[Clinical and hormonal development of a case of malignant exophthalmus after hypophyseal myskiotomy].

A pituitary myskiotomy was performed in a 26-years old man with Graves' disease and progressive exophtalmus. At the onset of Graves' disease a high level of serum TSH, LATS and thyroid hormones was found. Following pituitary miskiotomy a stationary exophtalmus and other signs of Graves' disease have been recorded. The serum levels of LATS and thyroid hormones were high. The serum concentration of TSH decreased to a level undetectable even after the TRH stimulation test. It is discussed the pathogenesis of Graves' disease: its onset due to neuroendocrine disturbances (S. M. Milcu) and its evolution due to autoimmune mechanisms.

The inhibitory action of the pineal extract on TRH.

The effects of a single dose and of 5 doses (5 days) of a bovine polypeptide pineal extract (PE) on the pituitary TSH reserves and on the blood level of thyroid hormones were studied on hypothyroid patients. The blood levels of PBI, total T4 (radiocompetitive), free T4, T3 and TSH (RIA) in basal conditions and during the TRH test (i.v. administration of 200 mug TRH), In the presence and absence of the treatment with PE were determined. Thirty minutes following i.m. administration of PE a decrease was noticed in the high level of the basal TSH in all the patients as well as maintenance or even exaggeration of the TRH response. After 5 days of PE administration a decrease was noticed in the basal TSH, with preservation of a strong response of the TSH release after TRH administration. In these patients a slight decrease was observed in PBI, total and free T4 and T3, when their initial level was close to normal. The results show the results show the intervention of the pineal extract by inhibiting the endogenous TRH.

Hyperthyroidism and pregnancy. I. Serum TBG level in normal pregnancy.

Methods for preparation and qualitative evaluation of the reagents necessary for direct radioimmunoassay of thyroxin-binding globulin (TBG) in serum are described. The reagents are: purified TBG necessary as antigen in preparing the labelled product and plotting the reference curve; 125I-TBG; anti-TBG rabbit serum. The technical stages characteristic of the RIA system are presented. TBG level determined in normal volunteers (about 80% males and 20% females, young subjects living in Bucharest area) was found to be 15.6 +/- 2.23 mg L-1 (M +/- SD) and the PBI, T4, and T3 values at the onset were normal. In another lot formed only from women TBG was 17.3 +/- 3.50. Assay of 136 pregnant women between the 2nd an 9th month of pregnancy showed values of 22.46 (2nd month) and 32.23 mg L-1(8th month). The shape of the increase curve during pregnancy shows two main stages. The importance of the serum TBG level as an additional criterion in the diagnosis of pregnancy (even in the first weeks of life of the foetus) and as an indicator of the placentar function is discussed.

Experimental studies concerning the mechanism of the thyroid-pineal interrelationship. I. The action of the pineal peptides on the thyroid gland.

Subcutaneous administration of a pineal polypeptide extract, "Crinofizin", to adult male Wistar rats having the thyroid function inhibited by prolonged administration of MTU, has important effects on both the whole organism (slightly correcting the weight increase rate, loweri g cholesterolemia) and the thyroid gland (reducing thyroid hyperplasia and playing a part in various stages of thyroid hormones metabolism). Thus under pineal treatment one can observe a clear tendency to return to the normal values of the thyroid parameters strongly modified by MTU, as RIC, intrathyroid proteic iodine, tissue thyroid hormones distribution, blood PBI and total T4. It appears that these effects of pineal peptides are induced directly on the thyroid and/or via the hypophyso-thyroid axis which they intercept and equilibrate (moderate).

Total blood thyroxine and TSH levels in the newborns of an endemic goiter area in Romania.

A neonatal screening of two approximately equal groups (376 in all) from (A) Bucharest (a non-endemic area) and (B) Cimpulung (an endemic area) was undertaken. RIA of T4 and TSH in dry whole blood spots was performed using blood collected 3--5 days after birth. The results show a significantly lower T4 valu in group (B) as against group (A), i.e. 105.5 +/- 3.61 nmol. L-1 as against 146.3 +/- 1.61 nmol. L-1, and a significantly increased TSH value, i.e. 19.1 +/- 0.76 microU. ml-1 as against 10.9 +/- 0.84 microU. sml-1. No sex-related differences were noticed. Our findings point to a neonatal thyroid hypofunction in the endemic area and to the need to establish the range of variation for the thyroid parameters used as criteria for congenital hypothyroidism in endemic areas. Iodine prophylaxis applied to pregnant women in such areas is also discussed.

TSH and thyroxine in cord blood of subjects from an endemic area.

Thyroxine (T4) and TSH were assayed in the triplet: maternal venous blood at delivery (M), cord blood (C) and total capillary dry spot blood of the corresponding children 4 - 5 days postpartum (Nb). Fifty-four triplets in all (spontaneous deliveries, full term infants from the Cîmpulung endemic goiter area) were thus determined by sampling out the native families. The controls were 10 triplets from Bucharest, a non-endemic goiter area. A group of 31 pregnant women from the endemic area who have received iodinated oil in the 3rd - 4th month of pregnancy for prophylactic purposes are brought into discussion. The mean thyroxine and TSH values in C were by 29% lower and 60% higher, respectively, as against the values found for the Bucharest subjects. The statistical analysis of the dependency of Nb thyroxine on the C values showed the existence of a close positive relationship between the two parameters (r = 0.81; p less than 0.001). Worth pointing out are the positive effects of iodinated prophylaxis in pregnant women, as demonstrated by the improvement of the thyroid parameters in C, i.e. 5 - 6 months after administration.

Hyperthyroidism and pregnancy. II. Thyroid function in normal pregnancy and in pregnancy associated with hyperthyroidism.

Starting from the diagnostic difficulties when hyperthyroidism is associated with pregnancy, PBI, total T4 and free thyroxin, triiodothyronine (T3), rT3, TBG, TSH and LATS were assayed in a group of euthyroid pregnant women (136) more or less equally distributed between month 2 and 9 of pregnancy; the same tests were applied to 14 pregnant hyperthyroid women (month 2-3), the latter also undergiving a test for urinary elimination of catecholamines. The results were referred to the normal values of techniques employed. The work presents the alterations in the thyroid status parameters when pregnancy is associated with hyperthyroidism, as against normal pregnancy, pointing out the need for performing a complex set of assays "in vitro" in order to establish correctly the diagnosis of hyperthyroidism in pregnancy and apply the right therapy at the right moment. Increased T3 and free T4 levels in hyperthyroidism referred to an increased TBG level in euthyroid pregnancy are considered as discriminators.
