Conformational changes in plant Ins(1,4,5)P3 receptor on interaction with different myo-inositol trisphosphates and its effect on Ca2+ release from microsomal fraction and liposomes.

The interaction of the only reported plant inositol trisphosphate receptor with different myo-inositol trisphosphates (InsP3 species), namely Ins(1,4,5)P3, Ins(1,3,4)P3, Ins(1,5,6)P3, and Ins(2,4,5)P3, were studied to assess the extent of Ca2+ mobilization from microsomes/vacuoles as well as liposomes in vitro. Ins(1,4,5)P3 and Ins(2,4,5)P3 bind with the receptor with comparable affinities, as evidenced from their dissociation constants (Kd approx. 100 nM at 5 degrees C), whereas the interaction between Ins(1,3,4)P3/Ins(1,5,6)P3 and the receptor was not detected even with these ligands at 5 microM. Ins(1,3,4)P3/Ins(1,5,6)P3 isomers also do not elicit Ca2+ release from liposomes or microsomes/ vacuoles. The ability of any InsP3 to bind the receptor for Ins(1,4,5)P3 is a prime requirement for Ca2+ release. However, the comparison of binding affinities at a single temperature does not help to correlate it directly with the extent of Ca2+ release from the intracellular stores because the concentration of Ca2+ released by Ins(1,4,5)P3 as estimated over a period of 20 s is 3500 +/- 200 nM/mg of protein and is about 4-fold higher than that by Ins(2,4,5)P3 under identical conditions. To understand the role of the receptor conformation in Ca2+ release by different isomers, we have probed the conformational change of the receptor when the different isomers bind to it. Accessibility of the tryptophan residues in the free and Ins(1,4,5)P3/Ins(2,4,5)P3-bound receptor was monitored by a neutral fluorescence quencher, acrylamide. The resulting Stern-Volmer-type quenching plots of the internal fluorescence indicate a change in the conformation of the receptor on binding to Ins(1,4,5)P3 and Ins(2,4,5)P3. It is also detected when far-UV CD spectra (205-250 nm) of the free and ligand [Ins(1,4,5)P3/Ins(2,4,5)P3]-bound receptor are compared. The results from CD spectroscopic studies further indicate that the conformational changes induced by the two isomers are different in nature. When thermodynamic parameters, such as enthalpy (delta H), entropy (delta S) and free energy (delta G), for the formation of the two InsP3-receptor complexes are compared, a major difference in the extent of changes in enthalpy and entropy is noted. All these findings taken together support the proposition that it is the overall interaction leading to the requisite conformational change in the receptor that determines the potency of the InsP3 isomers in their abilities of Ca2+ mobilization from the intracellular stores or reconstituted liposomes.

Stiff-persons' syndrome associated with thymoma and subsequent myasthenia gravis.

We report the first case of stiff-persons' (-man) syndrome in the setting of a histologically proven thymoma. Muscular hyperactivity was abolished under general anesthesia and the symptoms of stiffness resolved after thymectomy and three courses of intravenous immunoglobulins. After thymectomy, the patient developed ocular myasthenia gravis which later resolved spontaneously. We suggest that thymoma be sought for in cases with neuromuscular hyperactivity syndromes. Myasthenia gravis may develop subsequently in these cases.

Thalamic thought disorder: on being "a bit addled".

Humans can generate and maintain relatively coherent trains of thought in natural discourse. The neural mediation of this ability and the phenomenology of its breakdown are not well understood. We report a case of a woman with paramedian thalamic strokes involving the mammillothalamic tract, intralaminar nuclei, parts of the dorsomedial and ventral lateral nuclei bilaterally. She presented with a dense amnesia and confusion typical of the syndrome of bilateral paramedian thalamic infarcts. Her Tc-99m HMPAO brain SPECT scan showed decreased thalamic and basal ganglia blood flow. General diminution of cerebral blood flow and areas of further diminution in the right frontal, left temporal and left temporoparietal regions were also observed. Although her amnesia was characteristic of diencephalic amnesia, her most striking clinical feature was a bizarre, disconnected and at times incoherent speech output. Analysis of her speech revealed relatively preserved lexical and morpho-syntactic linguistic production. By contrast, analysis of the macrostructure of her discourse revealed frequent unpredictable topic shifts that were completely unconstrained by contextual factors. Many of her shifts were intrusions from previous topics. We interpret her severely disordered speech output as representing the surface manifestations of a thought disorder (rather than as a language disorder per se) characterized by an inability to maintain and appropriately shift themes that normally guide discourse. Median and intralaminar thalamic nuclei appear to be critical for the neurophysiologic regulation of thalamocortical and striatocortical circuits, which in turn may be critical for the functional regulation of contextually appropriate transitions of thought.

Contributions of the left and right cerebral hemispheres to line bisection.

Thirty-one right-hemisphere lesioned (RHL) patients, 11 left-hemisphere lesioned patients (LHL) and 10 normal controls (NC) bisected lines in three spatial location and four directional cuing conditions. The error direction and error size were analyzed as separate and combined variables. Seventy-seven percent of RHL patients and 45% of LHL patients made abnormally large errors in line bisection. Right-hemisphere lesioned patients were more sensitive to spatial location and directional cuing than NC subjects. In contrast, LHL patients were less sensitive to either condition than NCs. The error direction and error size emerged as dissociable components of line bisection. Right-hemisphere lesioned patients and NC subjects bisected lines consistently to one side of the true center. Left-hemisphere lesioned patients bisected lines equally often on both sides of the true center. Both RHL and LHL patients made larger absolute bisection errors than NC subjects, but the RHL patient's errors were larger than those of the LHL patients. We propose that the greater sensitivity of RHL patients to spatial location and directional cues and the directional consistency of their bisection errors represent contributions of the intact left cerebral hemisphere to line bisection. In contrast, the LHL patient's unrestrained ability to orient to both ends of the line reflects a contribution of the intact right cerebral hemisphere to line bisection. The failure of both groups to accurately bisect lines reflects a common visuospatial processing deficit that is more pronounced following RHLs than LHLs.

Results of excimer laser retreatment of residual myopia after previous photorefractive keratectomy.

BACKGROUND: Nine percent to 30% of all patients who undergo a single excimer laser photorefractive keratectomy (PRK) do not achieve an unaided visual acuity of 20/ 40 or better and may require optical correction to obtain adequate vision. METHODS: The authors performed a retrospective analysis of the records of 164 patients who had undergone retreatment with the excimer laser for residual myopia after a previous PRK. Mean follow-up was 35.5 +/- 15.2 weeks (range, 26-104 weeks). RESULTS: The mean spherical equivalent (MSE) before retreatment was -2.59 +/- 1.36 diopter (D) (range, -0.50 to -7.75 D). The final MSE after reablation was -0.52 +/- 1.36 D (range, 2.50 to -5.50). Of the 164 patients, 107 (65.2%) obtained a final refraction within 1.00 D of emmetropia and 111 (67.3%) achieved an unaided visual acuity of 20/40 or better. Only 10 patients (6.1%) lost more than one Snellen line of best-corrected visual acuity. The final MSE result for the subgroup of patients who had a pre-retreatment myopia of between -0.50 and -1.90 D (-0.31 +/- 1.09 D) was significantly closer to emmetropia than that of the subgroup with a residual myopia of -4.00 to -7.75 D (-1.62 +/- 1.94 D). CONCLUSIONS: Excimer laser retreatment may provide a relatively safe and predictable method of correcting residual myopia after an earlier PRK with a 25% extra correction recommended for residual myopia.

Astigmatism induced by spherical photorefractive keratectomy corrections.

PURPOSE: The purpose of the study is to evaluate the induced astigmatism after spherical photorefractive keratectomy on the Summit Omnimed (Summit Instruments, Waltham, MA) and the Nidek EC-5000 (Nidek Co. Ltd, Aichi, Japan) excimer lasers. METHODS: A total of 4269 eyes of 3289 patients were treated with a 5-mm optical zone using the Summit Omnimed excimer laser and 1825 eyes of 1303 patients treated with the Nidek EC-5000 excimer laser. The final astigmatic refractive outcome was compared with the initial refraction by vector analysis (Alpin and Jaffe method). RESULTS: Subjective astigmatic refraction for the Summit laser reduced from a mean of -0.39 diopter (D) +/- standard deviation (SD) 0.33 D (range, 0 to -2.50 D) to -0.33 D +/- SD 0.41 D (range, 0 to -3.00 D). Surgically induced astigmatism (SIA) had a mean of 0.42 +/- SD 0.34 D (range, 0 to 2.89 D). Mean SIA increased with increasing preoperative astigmatism by 0.60 D SIA for every 1.00 D of preoperative cylinder. For the Nidek laser, subjective astigmatic refraction changed from a mean of -0.18 D +/- SD 0.21 D (range, 0 to -1.25 D) to -0.30 D +/- SD 0.33 D (range, 0 to -3.00 D). Surgically induced astigmatism had a mean of -0.32 D +/- SD 0.29 (range, 0 to 3.05 D). Mean SIA increased with increasing preoperative astigmatism by 0.47 D SIA for every 1.00 D of preoperative cylinder. CONCLUSIONS: The authors show that spherical photorefractive keratectomy corrections can induce significant astigmatic change, particularly if a large amount of preoperative astigmatism is present.

Reduction in intraocular pressure after excimer laser photorefractive keratectomy. Correlation with pretreatment myopia.

PURPOSE: The authors relate the observed reduction in intraocular pressure (IOP) after excimer laser treatment to the degree of myopia treated. BACKGROUND: Intraocular pressure, measured by both Goldmann applanation and noncontact tonometry, has been reported to decrease after excimer laser photorefractive keratectomy (PRK). However, IOP readings after excimer laser PRK might be inaccurate as a consequence of changes in both the thickness and curvature of the cornea. METHODS: Baseline IOP readings were measured by noncontact tonometry in each eye of a group of 1320 patients at the time of their initial consultation. These were compared to readings obtained before treatment of the second eye, which took place a minimum of 4 months later. The untreated eyes served as controls. The paired Student's t test was used for statistical analysis. RESULTS: After PRK, a decrease was observed in the IOP of treated eyes that was related to the degree of myopia treated. A significant difference was observed between treated and untreated eyes (P < 0.0000). CONCLUSIONS: The IOP measured after PRK for myopia may be reduced because of changes in corneal thickness (absence of Bowman's membrane and central thinning) and topography. This is of particular relevance when monitoring the IOP of those patients who are given steroid drops to prevent regression. It also may be of importance in the management of any future glaucoma.

Computer simulation study on the conversion of spirolactone to enones over H-Y zeolite.

We report here the results of computer modeling studies and quantum chemical calculations on the spirolactone-to-enone conversion reaction over the zeolite catalysts, especially H-Y zeolite. We studied the adsorption mode of the molecules inside the supercage of H-Y and the mechanism of electron transfer between organic molecules and the framework of zeolite H-Y by density functional theory (DFT) calculations. Because the organic molecules considered in the present study are less symmetrical we docked the molecule inside the supercage of H-Y and energy minimization was then applied to these docked structures to yield representative low-energy binding sites for the molecules within the host structure. The interaction energy results show that the major interaction is between the methylene hydrogen of the molecule and the oxygen of the framework. The molecular electrostatic potential maps show that the ketonic oxygen of the reactant molecules abstract proton from Brönsted acid site. Thus the mechanism proposed by DFT calculation matches well with the experimental observations.

Stimulation of cyclic AMP, 17beta-oestradiol and protein synthesis by human chorionic gonadotrophin in human endometrial cells.

Human endometrial tissue was collected from 30-37 year old ovulatory women and enzymatically dispersed and processed to obtain endometrial stromal cells. Cells (2 x 10(4)/well) were incubated in vitro in the absence (control) or presence of human chorionic gonadotrophin (HCG) (10 IU/well) either for 1 h (to determine cAMP) or for 4 h (to estimate protein synthesis, 17beta-oestradiol and aromatase activity). HCG significantly stimulated cellular cAMP content and release in comparison to control. Both cellular accumulation and release of cAMP reached the peak at 10 min and then sharply declined in the absence of phosphodiesterase (PDE) inhibitor or remained steady for 1 h in the presence of PDE inhibitor. Content and release of 17beta-oestradiol from stromal cells steeply increased from 30 min to 1 h and then slowly increased for 4 h in response to HCG. Incubation of cells with HCG significantly stimulated (P < 0.01) cellular aromatase activity in comparison to control. HCG stimulated protein synthesis of the cells in two phases, initially at 15 min which remained steady for 1 h and then significantly increased (P < 0.01) between 1 and 2 h. The initial phase of protein synthesis appears to be the direct effect of HCG while the later phase was due to the higher concentration of oestradiol caused by HCG. The initial phase of protein synthesis was inhibited by cycloheximide but not by actinomycin D, whereas the later phase of protein synthesis was inhibited both by cycloheximide and actinomycin D. Results therefore indicate a probable mechanism of HCG action on stromal cells and demonstrate the physiological relevance of gonadotrophin receptor in human endometrial cells reported earlier from this laboratory.

Modulation of the clastogenic activity of bleomycin by reduced-glutathione, glutathione-ester and buthionine sulphoximine.

In this study an attempt has been made to establish a relationship between bleomycin (BLM)-induced DNA damage and buthionine sulphoximine (BSO)-mediated modified endogenous glutathione (GSH) status in normal human lymphocytes. Present results demonstrate that depletion of endogenous GSH by BSO reduced the clastogenic action of BLM, whereas elevation of endogenous GSH by treating the cells with GSH and GSH-ester, potentiates the cytotoxicity of BLM. A significant reduction in the frequency of deletions and chromatid breaks was observed when BSO-treated cells were treated with BLM. Again the frequency of these two types of aberrations was increased significantly when GSH- and GSH-ester-treated cells were treated with BLM. The observed reduction in the effect of BLM in GSH-depleted cells could be explained on the basis of the failure of reactivation of the oxidized BLM by reducing agent GSH which is present endogenously. Similarly, it appears that radicals which are generated due to reduction of oxidized BLM by the increased level of cellular GSH, after treating the cells with GSH or GSH-ester, could be responsible for the increasing frequency of deletion and chromatid breaks.

Comparative study of different anticonvulsants in eclampsia.

One hundred eclampsia patients were admitted in this hospital from January 1993 to November 1993 and they were subjected to different anticonvulsant regimes. There were 28 patients in lytic cocktail group, 16 in diazepam group, 16 in magnesium sulphate group and 40 patients in phenytoin sodium group. The results are compared in every aspect. Feto maternal outcome is best with magnesium sulphate regime followed by phenytoin sodium regime. They produce significantly better results than lytic cocktail and diazepam.

Phosphorylation sites in polyomavirus large T antigen that regulate its function in viral, but not cellular, DNA synthesis.

Polyomavirus large T antigen (large T) is a highly phosphorylated protein that can be separated by proteolysis into two domains that have independent function. A cluster of phosphorylation sites was found in the protease-sensitive region connecting the N-terminal and C-terminal domains. Edman degradation of 32P-labeled protein identified serines 267, 271, and 274 and threonine 278 as sites of phosphorylation. Analysis of site-directed mutants confirmed directly that residues 271, 274, and 278 were phosphorylated. Threonine 278, shown here to be phosphorylated by cyclin/cyclin-dependent kinase activity, is required for viral DNA replication in either the full-length large T or C-terminal domain context. The serine phosphorylations are unimportant in the C-terminal domain context even though their mutations activates viral DNA replication in full-length large T. This finding suggests that these sites may function in relating the two domains to each other. Although the phosphorylation sites were involved in viral DNA replication, none was important for the ability of large T to drive cellular DNA replication as measured by bromodeoxyuridine incorporation, and they did not affect large T interactions with the Rb tumor suppressor family.

Seasonal variations in refractive results following excimer laser photorefractive keratectomy.

PURPOSE: To evaluate the effect of seasonal factors on the final refractive outcome following excimer laser photorefractive keratectomy (PRK). METHODS: We examined the final refractive outcome for 5367 patients who had undergone PRK and photoastigmatic refractive keratectomy (PARK) over an 18-month period, April 1994 to August 1995, with a Nidek EC-5000 excimer laser and analyzed them by monthly and seasonal groups at time of treatment. RESULTS: We were unable to demonstrate any significant difference in patients grouped according to season at time of treatment. CONCLUSION: We would not recommend any seasonal-related adjustment for treatment with the Nidek EC-5000 excimer laser.

Effects of topical corticosteroids after photorefractive keratectomy.

PURPOSE: To assess the effects of topical corticosteroids (fluoromethalone) following photorefractive keratectomy (PRK). METHODS: Both eyes of 654 patients had PRK with a minimum of 4 months between treatments. Topical corticosteroids were prescribed for one eye only. Endpoints were assessed by comparing the ratio of intended to actual refractive change in the corticosteroid treated eye with the fellow eye (no corticosteroid use). RESULTS: The corticosteroid treated eyes showed a mean percentage correction of 95.7% compared to the untreated fellow eyes, which achieved a percentage correction of 103.3%-statistical significance of p = 0.00001. There was more of a trend toward regression in the corticosteroid treated eyes despite intervention with fluoromethalone. CONCLUSION: Topical corticosteroids showed very little positive effect on regression of effect after PRK.

Reaching out beyond the hills: HIV prevention among injecting drug users in Manipur, India.

Outreach interventions using ex-IDUs to inform and educate their peers about HIV/AIDS prevention measures have been found to be effective in the United States and other developed countries. While HIV/AIDS prevention programmes targeting IDUs have also been implemented in a number of developing countries, very little information is available on the process of implementation of these programmes. This paper attempts to document some of this knowledge by describing the implementation process of an outreach intervention targeting IDUs in a small town--Churachandpur--with high injection drug use and high HIV infection rates, in the north-eastern state of Manipur. The paper describes the barriers encountered in implementing the outreach and how these barriers were minimized. In conclusion, the paper makes the case for targeting outreach to the larger community before targeting the IDUs.
