Morphogenesis of adrenergic cells in a frog parasympathetic ganglion.

The development of the parasympathetic cardiac ganglion of the frog Xenopus laevis was marked by the differentiation of a population of adrenergic small intensely fluorescent (SIF) cells. The neural crest contributes precursors for both SIF cells and the well-studied cholinergic cardiac ganglion neurons; this situation provided an opportunity to determine whether morphogenesis of the two cell types was correlated. Accordingly, we examined the initial differentiation, developmental regulation, and territorial domain of cardiac SIF cells for comparison with their cholinergic neuron neighbors. Adrenergic SIF cells were present during the time when the first cholinergic precursors were becoming postmitotic. Although SIF cells were present first, cholinergic neurons differentiated almost 16 times faster during the first week of embryonic and larval development and outnumbered SIF cells at all subsequent stages. Nonetheless, the accumulation of both cell types were correlated, since the ratio of cholinergic neurons to SIF cells remained at approximately 10 to 1 up to adult life. Early in development, SIF cells and cholinergic neurons were clustered in the sinus venosus portion of the atrium. The asymmetric distribution of cholinergic neurons within the atrium was lost but that of the SIF cells was maintained throughout life. These results identify relationships between the morphogenesis of two cell types in an autonomic ganglion and place constraints upon the cellular mechanisms that could produce the cells from their neural crest precursors.

Intracellular heterogeneity in mitochondrial membrane potentials revealed by a J-aggregate-forming lipophilic cation JC-1.

By using a potential-dependent J-aggregate-forming delocalized lipophilic cation, 5,5',6,6'-tetrachloro-1,1',3,3'-tetraethylbenzimidazolocarbocyanine++ + iodide (JC-1), we find that membrane potentials across mitochondria in a living cell can be heterogeneous. Remarkably, even within a long contiguous mitochondrion, regional heterogeneity in membrane potentials appears to be possible.

Promoter independence as a feature of most skin papillomas in SENCAR mice.

In the present study, the fate of individual papillomas induced by initiation-promotion on the backs of SENCAR mice was monitored after discontinuation of limited promoter treatment. Groups of 40 SENCAR mice each were initiated by a single topical application of 7,12-dimethylbenz[a]anthracene (DMBA) at 2, 1, 0.5, or 0.25 micrograms/mouse. Animals were promoted with 2 micrograms of 12-O-tetradecanoylphorbol-13-acetate (TPA) twice weekly during 10 weeks. At that time point, 10 papilloma bearing mice from each group were randomly selected to follow the growth of their existing tumors. Animals and their individual tumors were identified, charted, and photographed weekly. After an initial increase, the average number of papillomas/mouse remained constant after discontinuation of TPA in all the groups except the group receiving the highest DMBA dose (Group 1) and with highest tumor load. Twenty-one weeks after TPA was discontinued, only 10-20% of the papillomas had regressed and no statistically significant differences were found among the different DMBA dose groups. On the other hand, Group 1 showed the highest percentage of coalescing tumors which was apparently a function of tumor load. In addition, no differences were observed in the proportion of positive tumors with activating point mutations at codon 61 of the Ha-ras gene when comparing samples of papillomas from the highest DMBA initiation dose group (2 micrograms) versus the lowest DMBA initiation dose group (0.25 micrograms). Our present data suggest that papillomas induced with low doses of DMBA in SENCAR mice are no more TPA dependent than those induced by higher initiating doses. Furthermore, in SENCAR mice at the doses used in the present study (0.25-2 micrograms/mouse), the number of so-called "promoter dependent" papillomas represents only a small percentage of the total papillomas produced using the initiation-promotion protocol.

Effects of high-energy shock waves on murine renal cell carcinoma.

The effects of high-energy shock waves (HESW) on a murine renal cell carcinoma (RenCa) was investigated. In vitro exposure of tumor cells to HESW resulted in a dose-dependent reduction in cell viability as determined by trypan blue dye exclusion, plating efficiency, growth curve, and soft agar clonogenic assays. Activity of lactic dehydrogenase (LDH) was detected in the supernatant after the HESW treatment due to cellular destruction, and a dose-dependent increase in cytocidal effect was demonstrated. Ultrastructural changes with swelling and distorted cristae of mitochondria, vacuolation, ribosomal lysis, and chromatinolysis were observed in HESW-treated RenCa cells. Flow cytometric (FCM) study revealed that DNA content of RenCa cells diminished after 200 HESW treatment, and RNA content of tumor cells decreased markedly after 400 HESW treatments. Partial or complete inhibition of tumor growth was shown in both animal modalities of subcutaneous inoculation and intravenous injection with sequential lung metastases. This study stressed again that HESW may play a role in combinational protocol for the treatment of human renal cell carcinoma in certain circumstances.

The role of estrogen receptors in the development of gallstones and gallbladder cancer.

The preponderance of gallstones in fertile women suggests that female sex hormones play a role in the pathogenesis of cholelithiasis. In a pilot study we found estrogen receptors and high concentrations of estrogen in gallbladders of persons who underwent cholecystectomy for cholelithiasis. This finding, as well as previous reports on estrogen receptors in diverse human tumors, support the hypothesis that estrogen influences the development of gallstone disease and gallbladder carcinoma.

Luteal enzymes of the luteinizing hormone and beta-adrenergic signal transduction pathways in hypophysectomized rabbits do not require pituitary hormone support.

Experiments were conducted to determine whether continuous pituitary hormone support is required for expression of the LH and beta-adrenergic cAMP signal transduction pathways in rabbit CL during pseudopregnancy. Parameters of the LH and catecholamine cAMP signal transduction pathways in CL of estrogen-treated hypophysectomized rabbits were compared to those of pituitary-intact rabbits. Results showed that each of the parameters of the LH and beta-adrenergic cAMP signal transduction pathways was retained in CL taken from estrogen-treated pseudopregnant rabbits that had been hypophysectomized for as long as 13 days at levels not significantly different from those of estrogen-treated pituitary-intact rabbits. These included luteal basal, and LH-, epinephrine-, and fluoride-stimulated adenylyl cyclase activities; total luteal cAMP levels; the number and affinity of cAMP-dependent protein kinase regulatory subunit cAMP binding sites; binding activity of the type I and type II regulatory subunits; and the amount of catalytic subunit protein of cAMP-dependent protein kinase. We conclude that expression of the proteins of the cAMP signal pathway for LH and beta-adrenergic hormones in CL of estrogen-treated rabbits does not require pituitary hormone support.

Noncompliance in community psychiatry: a review of clinical interventions.

A substantial number of psychiatric patients seen in community settings fail to comply with treatment. Noncompliance takes many forms, including failure to keep initial appointments, to complete outpatient referrals from emergency services, to keep aftercare appointments following hospitalization, to remain in treatment, and to take prescribed medication. The author reviews the literature on noncompliance in community settings and discusses interventions that have been found to reduce noncompliance. Based on the review, four general recommendations for improving patient compliance are given; they are scheduling appointments before release from inpatient treatment, shortening the waiting period for appointments, using prompts in the form of letters and telephone calls to encourage patients to keep their appointments, and offering orientation and education about treatments and medications.

The pressure and volume dependence of the rate of wash-out in the bovine eye.

The rate of increase of outflow facility (the wash-out rate) was measured in bovine eyes at 6 and 15 mm Hg. The time-rate-of-change of facility was less at 6 mm Hg (0.20: delta facility/hour) than at 15 mm Hg (0.44). However, when the data was analyzed as a function of volume passing through the outflow system, the volume-rate-of-change of facility was the same at 6 (0.35: delta facility/ml) and 15 mm Hg (0.34). This was consistent with the hypothesis of macromolecules "washing-out" of the aqueous outflow system, if these macromolecules were saturable in the perfusate.

[A clinical study on 268 patients with cervical carcinoma].

Cancer is the leading cause of death in Taiwan with cervical carcinoma having the highest incidence rate (29.65 per 100,000) of all cancers among women. In the present study, we collected 268 consecutive cases of cervical carcinoma, seen in Tri-Service General Hospital from January 1983 to December 1984. The evaluation included a review of the case data including demographic features, symptoms, stage of cancer, reproductive history, pathology, and survival time (product limit method for survival). Results showed that in situ cervical carcinoma was diagnosed in 31 (11.5%) patients with a mean age of 47.7 years. Two hundred and thirty-seven patients (88.5%) with a mean age of 55.6 years were noted to have invasive carcinoma, showing stage I 22.4%, stage II 46.6%, stage III 10.1% and stage IV 9.3%. The ratio of invasive carcinoma to carcinoma in situ was 7.7:1. Ninety-two percent of patients were symptomatic on admission with vaginal bleeding (78%), vaginal discharge (16%) and abdominal pain (16%) being the most common, and the majority of these patients were symptomatic before cervical carcinoma was pathologically proved. Survival analysis showed five-year survival rates of 100%, 84.6%, 63.2%, 54.1% and 13.2% for stages O, I, II, III and IV, respectively. Low cervical carcinoma survival rates seemed clearly related to progression of the disease the presence of more extensive disease. The average age at the time of diagnosis was older and the clinical stage of disease was more advanced when compared with most developed countries.(ABSTRACT TRUNCATED AT 250 WORDS)

Role of parathyroid hormone in rat remnant kidney ammonium metabolism.

The role of parathyroid hormone (PTH) in ammonium metabolism in the rat remnant kidney was studied by examining the effects of parathyroidectomy (PTx) in rats with intact kidneys and with 5/6 nephrectomy (Nx). PTx in rats with intact kidneys caused a rise in urine pH and a decrease in urinary ammonium excretion without affecting in vitro ammonium production rate or the ammonium content in the cortex. Unexpectedly, the ammonium content in the medulla was markedly reduced by PTx so that the corticomedullary ammonium gradient was inverted. As compared to control rats, rats with 5/6 Nx had a lower urinary ammonium excretion rate, a higher in vitro ammonium production rate, and an increase in ammonium content in both cortex and medulla with reduced corticomedullary ammonium gradient. PTx in rats with 5/6 Nx led to a further decrease in urinary ammonium excretion, attenuated the increase in the in vitro ammonium production rate, and lowered the ammonium content in both cortex and medulla with inverted corticomedullary ammonium gradient. These effects of PTx in Nx rats were corrected by continuous PTH infusion with Alzet minipump. In summary, results from these studies indicate that PTH plays an important role in maintaining the urinary ammonium excretion. In rats with intact kidneys, PTH contributes to urinary ammonium excretion by increasing urinary acidification and medullary ammonium accumulation. In rats with reduced nephron mass, PTH enhances urinary ammonium excretion by stimulating ammonium production and retaining medullary ammonium in the remnant kidney.

Coronary artery bypass grafting within thirty days of acute myocardial infarction. Early and late results in 406 patients.

Between 1981 and 1987, 1726 coronary artery bypass operations were performed by a single group of surgeons at a community hospital. Overall hospital mortality in this group was 2.4% (41/1726). Of these patients 406 were operated on within 30 days of an acute myocardial infarction. The hospital mortality rate in this group was 6.7% (27/406) versus 1.1% (14/1320) in patients operated on without evidence of recent acute myocardial infarction (p less than 0.0001). In these 406 patients, sex, location of acute myocardial infarction, type of infarction, coronary anatomy, presence of postinfarction angina, technique of myocardial preservation, and the time from infarction to operation were not associated with hospital mortality. Univariate and multivariate analyses showed that three factors were significantly associated with increased hospital death: poor ejection fraction, less than 30% (p less than 0.0001), preoperative shock (p = 0.0005), and age greater than 70 years (p = 0.004). Follow-up was 90% complete (365/406 patients) at a mean time of 35 +/- 21 months. Of these patients 80% (292/365) were in New York Heart Association functional class I, and 10% (36/365) were in functional class II. Of all patients 88% were alive at 3 years, and 84% were alive at 5 years after operation. Multivariate comparison of survival curves showed that ejection fraction less than 30% was associated with decreased survival (p = 0.0002), followed by age (p = 0.0009). Patients younger than 70 years with an ejection fraction greater than 30% and not in cardiogenic shock can be operated on at any time after acute myocardial infarction without increased risk. Long-term survival and freedom from symptoms can be expected in these patients.

The effects of dietary calcium on blood pressure in spontaneously hypertensive rats may be mediated by parathyroid hypertensive factor.

A high calcium intake has been shown to attenuate the degree of hypertension in spontaneously hypertensive rats (SHR) and some human hypertensives. Conversely, a low calcium intake has been associated with an increase in blood pressure in both groups. In the present study, the effects of a high (2%), medium (0.6%) and low (0.02%) calcium diet on mean arterial pressure (MAP) and on the expression of a novel circulating hypertensive factor--parathyroid hypertensive factor (PHF)--were examined in SHR. In rats on the low calcium diet, MAP and PHF activity were significantly higher at 8 weeks than in the other two groups. In the high calcium group, MAP was significantly lower than in the other two groups, although it was elevated compared to week 0 values. PHF activity was not detected in the plasma of this high calcium group. Overall, MAP was highly correlated with PHF activity (r = 0.78, P = .0001). These results suggest that the effects of dietary calcium on blood pressure in SHR may be mediated by a novel circulating hypertensive factor, PHF, such that a high calcium diet inhibits, and a low calcium diet stimulates, the expression of this factor.
