Antitumor activity of sorafenib in FLT3-driven leukemic cells.

Activating internal tandem duplication (ITD) insertions in the juxtamembrane domain of the FLT3 tyrosine kinase are found in about one fourth of patients with acute myeloid leukemia and have been shown to be an independent negative prognostic factor for survival. We show that sorafenib (BAY 43-9006, Nexavar) potently inhibits FLT3 enzymatic and signaling activities. In HEK293 cells stably transfected with FLT3-WT or FLT3-ITD, sorafenib blocked basal and ligand dependent FLT3-mediated tyrosine autophosphorylation as well as extracellular signal-regulated kinase1/2 and Stat5 phosphorylation. In leukemia cell lines MV4-11 and EOL-1, sorafenib treatment resulted in decreased cell proliferation and inhibition of FLT3 signaling. The growth of the FLT3-independent RS4-11 cell line was only weakly inhibited by sorafenib. Cell cycle arrest and induction of apoptosis were observed upon treatment with sorafenib in MV4-11 and EOL-1 cells. The antitumor efficacy of sorafenib was evaluated against the MV4-11 leukemia grown subcutaneously in NCr nu/nu mice. Doses of 3 and 10 mg/kg administered orally for 14 days resulted in six and nine out of 10 animals with complete responses, respectively. The demonstration that sorafenib exhibits potent target inhibition and efficacy in FLT3-driven models suggests that this compound may have a therapeutic benefit for patients with FLT3-driven leukemias.

Properties of matter matter in assessment of scald injuries.

Gathering information on the thermal characteristics of the causative agent in scald injuries provides clues as to the likely depth of injury. We theorize that viscosity and thermal capacity may have important roles to play when we view scalds as contact burns due to a liquid. From a 4-year review of our scald patients, we found that scalds due to thick food/drinks such as congee (a porridge made from rice) were associated with a higher rate of surgery. We determined the rate of cooling of seven common food/drinks and found little difference between water, tea, coffee and noodles, other than the starting temperatures. However, the rate of cooling of congee was significantly slower indicating a greater thermal capacity. A "drip" model found that a skin substitute exposed to congee cooled significantly more slowly compared to other food/drinks, suggesting that its greater viscosity plays a role. This supports the theory that the viscosity of food/drink is important.

Microsurgical reconstruction in children's burns.

There have been few reports on microsurgical procedures performed on children who have been left with disability and deformity as a result of burn injury. The reality is that whilst there are many children who could benefit from such procedures they do not have access to the resources that can provide them. Where these resources are available there has been a reluctance to use them partially due to a misconception that such procedures are technically challenging and carry significant risk. This paper presents a series of 21 children in whom 29 microsurgical procedures were performed. There were two failures but in all other cases there was significant improvements in the functional ability and/or the aesthetic appearance of the child. This series includes novel variations on the antero-lateral thigh flap; tissue engineering, dual flap harvest, size and age. Refinements of inset are described to prevent pin-cushioning and linear scar contracture. In selected cases microsurgical reconstruction can produce life long benefits, with minimal surgical and donor site morbidity.

Paediatric upper limb contracture release following burn injury.

PURPOSE: The aim of this study was to assess the functional outcome in children who had undergone release of upper limb burn contractures at the axilla, elbow and wrist. METHODS: We studied a series of 10 patients reconstructed with local flaps (n = 5), Integra artificial skin (n = 10) and micro-vascular free tissue transfer (n = 5) to release contracted axillae (n = 8), elbows (n = 5) and wrists (n = 7). Follow-up ranged from 6 months to 6 years and outcome was assessed by functional improvement. RESULTS: Full functional release was achieved in all cases at the time of the surgery. Local flaps resulted in good or excellent outcomes in all five cases. Six patients reconstructed with Integra were assessed as having an excellent outcome whereas four had a poor outcome. Free tissue transfer with excellent result was achieved in five children. CONCLUSIONS: This series assessed the three treatment options that we have used for the release of upper limb post-burn contractures. We have shown that local flaps fashioned from burnt skin survive, Integra requires extended post-operative care to prevent recontraction and free tissue transfer is safe in young children. Each method when used appropriately has produced excellent results which have been maintained over the follow-up period.

Chemical assaults in Hong Kong: a 10-year review.

The aim of this study was to assess the patients admitted to Prince of Wales Hospital Burns Unit, Hong Kong between September 1991 and August 2001 following chemical assault. A total of 2319 burns were admitted during the study period of which 19 had been subjected to chemical assault. All victims sustained a burn injury to the head and neck area and 47% of victims' eyes were burnt. A total of 84% required surgical intervention. Where a reason for the attack was admitted most were due to relationship difficulties. This study has defined the victims of chemical assault in Hong Kong. At a time when the overall number of assaults in Hong Kong is at its lowest level for 5 years it is of concern that there appears to have been a recent increase in the number of chemical assaults in Hong Kong.

Outcome analysis of 286 severely burned patients: retrospective study.

OBJECTIVE: To evaluate the outcomes of severely burned patients treated at a regional burns unit and to develop a predictive model for survival and length of hospital stay for major burn patients in Hong Kong. DESIGN: Retrospective study. SETTING: Burns unit of a regional public hospital, Hong Kong. PATIENTS: Two hundred and eighty-six severely burned patients treated from March 1993 to February 2000. MAIN OUTCOME MEASURES: Details of demographics, mechanism of burn, extent of burn, incidence of inhalation injury, length of hospital stay, and mortality rate were recorded and entered into a database. Stepwise logistic regression and linear regression were applied to develop a predictive model for mortality and morbidity, respectively. RESULTS: Of 286 major burn patients treated in this 7-year period, 25 patients died from their injuries, yielding a mortality rate of 8.7%. Stepwise logistic regression was applied to develop a predictive model for mortality. We found that inhalation injury, age, and total body surface area involvement were independent significant predictors of death. Accuracy of this predictive model reached 93%. Similarly, stepwise linear regression was used to develop a predictive model for length of hospital stay. Sex, inhalation injury, total body surface area of burn, and total body surface area(2) of burn were significant predictors of length of hospital stay (R(2)=0.2). Only three patients' duration of hospital stay was more than three standard deviations from the predicted length of hospital stay. CONCLUSION: A predictive model for mortality and length of hospital stay has been developed for major burn patients in Hong Kong. This model may help clinicians to counsel patients and relatives at an early stage of care, to provide a basis from which new treatment plans can be compared, and to facilitate efficient allocation of valuable medical resources.

The lateral distally based adipofascial flap of the lower limb.

Several authors have reported the advantages of adipofascial flaps. We describe a series of flaps based exclusively on the perforator of the peroneal artery. This is an axially patterned distally based adipofascial flap useful for small and medium-sized defects over the distal fibula, lateral malleolus, the Achilles region or the dorsum of the foot. It can be an excellent alternative to fasciocutaneous or free flaps, with minimum donor-site morbidity, good cosmesis and an excellent functional result.

Sub-lethal effects of exposing the human melanoma cell line SKmel-23 to 532 nm laser light.

The human melanoma cell line SKmel-23 has been used to investigate the sub-lethal damage that can occur as a result of exposing melanin containing cells to light (532 nm) from a frequency doubled Q-switched (Nd:YAG) laser. A dose response curve was obtained, which indicates that at energy levels of 0.6 J/cm2 and below no effect on either the viability or growth rate of the cell line was observed. Above this, cells rapidly died and at an energy level of 2.0 J/cm2, only approximately 15% of cells survived. This contrasts with the effects on the G361 melanoma line, which contains far less melanosomes, as an LD50 for this cell line was approximately 5.5 J/cm2. Exposing SKmel-23 cells to 0.4 J/cm2 of 532 nm light results in a diminution of the number of melanosomes within cells as well as a marked decrease in melanin content, as determined by spectrophotometric assay and electron microscopy. Using the reverse transcriptase polymerase chain reaction technique, the reduction in melanin content of the cells was accompanied by a selective decrease in mRNA coding for tyrosinase, the first enzyme in the biosynthetic pathway for melanin. No decrease in the mRNA coding for the GAPDH protein was observed. Our finding has implications for understanding the control processes that regulate the melanin content of cells and suggests that the model described can be used to further investigate changes that may occur in cells as a result of their exposure to sub-lethal levels of laser light.
