[Bacterial endocarditis from the stimulatory unit of a cardiac pacemaker].

Cardiac device endocarditis is an infrequent complication of permanent pacemaker and implantable cardioverter-defibrillator. A personal observation had been valued noteworthy in relation to the relatively low rates and clinical features. An old man presented recurrent fevers throughout 17 months before transesophageal echocardiography detection of intracardiac vegetations. The removal and replacement of the infected device via open thoracotomy had been successful, and subsequently confirmed in a prolonged follow-up.

[Primary adenocarcinoma of jejunum with obscure gastrointestinal bleeding].

A case of primary small bowel adenocarcinoma is reported because of the rarity of this malignancy. Interestingly, the location of the tumour was in jejunum, instead of the most common site in duodenum. The clinical presentation was anemia, with positive fecal blood test, under antiaggregant platelet treatment for coronary heart disease, initially related to endoscopic evidence of erosive gastroduodenitis. The exacerbation of gastrointestinal bleeding during proton pump inhibitors therapy and the recurrence of abdominal pain caused careful investigation. The source of the obscure gastrointestinal bleeding had been achieved by wireless capsule endoscopy, because of not diagnostic findings of conventional upper endoscopy control, colonscopy, double contrast enteroclysis and selective arteriography. The primary definitive therapy was a radical resection of the jejunal neoplastic loop, important prognostic factor.

[An epidemiological study of thyroid pathology in a group of hospitalized patients].

The epidemiological impact of thyroid disease was assessed in a group of in-patients from an area with a potential iodine deficiency. The overall frequency of thyroid disease in the three years studied was approximately 4.91%, with a prevalence of elderly patients and a bias towards the female sex of 8.14:1. Nodular, single and multiple lesions were observed in 3.74% of hospitalised patients, whereas malignant forms were limited to 1.98% of the patients with nodular pathologies. When thyrotoxic syndromes were classified it was found that there was a predominance of multinodular toxic goitres over Basedow's disease. These findings are discussed and compared to previously published data.

[Zieve's syndrome. Case report].

The Zieve's syndrome consists of the transient association of cholestatic jaundice, hemolytic anemia, hyperlipemia, in a chronic alcoholic. The Authors, after a short introduction regarding physiopathological problems, describe and subsequently examine on the clinical ground a recent personal observation, which had been mentioned for the rarene-s of this disease.

[A case of primary hyperparathyroidism. Clinical contribution].

The authors discusses some clinical, laboratory and instrumental problems of a reported case of primary hyperparathyroidism caused by adenoma from the point of view of medical literature. Nethertheless the lack of the specificity of the clinical manifestations and the slowly progressive evolution of this disease, primary hyperparatiroidism must be suspected in the presence of his most common multifarious complications. The finding of hypercalcemia in these conditions and from screening procedures requests parathyroid functional tests and exclusion of other causes of which chiefly pseudohyperparathyroidism may be difficult to be differentiated. The early diagnosis and correction of the parathyroid lesion avoid further damage and regression of many preoperatory complications.

[A case of primary hyperaldosteronism. Case report].

The authors discuss a reported case of primary aldosteronism, which is relatively uncommon within hypertensive population. The diagnosis of primary aldosteronism must be suggested by the presence of the association of arterial hypertension and hypokaliemia, which nevertheless is not pathognomonic. It is emphasized the significance of the detection of this syndrome on account of the correction following surgical removal of the adenoma of the adrenal cortex. Two attacks of paroxismal hypertension, which are atypical in primary aldosteronism, had been observed in the reported case; however, apart from these exceptions, arterial hypertension has resulted generally constant and of moderate degree, as well as the majority of the others descriptions. The personal experience confirms the need to determine plasma levels of renin and aldosterone before the therapeutic or diagnostic use of spironolactone.

[Hematological sequelae of splenectomy in a case of chronic myeloid leukemia].

Thrombocytosis is to be expected after splenectomy, but is usually slight and limited to a maximum period of 3-4 months. In contrast, the case reported of splenectomy, following traumatic rupture of the spleen during chronic myeloid leukaemia, figured a fairly substantial increase in platelet count which persisted in spite of treatment and featured haemorrhagic complications. The problems involved are discussed in the light of reports in the literature.

[Clinical aspects of the radioimmunological determination of serum ferritin].

Personal experience confirms the diagnostic value of serum ferritin estimation in the iron deficient microcytic anemia, where there are low levels. The clinical significance of the test is restricted by the not negligible incidence of levels like that of iron overload in the hepatic and neoplastic diseases. In hepatic diseases the test resulted to be related to the changes of serum transaminases. In malignant neoplasms the high serum ferritin levels had been only sometimes associated with transfusional siderosis.

[Vascular spiders, palmar erythema and Dupuytren's contracture in alcoholic hepatic cirrhosis. Clinical-statistical contribution].

Vascular spiders, palmar erythema and Dupuytren's contracture had been studied in four groups of patients with alcoholic cirrhosis, hepatic alcoholic involvement without cirrhosis, alcoholism without hepatic involvement, extrahepatic diseases without alcoholism. these cutaneous lesions had been observed more frequently in alcoholic cirrhosis, with an incidence of 72%, 38%, 24% respectively. Vascular spiders, palmar erythema and Dupuytren's contracture appeared related to the hepatic involvement from alcoholism rather than to the alcoholism by oneself. In the alcoholic cirrhosis the Dupuytren's contracture affected patients younger than those of the others groups and had been influenced from the association of diabetes mellitus, but not from the occupational activity.

[Chylous ascites. A case report].

The Authors report a recent personal case of chylous ascites associated to metastatic carcinoma of body's pancreas, because of uncommon absolute occurrence of this clinical manifestation. The diagnostic and therapeutic problems are discussed on the ground of the remarks from the review of medical literature.

[Carcinoembryonic antigen (CEA) and non-oncological liver diseases (case report)].

Carcinoembryonic antigen (CEA), an oncofetal glycoprotein, has been originally suggested as a tumor marker for colorectal cancer and afterwards has been regarded as a specific marker for different cancers. The determination of CEA in 73 patients with "benign" hepatic diseases points out the limitation of test's diagnostic value on account of the not infrequent observation of false positives in the examined cases. In particular the greatest incidence and intensity of elevated levels of CEA has been found in hepatic cirrhosis. However must be mentioned that the highest values of CEA has been documented nearly constantly in the comparison's group of some forms of malignancies, between which nevertheless there have been sporadically false negatives.

[Evaluation of carcinoembryonic antigen (CEA) in patients with neoplasms and hematologic diseases].

Carcinoembryonic antigen (CEA), a oncofetal glicoprotein, has been regarded as specific marker for colorectal cancer initially and restricted to the significance of tumor-associated antigen afterwards. Circulating CEA levels were determined in 47 patients with hematologic malignancies, resulting elevated in 10 (21%). The highest values had been discovered in a chronic lymphocitic leukemia complicated by primary hepatoma, causing the problem of the role played by the second tumor, likewise to another CEA-positive patient with the association "Hodgkin's disease-pancreatic carcinoma". The CEA employment had not been particularly satisfactory in the therapeutic monitoring and in the early detection of the relapses, in opposition to the results referred in the colorectal, mammary and bronchogenic carcinoma.

[Monolateral thrombosis of the carotid artery. Contribution of cases].

The clinical problems posed by a series of partial or total monolateral carotid occlusions complicated by cerebral infarction are discussed. Emphasis is laid on the frequent aspecificity of the symptomatology, the inconsistency of palpatory and auscultatory findings in the cervical region, and the value of certain medical techniques that may precede angiographic diagnosis. Considering the lack of reports in the literature, a case in which carotid stenosis was considered to be of luetic nature is noteworthy. Prophylactic treatment designed to prevent another cerebral attack hinges essentially on the correction of arterial hypertension and on the use of antiaggregating drugs. Endarterectomy is contraindicated by the presence of limiting factors.
