond to normal TSH stimulation. Because exocytosis and iodination would first proceed normally, the follicular lumina would become overdistended up to a point where the apical membrane is functionally impaired. With this sequence of events, "cold" follicles would have impaired iodination while being metabolically as active as hot ones. Thus, several puzzling reports on high metabolic activity of "cold" tissue could possibly be interpreted.

Iodotyrosine deiodination in the normal and acutely TSH-stimulated thyroid.

The deiodination of L-MIT-125I was measured in rat thyroid homogenates and slices before and after acute TSH stimulation. Slices and homogenates were incubated with identical concentrations of tissue and substrate in the presence and absence of NADPH. 1 USP unit TSH added in vitro to thyroid slices failed to stimulate deiodination; a single in vivo ip injection of 3 USP units TSH was also unable to raise deiodinating activity. In contrast to TSH, NADPH added to homogenates and slices enhanced deiodination significantly. However, several arguments, including a review of the literature, strongly militate against the hypothesis of an increased intracellular concentration of the coenzyme NADPH being the prerequisite to enhanced deiodination. The results suggest that deiodinase activity in acutely stimulated thyroids is not limited by the intracellular concentration of the enzyme itself nor by the availability of co-enzyme. Therefore, the increased iodide release induced by acute TSH stimulation is a mere consequence of the enhanced thyroglobulin proteolysis and does not require higher enzyme concentration. It will be shown subsequently that a different conclusion must be drawn in experiments with chronic TSH stimulation.

Hexose shunt dehydrogenase activity in leukocytes isolated from bovine milk.

A spectrophotometric method for measuring small changes in light absorption at specific wavelengths from highly turbid suspensions of biological material (dual-beam spectrophotometry) has been used to measure glucose-6-phosphate oxidation and nicotinamide adenine dinucleotide phosphate (oxidized) reduction catalyzed by lysates of milk leukocytes. This reduction was correlated .84 with cellular protein concentration and, therefore, with cell concentration.
