Congenital agenesis/aplasia of the internal carotid arteries: MRA and SPECT findings.

Single photon emission CT (SPECT) and MRI and MR angiography studies were performed in a patient with right internal carotid artery (ICA) agenesis, left ICA aplasia, and multiple associated vascular abnormalities identified with digital subtraction angiography. Magnetic resonance angiography provided an excellent depiction of the extra- and intracranial vascular abnormalities. No evidence of brain lesions or perfusion defects was demonstrated with MRI and SPECT, despite the markedly altered vascular anatomy.

A real-time classification system of thalassemic pathologies based on artificial neural networks.

Thalassemias are pathologies that derive from genetic defects of the globin genes. The most common defects among the population affect the genes that are involved in the synthesis of alpha and beta chains. The main aspects of these pathologies are well explained from a biochemical and genetic point of view. The diagnosis is fundamentally based on hematologic and genetic tests. A genetic analysis is particularly important to determine the carriers of alpha-thalassemia, whose identification by means of the hematologic parameters is more difficult in comparison with heterozygotes for alpha-thalassemia. This work investigates the use of artificial neural networks (ANNs) for the classification of thalassemic pathologies using the hematologic parameters resulting from hemochromocytometric analysis only. Different combinations of ANNs are reported, which allow thalassemia carriers to be discriminated from normals with 94% classification accuracy, 92% sensitivity, and 95% specificity. On the basis of these results, an automated system that allows real-time support for diagnoses is proposed. The automated system interfaces a hemochromo analyzer to a simple PC.

[Magnetic resonance in the diagnosis and follow-up of soft-tissue sarcomas].

The authors analyze the advantages of MR imaging in the detection, staging and follow-up of malignant soft-tissue sarcomas. The role of MR imaging is retrospectively evaluated in 34 patients (16 men and 18 women) ranging in age from 3 to 82 years (mean: 38); 19 of them had a primary neoplasm, while the extant 15 patients were examined after surgery, and residual or recurrent tumors were present. Ten examinations were performed on an 0.5 T imager (CGR Magniscan 5000), and 27 were obtained with a 1.5 T system (Magnetom 42 SP Siemens). In all cases T1 and T2 sequences were acquired; in 3 patients T1-weighted sequences were performed after Gd-DTPA administration. In the 19 patients with primary tumors pathologic specimens were obtained. The value of MR imaging in the evaluation and characterization of soft-tissue sarcomas and its capabilities in detecting neurovascular encasement and bone involvement are discussed. Post-Gd-DTPA scans may help in the differentiation of necrotic areas from active tumors. MR diagnostic criteria of malignancy, including tissue heterogeneity and irregular margins, were not fulfilled in 3 of the studied cases.

[Doppler measurement of tibial pressure in the diagnosis of peripheral arteriopathies].

Reference is made to the importance of determining tibial artery pressure in the diagnosis of peripheral arterial insufficiencies of the lower limbs. A series of 83 cases in which a Doppler ultrasonography was used in its evaluation at rest and during the Strandness exercise test at the same time as oscillography is presented. The Doppler examination proved far more sensitive in the detection of slight arterial insufficiencies, including those devoid of clinical symptoms in some cases. There was also a parallel between the Doppler findings and the metabolic picture in one group of patients, together with a certain correspondence with dyslipidaemia. Emphasis is placed on the importance of determining tibial artery pressure. This simple diagnostic examination can easily be performed, even by the general practitioner.
