A comparison of tilidine hydrochloride and morphine in the treatment of postoperative pain.

Intramuscular injections of 10 mg morphine or 100 mg tilidine hydrochloride were administered in a double-blind fashion and in random order to 40 patients after minor surgical operations. The therapeutic effect of each analgesic was assessed by comparing the severity of pain immediately postoperatively with that 1, 2, 3 and 4 hours after injection of the relevant analgesic. Tilidine and morphine relieved pain to a similar degree. Within the limits of the protocol followed, morphine appeared superior to tilidine in some tests. No major side-effects occurred.

Plasma angiotensin II levels in African hypertensives.

Plasma angiotesin II levels were measured by immunoassay in twenty-seven Zulus with raised arterial pressures. Angiotensin levels were higher in patients with malignant than benign essential hypertension, mean values being 260.2 and 90.7 mumicrogram/ml with stanard deviations of 119.4 and 25.1 respectively (P = less than 0.0005).

Recessive inheritance of diabetes: the syndrome of diabetes insipidus, diabetes mellitus, optic atrophy and deafness.

A few rare syndromes have been delineated in which diabetes mellitus is inherited in association with other conditions. This paper describes five patients, including four siblings in one family, who have diabetes insipidus, diabetes mellitus, optic atrophy and deafness (the DIDMOAD syndrome). The parents of both families are normal but are first cousins. All the patients have insulin-dependent diabetes mellitus with a typical juvenile-onset. The onset of diabetes insipidus was insidious and the symptoms could easily have been ascribed to poor control of diabetes mellitus. The importance of diagnosing diabetes insipidus is that all these patients had dilatation of the urinary tract varying from mild hydroureter to severe hydronephrosis and this improved with treatment of the diabetes insipidus. It is suggested that patients with diabetes mellitus and optic atrophy should have regular screening tests for diabetes insipidus since it is likely that they represent cases of the full syndrome with incomplete clinical expression. The occurrence of this rare syndrome in four siblings of unaffected parents indicates that the syndrome is due to a recessive gene, but the pathogenesis is unknown.

A preliminary report on cephradine.

Plasma cephradine levels were measured after parenteral and oral administration to healthy adults. Plasma cephradine contrations of 7 - 15 mug/ml were measured after 0.5 g had been administered by mouth, and concentrations of 12 - 57 mug/ml were measured after parenteral administration; they fell to 3 mug/ml within 240 minutes except after intramuscular administration. The results of an open clinical trial conducted among 10 patients suffering from urinary tract infections were satisfactory.

Plasma triglyceride measurement. A comparison of four commercial methods.

This study investigated the reliability of 4 commercially available methods for the assay of plasma triglycerides. The kit methods were those of Boehringer, Dade, Haury and Oxford. The Boehringer method showed a high degree of precision, acceptable accuracy, and consistent day-to-day reproducibility.

The effects of sodium pentosan polysulphate on peripheral metabolism.

Sodium pentosan polysulphate is a polymer of beta-D-xylopyranose which possesses some of the biological properties of the endogenous acid polysaccharides such as heparin. The actions of this compound on plasma lipids, carbohydrate metabolism and fibrinolysis have been studied using the human forearm technique. Ten healthy volunteers were investigated in the fasting state, and blood flow measurements and blood samples were taken before and at 60 and 90 minutes after 100 mg sodium pentosan polysulphate by intramuscular injection. Following administration of the drug, significant changes occurred in plasma triglyceride, free fatty acid (ffa) and glycerol levels. Triglycerides fell (P less than 0,001) and glycerol and FFA rose (P less than 0,001. Changes were largely owing to activation of intravascular lipolysis, although an effect on stored triglycerides was also noted. Fibrinolytic activity also increased after drug administration, but no changes were noted in insulin exchange or glucose metabolism.

Carbohydrate tolerance, plasma insulin, growth hormone and lipid levels in Indian and Black diabetics.

Glucose tolerance tests were carried out on 35 recently discovered Indian and Black maturity-onset diabetic patients. Differences were found between the two ethnic groups although they were of comparable age, body weight, and probable duration of diabetes at the time of investigation. Glucose tolerance was worse among Blacks and was associated with low plasma insulin levels. Blacks also had lower plasma triglyceride and cholesterol values than Indian diabetics. Plasma growth hormone respone was similar in both groups. The possible role of these factors in determining the relative frequency of ischaemic heart disease in the two racial groups is discussed.

Plasma renin levels in Zulu hypertensives.

Plasma renin was measured by immunoassay in 10 normal Zulu controls and 72 hypertensive Zulu patients. Plasma renin values were similar in controls and patients with benign essential hypertension (1,6 plus or minus 0,83 and 1,7 plus or minus 0,21 ng/ml).

The cardiovascular effects of etilefrine.

Intravenous etilefrine increases the pulse rate, cardiac output, stroke volume, central venous pressure and mean arterial pressure of healthy individuals. Peripheral vascular resistance falls during the infusion of 1-8 mg etilefrine but begins to rise at higher dosage. Marked falls in pulse rate, cardiac output, stroke volume and peripheral bloodflow, accompanied by rises in mean arterial pressure, occur when etilefrine is infused after administration of intravenous propranolol 2,5 mg. These findings indicate that etilefrine has both beta 1 and alpha adrenergic effects in man.
