A 15-item short form of the psychological and interpersonal relationship scales.

Erectile dysfunction (ED) is a prevalent condition associated with complex psychological and interpersonal issues. The Psychological and Interpersonal Relationship Scales (PAIRS) is a valid and reliable measure of Sexual Self-Confidence, Spontaneity, and Time Concerns associated with ED and its treatment. This paper describes the development and validation of a 15-item, short form of PAIRS (SF-PAIRS). Domain scores and psychometric properties of SF-PAIRS were assessed using data from the validation studies of the full-length (23-item) PAIRS. Items in the two briefer scales (Spontaneity and Time Concerns) were identified using internal consistency analyses and crossvalidated during two multinational clinical trials and a community sample. The scales in SF-PAIRS showed good internal consistency, convergent and discriminant validity, were stable over a 2-week period, and showed sensitivity to treatment. SF-PAIRS is a valid, brief measure of psychological and interpersonal outcomes associated with ED.

Macrosomia in pregnancy complicated by insulin-dependent diabetes mellitus.

We assessed the factors influencing the birth weight of infants born to 83 women with insulin-dependent diabetes mellitus (IDDM) over a 5-yr period. Maternal glycosylated hemoglobin (HbA1) concentrations at delivery correlated with the percentile birth-weight ratios (r = .43, P less than .001) and indicated that approximately 18% of variance in the birth weight could be ascribed to glycemic control in the third trimester. Fetal macrosomia occurred in 22 (27%) pregnancies. When 20 of these pregnancies were compared closely with 20 nonmacrosomic pregnancies in diabetic women, the mothers of macrosomic infants were found to be more obese, have a history of previous macrosomic birth, and have higher concentrations of serum human placental lactogen and urinary estriols in the third trimester. Macrosomic pregnancy was further distinguished by accelerated fetal growth (judged by serial ultrasonography) from the 32nd wk of gestation and by biochemical (but asymptomatic) hypoglycemia in the neonate. In our study, no serious neonatal morbidity could be attributed to macrosomic pregnancy. Good glycemic control was attained in both groups, and no significant differences between the groups in overall glycemic control throughout pregnancy were noted. Thus, despite good glycemic control, macrosomia remains comparatively common in modern pregnancy complicated by IDDM, and factors other than maternal hyperglycemia must contribute to its etiology.

Salinity tolerance of Anopheles farauti Laveran sensu stricto.

To assess the salt tolerance of the malaria vector Anopheles farauti sensu stricto, larvae were collected from a freshwater environment on the outskirts of Honiara, Solomon Islands and placed in trays containing water with salinity varying from freshwater to seawater. Dead larvae and pupae and emerged adults were recorded and preserved. Most adults and nearly half of the larvae and pupae were then subjected to DNA analysis for species identification. No adult An. farauti emerged after prolonged immersion of larvae in undiluted seawater (3.5% salinity), although temporary immersion before pupation was compatible with survival. Salinities of up to 2.2% to 2.5% were compatible with good survival and adult emergence, at least from fourth instars. The results suggest that higher salinities may slow larval development and show that mortality at a given salinity is not uniform.

Prospective study of feasibility of routine use of multiple internal mammary artery anastomoses.

A study of 100 consecutive patients requiring multiple coronary artery bypasses was undertaken to evaluate the feasibility of routine use of multiple internal mammary artery (IMA) anastomoses. In 99 patients the IMA was used and in 80 patients bilateral IMA bypasses were done. In 15 patients multiple anastomoses with a single IMA precluded the need for bilateral IMA bypasses. In only 4 patients were bilateral IMA left to be contraindicated. By combination of use of both IMAs, free grafts and sequential grafts, the IMA accounted for 70% of the 318 anastomoses in these 100 patients. Scrupulous attention to technique and the use of the operating microscope are necessary to achieve multiple IMA anastomoses. It was concluded that the IMA can be used to supply the majority of anastomoses needed for coronary artery revascularization in nearly all patients.

Fetal tachyarrhythmias.

Fetal tachyarrhythmias are uncommon but are associated with significant perinatal mortality and morbidity. Fetal echocardiography permits the accurate determination of the nature of the arrhythmia. Transplacental or direct fetal therapy with anti-arrhythmic drugs can successfully cardiovert the fetus in utero with a subsequent reduction in perinatal problems. The investigation and management of these unusual conditions is reviewed.
