Bullous dermatosis and myeloma. Monoclonal anticytoplasmic antibody activity.

A case of a recurrent bullous eruption in a patient who had myeloma is reported. This eruption was differentiated from pemphigus and bullous pemphigoid by the histological and immunological findings. In addition, antibodies against the cytoplasm of basal cell layer cells in the human epidermis were detected in the patient's serum and in diseased skin. The myelomatous monoclonal protein and the antibody fixed on the cell cytoplasm had an identifical isotypical marker.

Impairement of thymus derived rosette forming cells during photochemotherapy (psoralen--U.V.A.).

Lymphocyte subpopulations were studied in 45 PUVA treated patients and in 45 controls matched for sex and age. The results demonstrate that, in vivo PUVA induces a decrease in the capacity of the E-Rosette formation by lymphocytes separated from the peripheral blood. However, the circulating lymphocytes count remains unchanged. Mitogen induced blastogenesis is not affected. No correlation is found between the decrease in the level of E-rosettes, the disease which motivated PUVA therapy, the energy in UVA and the number of irradiation. Electrical and/or chemical modifications on the cell surface of the lymphocytes during PUVA therapy are postulated to explain this phenomenon.

[Evaluation of prognostic factors in malignant melanoma according to the actuarial method: report of 75 cases (author's transl)].

The survival rate of 75 patients with malignant melanomas has been calculated according to the actuarial method. Various parameters have been evaluated such as sex and site distribution, histologic data and levels of invasion. The actuarial method appeared to be the more accurate way to appreciate the percentage of survivors in a high-mortality group. Thus, although the incidence of melanoma was higher among females, their prognoses were better than those of males. The localisations on the trunk and the lower extremities were those with the poorest prognosis, nodular melanomas had a survival rate below those of superficially spreading melanomas. Level III of invasion should be regarded as a hinge in terms of lymph node metastasis and survival.

[Immunopathological study of Gougerot-Ruiter's vasculitis (author's transl)].

The authors report 10 cases of selected Gougerot-Ruiter's disease and 13 cases of various vasculitis. Direct immunofluorescence studies were performed on the skin lesions and the results were correlated to various biological parameters such as dosage of serum IgA, IgG and IgM, antinuclear antibodies, Australia antigen. Complement (beta-1-C/beta-1-A) deposition was found in 9 out of 10 cases of Gougerot-Ruiter's disease as opposed to immunoglobulin deposition found only in 4 out of 10 cases. Both C3 and immunoglobin were regularly found in the vessel walls of 2 cases of cryoglobulinemia and 1 case of SLE. In the cases of thrombopenic purpura, simplex purpura. Schoenlein Henoch purpura and drug induced purpura, complement (beta-1-C/beta-1-A) and immunoglobulin were absent in the lesions. The authors discuss the different physiopathological mechanisms which may be involved in the development of Gougerot-Ruiter's vasculitis.

[Pseudo-late onset cutaneous parphyria in haemodialysis patients. Clinical and histological features. 9 cases (author's transl)].

In the context of a study of cutaneous problems seen in chronic renal failure patients on intermittent haemodialysis, the authors noted bullous eruptions very similar to late onset cutaneous porphyria. The bullae occur during the summer and are situated on exposed areas, in particular the head and neck. There is skin fragility and the bullae are transient, giving place to erosions and later to atrophic scarring. Even pseudo-miliary cysts have been observed. Histology reveals a sub-epidermal bulla, the basis of which consists of spiky papillary dermis. Immunofluorescence show immunoglobulin deposits in the vessels and basal membrane. All porphyrin measurements are negative. This disorder may be due to the presence of a non-porphyrinogenic photo-sensitising substance, found in certain types of plastic tubing.

Multiple keratoacanthomas: association with deficient cell mediated immunity.

A case of multiple keratoacanthomas associated with deficient cell mediated immunity in a 44-year-old male Caucasian is presented. Many attempts to restore the lymphocyte function (transfer factor, levamisole) have failed but the treatments may have had a curative effect on the development of new lesions. The significance of impaired cellular immune defense mechanisms is discussed.
