Transvaginal sonography (TVS) of the endometrium: current and potential clinical applications.

This review has discussed and illustrated the numerous applications of TVS for the sonographic detection of the endometrium. Its major applications are in early pregnancy evaluation and in evaluation of patients with endometrial carcinoma. As more clinical experience with this technique is gained, more extensive application of this technique will undoubtedly come about.

Sonographic evaluation of maternal disorders during pregnancy.

This article has reviewed and illustrated the sonographic features of the most common pelvic masses that can be encountered in pregnant patients. Sonographic evaluation allows delineation of the size, consistency, and location of pelvic masses occurring during pregnancy. It has an important role in establishing the enlargement or regression of pelvic masses that occur during pregnancy. Sonography also has an important role in the diagnosis of cholecystitis and upper urinary tract obstruction and/or infections in the pregnant patient.

Preoperative assessment of myometrial invasion of endometrial adenocarcinoma by sonography (US) and magnetic resonance imaging (MRI).

The presence and/or depth of myometrial invasion of endometrial adenocarcinoma has important prognostic and therapeutic implications. Fifteen patients with histologically proven endometrial cancer underwent preoperative evaluation with sonography (US) and magnetic resonance imaging (MRI) to assess depth of invasion. Using criteria of greater than or equal to 50% of myometrial wall involvement as representing deep invasion, and less than 50% as superficial invasion, US was more accurate than MRI in five cases; in three MRI was more accurate than US; both MRI and US were equally accurate in four; neither was accurate in three. Polypoid lesions caused the greatest number of false positive reports of deep invasion with both MRI and US. Preliminary results indicate that US and MRI have promise as preoperative tests to assess the extent of myometrial invasion.

Transabdominal and transvaginal sonography of pelvic masses.

Determination of the type of pelvic mass by pelvic sonography was improved significantly when a transvaginal study was performed after transabdominal sonography. Of 34 patients who underwent both studies, transvaginal sonography (TVS) was considered to add diagnostic information in 24 (70%); transabdominal sonography (TAS) was better than TVS in 4 (12%); TVS was equivalent to TAS in 5 (15%), and neither were accurate in 1 (3%). It is concluded that the use of transvaginal sonography as an adjunct to TAS is warranted for evaluating most pelvic masses; especially those confined to the true pelvis.

A modified scheme for biophysical profile scoring.

A modified scheme for biophysical profile scoring based exclusively on real-time ultrasonographic examination is proposed and includes expanded scores of fetal movements and fetal breathing and only qualitative assessment of accelerated placental maturation, subjective ultrasonographic impression of oligohydramnios, and decreased fetal tone. This method was compared with the method of Vintzileos et al. in the first phase of this observational study and was found to have good correlation in different score categories. This method was then applied in the second phase of the study to 180 high-risk pregnancies to examine correlation with perinatal outcome variables. The relationship between results of the last total score and perinatal outcome variables shows good predictive values. Results of this preliminary study suggest that real-time ultrasonographic evaluation-based scoring of acute fetal events, namely, movements and breathing alone, may have an important role in perinatal management.

Endovaginal sonographic diagnosis of dilated fallopian tubes.

Twelve cases of fallopian tube dilatation were diagnosed in 10 patient by using endovaginal sonography. The diagnosis was confirmed by surgery in seven patients and by hysterosalpingography in three. A tubular shape was present in every case. Other sonographic features included a well-defined echogenic wall, a folded configuration, and linear echoes protruding into the tube lumen. Dilated tubes were distinguished from bowel loops by a lack of peristaltic activity and from pelvic veins by a lack of moving low-level echoes on real-time sonography. We conclude that the findings of dilated fallopian tubes on endovaginal sonography are sufficiently characteristic to allow the diagnosis to be made with this technique.

Wrongful life and wrongful birth. Implications for diagnostic sonography.

We trust that this communication will further the understanding of the concepts involved in wrongful life and wrongful birth and will assist the conduct of medical practice in such a way to minimize risks to patients and ultrasound physicians with regard to their liability. When these physicians are involved in wrongful life cases, we believe that the concepts presented will assist them in understanding the legal and societal environment in which they find themselves. We hope also that this communication may assist the defense attorney to more effectively prepare a response to the plaintiff's legal theory. Knowledge should provide understanding, if not relief.

Duplex Doppler sonography of renal transplants. Correlation with histopathology.

The correlation of histopathologic findings with duplex Doppler sonography (DDS) in 24 biopsies in 21 patients was studied. The resistive indices (RI) of the acute rejection group were statistically different (p = .04) from those that were negative for rejection or clinical controls. Although the number of cases of moderate to severe interstitial and/or vascular rejection was relatively small compared to the mild group, patients with the more severe forms of rejection (moderate to severe interstitial with or without vascular rejection) tended to have higher RIs (.89 +/- .04) than those with the milder forms of rejection (.75 +/- .05).

Hand-held and automated sonomammography. Clinical role relative to X-ray mammography.

A retrospective analysis of 73 patients who had undergone X-ray mammography (XRM), automated sonomammography (ASM), and hand-held sonomammography (HSM) was performed to evaluate the clinical role of both types of sonomammography (SM) relative to XRM. The results were classified on the basis of mammographic and sonographic findings. Clinical and/or surgical proof was obtained in all 73 patients. The SM was complementary to XRM, depending on the radiographic breast density. When comparison was made between ASM (4.5 MHz) and the HSM (7.5 MHz), the pathologic condition was better identified by HSM in 86% of the cases. Moreover, in no cases was ASM better than HSM.

The concepts of wrongful birth and wrongful life and their relation to medical imaging.

Although often used interchangeably, "wrongful birth" and "wrongful life" are distinct legal concepts. Wrongful birth involves the physician's duty to impart to expectant parents information about potential fetal defects. Wrongful life suits reflect the child's right to recovery for life-long suffering as a result of the physician's inadequate pre- or peri-natal care. Modern medical imaging techniques can play a major role in the diagnosis of interuterine malformations; this article explores the legal duties which may be imposed upon physicians who interpret such tests.

Hydrops fetalis.

Sonography has a major role in the identification, evaluation, and treatment of the hydropic fetus. Although the prognosis of the NIH fetus is generally poor, the IIH fetus can frequently be treated effectively by transfusions performed by percutaneous needling of the umbilical vein. In addition, sonographic guidance of percutaneous umbilical vein sampling affords an accurate assessment of the etiology of NIH by facilitating karyotyping of the fetal blood.
