[Exercise testing in patients with the sick sinus syndrome].

Heart rate response to exercise was compared in three groups of subjects; 22 patients (mean age 63.7 years) with sick sinus syndrome and no other significant heart disease (Group I); 10 subjects of the same age with stable, asymptomatic sinus bradycardia at rest (Group II); 29 age-matched controls (Group III). All subjects underwent maximal, symptom-limited exercise testing and the maximal heart rate (HR max), the ratio between HR max and the theoretical maximal heart rate (HR%), exercise capacity (EC) and the ratio between heart rate % and exercise capacity (HR%/EC) % of the three groups were compared. Maximal heart rate, heart rate % and (HR%/EC) % in Group I patients were significantly lower than in Group III subjects (119.1 +/- 24.0 vs 139.0 +/- 18.2; 76.0 +/- 13.9 vs 87.9 +/- 10.8 and 83.0 +/- 19.3 vs 97.5 +/- 15.1 respectively); (HR%/EC) % was significantly lower in Group I patients compared to Group II subjects (83.0 +/- 19.3 vs 101.5 +/- 28.6). Heart rate response was the same in Group I and Group II patients and exercise capacity did not differ in the three groups. Maximal heart rate, heart rate % and (HR%/EC) % were similar in Group II and Group III subjects. The association of (HR%/EC) % less than or equal to 85% with either HR max less than or equal to 110/min or HR% less than or equal to 70% at the end of maximal exercise testing may be suggestive of sick sinus syndrome. The reduced heart rate response during exercise may be helpful in assessing sick sinus syndrome in patients with no other signs of heart disease.

Blood levels and electrophysiological effects of intravenous amiodarone in patients with junctional reciprocating tachycardia. Preliminary observations.

The time course and the mode of antiarrhythmic action of Amiodarone was studied after acute i.v. injection of the drug (5 mg/kg in 5 min) to four patients with junctional reciprocating tachycardia. Electrophysiological studies were performed before and 1 and 2 hours after Amiodarone injection. It was impossible or considerably more difficult to induce junctional reciprocating tachycardia after Amiodarone, and the drug's effect was detectable mainly on the AV node. Kinetic analysis of blood levels of Amiodarone during the electrophysiological study did not support a direct association between blood concentration and its effects.

Venous imprint on the superior infundibulum. An unusual pyelographic appearance.

Venous imprints on the upper urinary tract are common urographic findings but are rarely documented by renal angiography. The authors describe an unusual urographic appearance of a vascular imprint on the superior infundibulum due to a normal venous trunk. In this case retrograde phlebography was performed for a complete diagnosis.

Choice between US and CT in the radiological approach to the renal masses.

The diagnostic reliability of ultrasound (US) and computed tomography (CT) has been analyzed in 114 patients with renal masses (RM) detected by urography and consequently submitted to both exams. A flowchart is suggested in order to avoid useless examinations; accordingly, the US examination is always justified in the study of RM as opposed to the CT examination (requested only in 50% of RM) and angiography (requested only in 35% of RM).

[Dyssynergia of the left ventricle in idiopathic prolapse of the mitral valve. Kinetocardiographic study of 50 cases].

Fifty patients with idiopathic mitral valve prolapse (MVP), diagnosed by M-mode ecocardiography and free of other cardiovascular disease, were studied by kinetocardiography (KCG), to detect the presence of left ventricular dyssynergy. Kinetocardiography showed paradoxical outward movements (POM) in 27 patients: these movements were pansystolic in 13 patients. The results were normal in 20 patients, unclear in 3 patients. POM were more frequently found in positions K4, K3 and K5. A comparison among results obtained with KCG and the other noninvasive techniques we used (ECG, ecocardiography and phonocardiography) showed that ectopic beats were more frequently associated with POM, which also were more frequent in patients with prolapse of both mitral leaflets and with pansistolic prolapse. Finally POM were recorded in patients with atypical or silent MVP. We conclude that KCG is an useful noninvasive technique to discover left ventricular dyssynergy in patients with MVP.

Preoperative arterial occlusion in renal tumors: 3 years experience.

The authors describe their experience in preoperative embolization of 42 renal neoplasms. In all cases but one Gelfoam has been used for arterial occlusion with just a few and transient complications. Good hemostatic effect and décollement have been obtained irrespective of the diameter and vascularization of the tumor. To avoid late décollement difficulties and reduce the duration of clinical sequels the authors prefer, at present, to resort to surgery within 24 hours of embolization.

[Threatening arrhythmia by uncommon digitalic toxicosis (author's transl)].

The authors describe the case of a 38 year old woman who ate some buds of "Helleborus viridis" collected in the mountains mistaken for wild asparagus. The clinical patterns were like in the digitalic toxicosis: the woman presented both ventricular and supraventricular threatening arrhythmias, gastro-enteric troubles, systemic hypotension. The patient improved with the glucose, potassium chloride and magnesium sulphate infusion and intravenous xylocaine. The authors conclude that in case of patient with threatening arrhythmias of unknown genesis it is useful to pay attention to the wild vegetables ingestion.

Molding catheterism in the treatment of TB ureter stenoses: a ten-year experiment.

The authors describe their experience with molding catheterism for the treatment of TB ureter stenoses, both primitive and secondary after using chemotherapy. 85% of the cases treated with this method proved to be successful, which led the authors to recommend it prior to proceeding to any surgical treatment.

Cystic duplication of ejaculatory duct: case report.

A case of complex malformation of the genito-urinary tract, including cystic dilatation and partial duplication of the ejaculatory duct is reported. The lesion was visible on retrograde and anterograde vasovesiculography; its complete removal was possible using an abdominal approach.

[Radiologic and clinico-functional correlations in chronic lung diseases during congestive heart failure].

Two groups of patients were studied with serial correlated radiological functional analysis. Each group had a previous history of chronic obstructive pulmonary disease; however, on admission, only one group exhibited the clinical signs of this superimposed disease. The results of this research showed that chronic pulmonary disease is better defined by functional studies while cardiac failure (even in early stages) is always revealed by radiological examination.
