The effect of autoclaving on the mechanical properties of bank bovine bone.
Xenologous bone grafts are frequently used in modern orthopaedic surgery. There is a risk of infection associated with the transmission of diseases among different species for these types of grafts. As for bovine bone grafts in recent times attention has been paid to the diffusion in various countries of the European Economic Community of "bovine spongiform encephalopathy"; this pertains to the same group of encephalopathies as Creutzfeldt-Jakob disease which strikes humans and has a lethal outcome. Thus, in the absence of clinical evidence, there is a potential source of risk of infection associated with specimens of bovine origin in relation to which a document of the European community suggests proceeding by sterilization by autoclaving at 132 degrees C. for at least 1 hour. It is the purpose of this study to verify the effect of this treatment on the mechanical properties of bone graft. The results show how this procedure reduces the graft's resistance to compression by approximately 70%. Thus, it is not acceptable for all bone grafts for which good mechanical resistance is required. In cases such as these, bovine bone grafts may be used only when the immunity of the farm of origin has been certified; then sterilization may be conducted using methods that are less destructive for the mechanical integrity of the bone tissue.

[Indications for hepatic resection: personal experience].
The Authors, before examining their case history, which includes 103 major and minor hepatic resections performed during the last decade, briefly show the surgical technique concerning hepatic ischaemia and the new technologies proposed to reduce the hematic loss during hepatic resections. The constant adoption of these techniques and the careful evaluation of some parameters concerning hepatic functionality allowed to decrease postoperative complications and mortality, which currently is around 5-10%. These data are confirmed by the Authors experience.

[Ileal duplication in adults. A very rare nosological entity].
The report presents a rare case of intestinal duplication in a 43-year old female. Intestinal duplication is a rare congenital malformation and is extremely exceptional in adults. A lot of etiopathogenic theories have been advanced to explain this malformation that can occur anywhere along the alimentary tract, even if the ileum remains the most common. It may be cystic or tubular. An important aspect of mucosal histology is the possibility of gastric heterotopy, conditioning a particular treatment. The literature shows 14 cases with clinical very different presentations and instrumental exams were rarely helpful for correct diagnosis. Treatment of choice is surgical complete resection of the duplication. When contiguous structures are involved intestinal bypass or Roux-on-Y anastomosis may be necessary with mandatory stripping of the mucosa when heterotopic gastric mucosa is present in order to prevent the risk of gastrointestinal haemorrhage or malignant transformation, an event possible in about 25% of the cases reported in the literature.
