Specific selection of osteosynthetic material in the treatment of thoracic or lumbar spinal injuries by the posterior approach. A review of 165 cases.

The authors report 165 cases of thoraco-lumbar lesions with neurological dysfunction. All the patient were operated. They analyze the neurological and mechanical results and indicate the use of different osteosynthesis apparatus according to the type and level of lesions. Harrington's rods seem to give more precise repositioning while Roy Camille's plates give more stability. When the posterior wall of the spinal canal is intact, Kempf's compression rods can be used. Thoraxic spine injuries seem to be an indication for Harrington's rods, while lumbar injuries seem to call for Camille's plates.

Spinal intradural arachnoid cysts.

Based on the study of 8 cases of spinal intradural arachnoid cysts, the authors underline that the diagnosis is sometimes difficult because of the limitations of the paraclinical examination. They discuss aetiopathological problems.

Amniotic membrane diffusion chambers: a new possibility for pancreatic islet transplantation.

In order to avoid the host foreign-body reaction which is probably responsible for the long-term failure of the artificial membrane diffusion chambers currently used for protecting the implanted islet tissue from rejection, we tested the human amniotic membrane as a possible new tool for enclosing transplanted islets of Langerhans. Bag-shaped chambers were made with human amniotic membranes and the diffusion of 125 I-Insulin out and of D-glucose into the chambers was measured. The diffusion patterns of insulin and glucose were shown to be better than those described for the best artificial membranes, the 50% diffusion being reached at min 42 for 125 I-Insulin and at min 11 for D-glucose. Moreover, it was shown that no lymphocytes diffused into the amniotic membrane chambers. These results, together with the already known characteristics of the human amniotic membrane, induce us to propose it as a suitable material for further experimental work in islet tissue transplantation.

[Diagnostic and prognostic value of diaphragmatic electromyography and somatosensory evoked potentials in cervical spine and spinal cord injuries].

SEP contribution to diagnosis is interesting but limited to hardly examined complete syndromes in an emergency care unit. The predictive value of SEP is high if electrophysiological data are correlated the 10th. day with the clinical status. A good prognostic value is shown when the clinical posterior column and pyramidal tract dysfunctions are equal in intensity and distribution, i.e. in complete syndromes or central spinal cord syndromes or Brown-Sequard's ones. However, not any correlation exists in cases of anterior spinal cord syndromes. Dg.EMG. is an easy, atraumatic useful tool for the clinician. It must be a systematic approach of the brain stem and cervical spinal cord phrenic centers vitality that may be involved by the trauma. When a respiratory deficiency occurs, it allows the diagnosis of a "peripheral" or "neurological" etiology. Its high prognostic value for m tor diaphragmatic function (increase or decrease) must be discussed before any therapeutic decision.

Use of acrylic prosthesis in cervical spine pathology (report of 45 cases).

The authors report on their experience with the use of a reinforced acrylic prosthesis in cervical spine pathology. The study of their own 45 cases enables the advantages and disadvantages of the technique to be demonstrated. A discussion of possible improvements in the technique and its indication completes the report.

The anterior approach and plates in lower cervical posttraumatic lesions.

Between 1949 and 1982, 290 patients were operated on for lesions of the lower cervical spine. Since 1970 the combined use of the anterior approach and more and more sophisticated osteosynthesis equipment has led us to revise our therapeutic policy with regard to 160 patients. The results, advantages, and disadvantages of this policy are discussed hereafter.

A giant intrasacral schwannoma revealed by lumbosacral pain.

A case of giant intrasacral schwannoma is reported. This tumour was revealed by a lumbosacral pain without any neurological deficit. It appears that the lesion is surrounded by a dense layer of nucleated cells which resemble immature cells. This corona of young cells could explain the tremendous size of this tumour.

[Role of surgical decompression by the anterior approach in myeloradiculopathies caused by cervical arthrosis. Apropos of 850 cases].

The authors report their experience of the different clinical aspects of myeloradiculopathy due to cervical arthrosis. From 850 cases with surgical treatment, they describe the radicular, medullar and radiculomedullar features of this condition. The indications for an anterior decompressive procedure are analyzed according to clinical and paraclinical findings and results of the different methods used are compared.

Cyst of the choroid plexus of the left lateral ventricle.

A cyst of the choroid plexus of the left ventricle is described. The cyst was lined by a choroidal epithelium and filled with cerebrospinal fluid. It produced an intermittent obstructive syndrome. The cyst was almost completely invaginated into the third ventricle, simulating an expanding process in the third ventricle. After microsurgical removal, the recovery of the patient was complete. Electronmicroscopy of the cyst wall is reported. Histological and neuroradiological features are discussed.
