[Physical and psychological future of patients with coronary disease. To whom should ambulatory rehabilitation be proposed? (author's transl)].

To ambulatory rehabilitation programme was proposed to 91 patients during their stay in hospital for myocardial infarction or aortocoronary bypass surgery. A retrospective study showed that 51 patients did follow the programme, whereas 40 patients preferred their own method of self-rehabilitation. Serial exercise tests demonstrated progressive and considerable improvement in physical aptitude among those who followed the programme, but the psychological, socio-familial and professional outcome was globally the same in both groups. The present study therefore confirms the numerous advantages of the rehabilitation programme but also shows that many of the potential candidates to such programmes are capable of returning by themselves to a normal and active life.

Parenteral diphosphonates for treating malignant hypercalcemia.

Two diphosphonates, EHDP (disodium etidronate, or ethanehydroxy-diphosphonate) and Cl2MDP (disodium clodronate, or dichloromethylene-diphosphonate) were injected in various doses in six patients with one or multiple episodes of malignant hypercalcemia. All patients responded well to the drugs after a delay period of two to seven days. The tolerance of the drugs was excellent. EHDP, and especially Cl2MDP, are promising agents for treating hypercalcemia and inhibiting bone resorption in malignant disorders.

Para-osteo-arthropathies. A new theory and mode of treatment.

The clinical and research experience of para-osteo-arthropathies (POA) observed in paraplegia is presented. From clinical findings and our own research studies, a new theory regarding the etiopathogenesis of the ectopic ossifications has been elaborated. POA is a biological process in which new bone is formed in tissues which in the normal condition do not ossify. A 45Ca kinetic study showed that POA are young bones with an active 'turnover'. The urinary hydroxyproline level, already increased from the onset of the paraplegia, rises at the moment of POA formation. On the other hand HLA B 18 and HLA RW 7 antigens are significantly increased in patients with POA. Several factors such as vascular disorders (venous stasis, modification of gaseous exchange) repeated microtraumatism at the site of the joints, injured perimuscular connective tissue, as well as increased incidence of some antigens, can play a role in the formation of POA. Metabolic changes related to vascular disorders probably influence cell differentiation, mostly the differentiation of fibroblasts in the perimuscular connective scar tissue following repeated microtraumatism. On the basis of observations the authors have tried to devise a treatment. Prevention remains the best way to avoid ectopic ossification but, once present, medical and surgical treatment must be undertaken.

[Treatment of hypercalcemia of tumoral origin with two diphosphonates].

Two diphosphonates, EHDP (disodium etidronate, or ethane-hydroxy-diphosphonate) and Cl2MDP (disodium clodronate, or dichloromethylene-diphosphonate) were injected in various doses in 8 patients who had had one or more episodes of malignant hypercalcemia. All patients responded well to the drugs after a time lapse of 2-10 days. Tolerance of the drugs was excellent. EHDP, and especially Cl2MDP, are promising agents for the treatment of hypercalcemia and the inhibition of bone resorption in malignant disorders.

Spinal cord involvement in systemic lupus erythematosus.

A review is made of the literature on central nervous system involvement in systemic lupus erythematosus (CNS-SLE). Particular attention is focused on lupus myelopathy. Referring to two well-documented personal cases, the authors describe the characteristic multisystem involvement, protean clinical pattern, serum and cerebrospinal fluid changes, histoimmunologic findings, response to corticosteroids and immunosuppressive agents and some prognostic features.

Intra-medullary pressure and intra-osseous phlebography in paraplegia.

Intra-medullary pressure and intra-osseous phlebography have been studied in the femurs of ten paraplegics. These methods are reliable for studying bone circulation. Results show that there exists an increase of intra-medullary pressure of more than twice the normal value and signs of delayed blood flow as well as a diaphyseal reflux in the bone. The blood gases are also modified (O2 saturation; pO2 and pCO2). These signs indicate that there is a venous stasis. These circulatory disturbances are due to an autonomic nervous system lesion. Furthermore it seems that these vascular modifications below the neurological lesion have an influence on the osteoporosis in paraplegia.

Actual concept of osteoporosis in paraplegia.

The osteoporosis which appears in paraplegics below the neurological lesion has been studied in 100 patients with the following parameters: quantitative X-rays, urinary hydroxyproline excretion, kinetic study of calcium metabolism by 45Ca, serum phosphorus and calcium, parathormone and calcitonin radio-immunoassay, quantitative histology, density fractionation of bone and amino-acid composition of fractionated bone analysis. All our results show that the important bone resorption occurring very early in the paraplegia is immediately followed by an increase of the bone repair. This osteoporosis below the neurological lesion in paraplegia represents an unbalance between the synthesis and the resorption of bone which possesses a large degree of activity. Furthermore, we demonstrated that the collagen of this bone is underhydroxylated. Following the quantitative X-rays and intraosseous phlebography results we have considered the aetiopathogenesis of this osteoporosis. It seems that vascular modifications due to the lesion of the autonomic nervous system play an important role and furthermore we believe that immobilisation represents only a minor factor in the aetiology of this osteoporosis.

Hydroelectrolytic determination in paraplegics (total body water; exchangeable sodium and total body potassium).

Measurements of total body water and total exchangeable sodium, by isotopic dilution, and of total body potassium by whole body counting of the naturally occurring 40K isotope, were performed on stabilised paraplegic or tetraplegic subjects. Nineteen patients were investigated by the complete set of tests, six by only body counting. The results of total body potassium are expressed, following a uniform scale, as "relative values", which were established by the processing of reference values, from numberous paired healthy subjects.
