Biochemical and anatomical effects of antibodies against nerve growth factor on developing rat sensory ganglia.

The importance of nerve growth factor (NGF) for the development of sensory ganglia was investigated by injecting rat fetuses (16.50 days of gestation) with a single dose of anti-NGF antiserum. Four months later the treated animals showed a very large decrease in substance P- and somatostatin-like immunoreactivities in dorsal root ganglia and skin with a lesser decrease in trigeminal ganglia. Fluoride-resistant acid phosphatase, substance P-, and somatostatin-like immunoreactivities were greatly decreased in the dorsal horn of the spinal cord. No change in neurotensin- and [Met]enkephalin-like immunoreactivities was observed. The anti-NGF antiserum treatment produced a greater than 90% decrease in the number of unmyelinated dorsal root fibers and a 35% decrease in the total number of myelinated fibers. The loss in myelinated fibers was restricted to small-diameter fibers with no change in large-diameter fibers. No change in taste bud morphology was noted, thereby refuting the proposal that anti-NGF antiserum treatment may represent an animal model for familial dysautonomia. The present results indicate that NGF is a necessary requirement for the normal development of a significant population of prenatal rat dorsal root ganglion cells.

The effects of non-prescribing of anxiolytics in general practice. II. Factors associated with outcome.

Factors associated with psychiatric outcome were examined in a series of 87 patients who had presented in general practice with new episodes of minor affective disorder. Two outcome measures were used: (i) Status on Present State Examination (PSE) seven months after initial consultation; (ii) prescribing of psychotropic medication between one-month and seven-month follow-up assessments. Outcome according to the PSE was significantly associated with: worse measures of psychiatric state (General Health Questionnaire, Profile of Mood states) and of social functioning (SAS-M) at initial consultation and one month later; and with persistent anxious or depressed mood during the follow-up period; but not with life events. During the follow-up period 20 patients received at least one psychotropic prescription; they were significantly associated with worse initial GHQ scores, consumption of tobacco and non-prescribed medication, and initial anxiety. A sub-group of 11 patients received multiple psychotropic prescriptions; they were significantly associated with the same initial measures, and also with poor outcome measures (psychiatric and social).

Comparative effects of a repeated dose regime of diazepam and buspirone on subjective ratings, psychological tests and the EEG.

Two doses of buspirone (5 and 10 mg tds), 1 dose of diazepam (5 mg tds) and placebo were administered to 8 normal subjects for a period of 8 days. Each subject received each drug in a balanced order with a minimum interval of 1 week between courses. Psychotropic effects were assessed with a battery of physiological, psychomotor and subjective tests on the first, third and last days of treatment both before the first daily dose and at 1 h and 3 h after it. Diazepam showed a characteristic profile of action producing EEG changes and psychological impairment after a single dose which were still present after a week's treatment. Such effects were minimal after buspirone. Both drugs increased subjective ratings of drowsiness but these feelings tended to decrease after a week's treatment on the clinical doses. Buspirone (10 mg tds) produced some unpleasant side-effects initially but tolerance to these invariably developed after 3 days treatment.

The influence of GABAergic drugs on PGO activity in the cat.

The effects of drugs modifying GABAergic neurotransmission have been examined on ponto-geniculo-occipital (PGO) waves induced in the encéphale isolé cat by reserpine (PGOres), Ro4-1284 (PGO1284) or PCPA (PGOPCPA). The GABA agonists muscimol and THIP both caused large increases in density of PGOPCPA. The PGO1284 and PGOres were less affected although, of these, a larger increase in PGO1284 density was recorded. None of the increases could be reversed by subsequent injection of bicuculline. Chlordiazepoxide brought about large increases in PGOPCPA density but was ineffective in altering PGOres or PGO1284. The GABA transaminase inhibitor gamma-acetylenic GABA increased the density of all PGO waves but was not effective in the case of PGOPCPA. These results confirm a role for GABA in modulating PGO activity. The pathways involved in this GABA modulation are discussed.

Correlations among measures of response to benzodiazepines in man.

A retrospective analysis of five studies using benzodiazepines and one using a benzodiazepine-like drug, zopiclone, and involving 61 subjects in toto was carried out. All used a placebo control and all acute studies incorporated pre-treatment values as well. Eight variables which have previously been shown to be sensitive to benzodiazepine action and which covered a range of physiological, psychological and subjective measures were chosen. Within-subject correlations between drug minus placebo values for these variables were computed to examine relationships between the measures while the subjects were taking benzodiazepines. It was found that closely related measures within system were significantly correlated but other correlations were sparse. However under limited conditions, i.e., several doses of one drug in the same subjects as in the zopiclone study, a consistent pattern of effects was shown across measures, resulting in half of the possible correlations showing statistical significance. A similar pattern was shown in patients after 2-4 weeks treatment with benzodiazepines.

Restricted antigenicity of thyroxyls in human thyroglobulin.

Interactions between two human iodothyronine-binding autoantisera and three preparations of human thyroglobulin (Tg) were not proportional to the latter's thyroxyl residue content. Probably only one of the several thyroxyl-containing sites in Tg reacted with the immunoglobulins from both antisera. In the case of one of the antisera, which was thyroxine (T4)-specific, the thyroxyl residue was the immunodominant feature of the antigenic site. The other antiserum, which had a specificity for 3,5,3'-tri-iodothyronine (T3), recognized different determinants around the same thyroxyl residue, but this residue was not itself an important element of the binding site. Thus, despite the specificity for T3 free in solution, the presence of T4 in the complete antigenic site was tolerated, since other structures supplied the bulk of the binding energy. 'Specificity' of this antiserum for T3 in solution is therefore coincidental and need not be ascribed to the presence of T3 in the original immunogen. Some results obtained in these studies may be interpreted as supporting the possibility that a modified Tg was the immunogen for the generation of these naturally occurring human antisera.

A comparison of the psychotropic profiles of tofisopam and diazepam.

Twelve normal subjects were tested on a number of measures both before and 1,3 and 5 h after 10 mg diazepam, 100 and 200 mg tofisopam and a placebo. The measures included self-ratings of mood, bodily symptoms, hostility and sleep, the electroencephalogram (EEG), reaction time, tapping, digit symbol substitution, the symbol copying test, and plasma levels. Diazepam showed a clear profile of action, producing EEG changes, pronounced sedation and psychological impairment. The last two effects were maximal at 1 h and had worn off by 5 h. The EEG was recorded at 3 h only. Tofisopam in no way resembled diazepam. It produced no changes on the EEG or psychological tests and a very mild stimulant effect was apparent on the ratings. While diazepam was easily detectable in the blood, tofisopam did not bind to benzodiazepine receptors.

The nature of the substance P-containing nerve fibres in taste papillae of the rat tongue.

The nature of the association of substance P (SP) with taste buds in the rat tongue was investigated by immunohistochemical and radioimmunoassay techniques. Both the circumvallate and fungiform papillae were found to receive a rich innervation by substance P-containing fibres. Although these fibres were closely associated with the taste buds in these structures, they assumed a perigemmal rather than an intragemmal location. Bilateral lesions of the glossopharyngeal nerve resulted in the depletion of taste buds from the vallate papilla and a large reduction in substance P immunoreactive fibres in this area. Lesions of the chorda tympani, which led to the degeneration of taste buds in fungiform papillae, had no effect on the immunohistochemical appearance of substance P in these papilla or on the substance P levels in the anterior part of the tongue. Lesions of the mandibular division of the trigeminal nerve or neonatal capsaicin treatment had no effect on the structural integrity of taste buds in fungiform papillae but led to the depletion of substance P-immunoreactive fibres from these papillae. Both of these procedures caused a 71% reduction in the substance P content of the anterior tongue, ipsilaterally after the nerve lesion and bilaterally after capsaicin treatment. The results are discussed in relation to the possible functional role of substance P-containing fibres within nerves supplying taste structures of the tongue.

Hysterectomy and psychiatric disorder: II. Demographic psychiatric and physical factors in relation to psychiatric outcome.

One hundred and fifty-six patients with menorrhagia of benign origin were interviewed before hysterectomy, and again six months (n = 147) and 18 months (n = 148) after the operation. Psychiatric outcome, measured in terms of PSE cases detected at 18 months, was strongly associated with pre-operative mental state, neuroticism, previous psychiatric history, and family psychiatric history. No association was found with organic pathology in the uterus, bilateral oophorectomy or demographic variables.

The effects of dietary fibre on glucose tolerance in healthy males.

Previous studies have shown that the addition of non-absorbable carbohydrate (NAC) to test meals decreases the glucose and and insulin response both in normal and diabetic subjects. However, these studies appear to have used brain in the basic test meal without a knowledge of the effect of bran itself or of the added NAC alone. In the present investigation bran alone, pectin alone, guar alone and pectin with guar have been studied. Guar alone added to the test meal significantly lowered blood glucose at 90 minutes. Pectin alone did not have a significant effect. Pectin and guar together resulted in a blood glucose lower at 30 minutes and greater at 120 minutes. No significant changes in insulin response were noted to our study. It appears that NAC alters the glucose response to a given meal, but the extent to which this occurred in the present study was less than in previous studies. The differences may reflect synergism between bran and other NAC's to lower blood glucose response after a meal.

Decahydroisoquinolines: novel competitive AMPA/kainate antagonists with neuroprotective effects in global cerebral ischaemia.

In the present studies, we have evaluated the activity of a series of glutamate receptor antagonists from the decahydroisoquinoline group of compounds both in vitro and in vivo. Compound activity at alpha-amino-3-hydroxy-5-methylisoxazole-4-propionic acid (AMPA) and kainate receptors was assessed using ligand binding to cloned iGluR2 and iGluR5 receptors and on responses evoked by AMPA and N-methyl-D-aspartate (NMDA) in the cortical wedge preparation. In vivo, compounds were examined for antagonist activity electrophysiologically in the rat spinal cord preparation and in the gerbil model of global cerebral ischaemia. Compounds tested were LY293558, which has been shown to protect in models of focal cerebral ischaemia, LY202157 (an NMDA antagonist), LY246492 (an NMDA and AMPA receptor antagonist), LY302679, LY292025, LY307190, LY280263, LY289178, LY289525, LY294486 (AMPA/kainate antagonists) and LY382884 (an iGluR5 selective antagonist). Results obtained support a role for AMPA receptors in cerebral ischemia. LY377770 (a mixed AMPA/iGluR5 antagonist and active isomer of LY294486) demonstrated good neuroprotection with a 2-h time window and may therefore be useful in the treatment of ischaemic conditions.

Frequent attenders without organic disease in a gastroenterology clinic. Patient characteristics and health care use.

We used a hospital computer to identify 50 patients (35 women, 15 men) satisfying research criteria for "frequent attenders" at a gastroenterology outpatient clinic (four or more visits to a general hospital clinic in the previous 12 months). Their mean duration of symptoms was 5 years, and 80% reported fatigue as a significant complaint. Thirteen (37%) of the women were also consulting a gynecologist, and in nine of these their status was normal. Seven (21%) of the 35 women who were interviewed had a history of childhood sexual abuse, and these patients reported significantly more lifetime somatic symptoms (9.7, SD = 3.8) than those without such a history (5.4, SD = 3.5, p = < 0.01). The 50 patients reported high levels of disability and psychological distress, and were more likely to rate the probability of their symptoms as being due to "bowel disease" than to "stress" or "other problems." Forty-five patients had at least one current psychiatric diagnosis and 24 at least two, with somatoform disorders being the most common. Nineteen (38%) reported infrequent panic attacks, but only three had somatization disorder. The mean number of lifetime somatic symptoms was 5.9 (SD = 3.6; range 1-14). Seventeen patients (35%) also satisfied criteria for frequent attending in primary care (> 12 visits over the previous 12 months), and the patients reported a mean number of 5.7 (SD = 2.1) specialist appointments in the previous year. There may be a case for using the hospital computer to identify frequent attenders proactively at an earlier stage of their hospital visits so that appropriate management can be instituted. If such patients can be identified in this way, their assessment and management might be more appropriately supervised in designated clinics by more experienced gastroenterology staff.

Condition specific antepartum testing: systemic lupus erythematosus and associated serologic abnormalities.

Antepartum fetal surveillance is well established in the optimal management of any pregnancy complicated by maternal systemic lupus erythematosus and/or its associated serologic abnormalities. Our experience in antepartum surveillance of the "lupus pregnancy" from 1980 to 1988 is reviewed. An overall perinatal mortality rate of 6.06% compares favorably to the total intrauterine death rate range usually reported for this disease.
