[Mass treatment of Schistomsoma haematobium bilharziasis in the member states of the Organization for Coordination of the Campaign against Endemic Diseases in Central Africa].

The authors analysed the results of the control of Schistosoma haematobium bilharziasis in two central African countries, the Congo and the Cameroun . These results demonstrated: --The feasibility of mass screening by filtering urine; --The acceptability and efficacy of new antibilharzian drugs, praziquantel and oltipraz . With the aim of improving the cost/effectiveness ratio of these campagnes and taking into account the health requirements which exist and/or are desired by the population, the authors propose four modifications of the strategy of mass control: --The integration of Schistosoma haematobium bilharziasis control with the other regional health priorities; --Screening target groups;

The use of plasma androstenedione in monitoring therapy of patients with congenital adrenal hyperplasia.

Therapy of congenital adrenal hyperplasia tranditionally is monitored by the amount of urinary 17-KS. However, 24-hour urine collections are difficult to obtain and are often unreliable. Measurement of the plasma concentrations of androgens, such as delta or T, would therefore be a more convenient way to determine the efficacy of treatment. Over a period of 2 to 24 months, 23 patients were periodically assessed by clinical examination, bone age, and determinations of plasma delta, plasma T, and 24-hour urinary 17-KS. Plasma T concentration correlated well with clinical control in females and in preadolescent males, but not in infant and pubertal males. By contrast, plasma delta concentration correlated well with clinical control in either sex, regardless of stage of puberty. The present study suggests that monitoring plasma delta concentration is useful in the long-term management of patients with CAH.

Tracheal stenosis. A study of 100 cases.

A series of 100 cases of tracheal stenoses, 66 non-neoplastic and 34 neoplastic, are reviewed; tracheal resection was performed in 28 inflammatory stenoses and in five tumours. A T-tube stent was used in 12 patients. Results were good in 81% of resections for non-neoplastic stenoses; two out of three resections for adenoid cystic carcinoma are alive after ten years and the third died after two years with metastases; two patients who received a tracheal resection for epidermoid carcinomas died after three and four years with metastases. Inflammatory tracheal stenoses treated using T-tube achieved good results only in one third of cases. Pre-operative preparation is the most important factor to obtain good results: end-to-end anastomosis requires a healthy mucosa without infection or ulcers.

[Dilatation of benign urethral stenosis using a balloon catheter].

The authors report their experience of benign urethral stricture dilatation by balloon catheter in 11 male patients. Ten posterior and 2 anterior urethral strictures were treated; in 1 patient several narrowings coexisted at various levels. Etiology was inflammatory in 4 cases, iatrogen in 3, post-traumatic in 2, and equivocal in 2. The patients were studied both before and soon after dilatation by means of retrograde and voiding cystourethrogram and uroflowgraphy; the follow-up (2-14 months) was performed by urodynamics alone. In all cases, dilatation was followed by the restoration of urethral gauge, together with prompt functional improvement of urodynamic parameters. The latter result subsisted in time in 9 patients. In 2 cases recurrences were observed, demonstrated at once by clinics and urodynamics. Both lesions were successfully re-treated. Neither early nor late complications occurred. In spite of the limited material, the valuable results obtained, together with the absence of complications, the peculiar morphology of recurrences, and the chance of repeating it make the procedure advisible as a valid alternative to conventional techniques for these pathologies.
