Long-term follow-up of patients receiving aprindine. Safety and efficacy.

Aprindine hydrochloride is an antiarrhythmic agent presently undergoing clinical trials in the United States. Because of the narrow therapeutic-toxic ratio observed for aprindine, the long-term follow-up of these patients is important in determining the potential clinical effectiveness of this drug. In this report we examine our experience with 30 patients with drug-resistant arrhythmias who were discharged receiving aprindine and who were followed up for a mean period of 25 months.

Abnormal cointegrate structures mediated by gene B mutants of phage Mu: their implications with regard to gene function.

A system where the transposition of MupApl (a derivative of phage Mu carrying a determinant coding for ampicillin resistance) is followed from the small plasmid pML2 into the conjugative plasmid R388 has been used to investigate the influence on Mu transposition of B, an early Mu gene which is involved in normal phage DNA synthesis. In the absence of active B protein a low level (about 1% of normal) of transposition was detected. Roughly a third of these transpositional events was found to lead to the formation of cointegrate DNA structures which were shown to consist of R388, two complete copies of Mu and part only of pML2. The pML2 deletions vary in size but all those investigated appear to originate at an end of Mu. An explanation of these observations is proposed which envisages the B protein as part of the normal transposition complex.

Fibrous dysplasia and the temporal bone.

The cause of fibrous dysplasia of the bone is obscure. We reviewed the available literature concerning temporal bone involvement and analyzed temporal bone involvement and analyzed the most common aspects of this fascinating disease, adding three cases of our own. A clinical staging is proposed: stage 1, latent; stage 2, symptomatic; and stage 3, complication. Clinical evolution and follow-up are described and treatment is suggested.

Carotid endarterectomy without temporary intraluminal shunt. Study of 309 consecutive operations.

A prospective study was undertaken to determine the efficacy of performing carotid endarterectomy without an intraluminal shunt. During a two-year period, 240 patients, ranging in age from 36 to 89 years, underwent 309 consecutive carotid endarterectomies. The indication for operation was transient ischemic attacks in 151 (63%) patients, asymptomatic carotid bruit in 67 (28%), and previous stroke in 22 (9%). Internal shunts were not used in any patients and all arteriotomies were patched with a preclotted knitted Dacron velor patch. Systemic heparinization was used during the procedure. The early postoperative mortality was 0.64% (2/309). Both deaths were caused by myocardial infarction. The incidence of stroke after operation was 1.29% (4/309). Neither carotid clamp time nor the presence of contralateral disease correlated with the occurrence of postoperative stroke. According to results of angiography, 22 patients had total occlusion of the contralateral internal carotid artery with satisfactory intracranial circulation. No postoperative strokes occurred in this subgroup. Results of this study revealed that equally good or superior results may be obtained without a temporary shunt in performing carotid endarterectomy.

Reoperation for recurrent coronary artery disease: causes, indications, and results in 168 patients.

Of the 13,049 patients who underwent coronary artery bypass surgery, 168 underwent a second or third operation because of the failure of the first. The early mortality was eight of 168. Good to excellent results were obtained in 141 patients. The cumulative survival rate was 82% at six years. Graft failure and the progression of atherosclerosis to a critical lesion were the most common reasons for reoperation, and perioperative myocardial infarction and incomplete initial operation were the least common. Long-term follow-up showed a peak of graft failure and progression of disease in nongrafted arteries at three years and decreased thereafter. Progression of distal disease in grafted arteries was not temporally related and was uncommon. Thrombosis of the vein graft could be related most frequently to the distal circulation rather than the vein graft itself.

Genetic investigations in a Northern Brazilian island. II. Random drift.

18 albinos were born on Lençóis island. Since 3 of them died and 5 emigrated, the prevalence of albinism is about 3% in the island. 2 inbred brothers with brachydactyly of the index fingers and 1 case of 'achondroplasia' were also found. The analysis of the population structure of the island suggests that its high frequency of albinism may have been produced by random drift. The index of isolation of its present population is roughly 17--27. Genealogical, clinical and histological data are presented and anslyzed. Natural selection is not acting against the albinism gene at a measurable rate. Cytogenetic investigations among albinos and normals did not reveal any difference as regards frequency of aneuploidy, association of acrocentrics and chromatid gaps. The role of random processes in evolution is mentioned and the possibility that they may be more important than it is sometimes assumed is stressed.
