Clinical experience with pentoxifylline in occlusive cerebrovascular disorders.

Pentoxifylline--3,7-dimethyl-1(5-oxo-hexyl) xanthine--a new vasodilator compound, was tried in 28 cases of occlusive cerebrovascular disease. A similar group of 24 patients were given xanthinol nicotinate. Although in terms of degree of improvement there was no significant difference in the two treatment groups, improvement was earlier in the group treated with pentoxifylline. No significant side effects were observed with any of the two drugs used in the present study.

Studies on the maturation of immune responsiveness in the mouse. II. Role of the spleen.

Experiments were designed to investigate the role of the spleen in the development of the murine immune system. By using mice splenectomized within 24 hr of birth, as well as mice with a hereditary, congenital absence of the spleen, the primary immune response to sheep erythrocytes was examined. The immunocompetence of lymph node cells from spleenless or control mice was assessed in vitro, in organ and in cell suspension cultures, and in vivo, by transfer into lethally irradiated syngeneic recipients followed by antigenic stimulation. The immunologic capacities of thymus and bone marrow cells were similarly tested by injection separately or in combination into irradiated syngeneic mice. Lymph node cells from spleenless animals appeared fully competent both in vitro and in transfer experiments. Neither neonatal splenectomy nor congenital absence of the spleen significantly reduced the capacity of bone marrow or thymus cells to participate in the immune response to sheep erythrocytes.

Vascular anastomoses in twin placentae and their recognition.

166 placentae from multiple pregnancies (including three triplets and 17 monochorral) have been studied by injection corrosion technique, using cellulose acetate buterite in acetone, under continuous pressure within physiological limits. It has been shown that vascular anastomoses in twin placentae are usually multiple. The direct type is frequently found in monochorial twin placentae and is mostly arterial or venous in nature. The indirect type is observed more commonly in dichorial twin placentae and is mostly arteriovenous in nature. Both types are almost universally found in monochorial monoamniotic placentae. The presence of direct anastomotic channels has a highly significant association with the incidence of marginal vessels and an exaggeration of parameters of stress to an abnormal quantum--both singly and in combinations of three or more-, and is independent of the pattern of branching. In the ontogenetic time-sequence perspective, direct anastomoses, marginal vessels and parameters of stress in a twin placenta can be regarded as different phases of the sequelae of an altered vascular relationship caused by disturbances in the vicinity of the 12th week of gestation. It has been further shown that direct vascular anastomoses are a greater risk to the twins than indirect ones. The importance of early and timely recognition of such channels, by an examination of twin placentae for the presence of marginal vessels, is stressed.

Fetal echocardiography--norms for M-mode measurements.

M-mode echocardiography was performed in 110 normal fetuses to study the norms for various cardiac parameters at varying gestational ages. The measurements were done in normal pregnancies with gestation varying from 14-38 weeks. The parameters studied were aortic root diameter, left atrial and right ventricular outflow tract dimensions, left and right ventricular cavity dimensions, and left ventricular, septal and right ventricular wall thickness. The values were analysed statistically by linear regression analysis and 95 per cent confidence limits were derived. It was found that values for right and left ventricles increased with gestation such that at term both ventricles were equal in size. The aorta and left atrium also increased with increasing gestation but the value for left atrium increased more than that of aorta giving increased left atrium to aorta ratio. These values could be utilised as norms for Indian fetuses and could help in detecting abnormalities of size.
