Function of the rabbit supraspinatus muscle after large detachment of its tendon: 6-week, 3-month, and 6-month observation.

Thirty rabbits were anesthetized, and the supraspinatus tendon was detached from the greater tuberosity and the infraspinatus and subscapularis tendons. After 6 weeks (group A), 3 months (group B), and 6 months (group C), in vivo evaluation of twitch tension and fatigue index was done. Next, scapulas with muscles were examined with computed tomography. The most prominent decrease of twitch tension and fatigue index was noted during the first 6 weeks. The twitch tension and fatigue index after 2 and 4 minutes of the muscle operated on expressed as a percentage of the values developed by healthy contralateral nonoperated muscle were 58.2%, 34.5%, and 32.7% at the beginning of stimulation, 45.4%, 35.3%, and 26.7% after 2 minutes, and 42.5%, 32.6%, and 18.5% after 4 minutes of stimulation, respectively, in groups A, B, and C. Computed tomography examination showed fatty degeneration of the supraspinatus averaging 1.85 (group A), 2.28 (group B), and 2.43 (group C). Moderate infraspinatus fatty degeneration was noted in all groups. There was no significant fatty degeneration of subscapularis muscle.

[Selected clinical and pathogenetic problems of Binswanger's subcortical arteriosclerotic encephalopathy--analysis of 19 cases].

In 19 patients (14 males, 5 females) the clinical diagnosis of subcortical, arteriosclerotic encephalopathy (s.a.e.) was established on the basis of CT scanning, which showed in subcortical white matter low-attenuation areas and, often, focal ischaemic changes. The age of patients ranged from 46 to 85 years, the duration of illness from 6 months to 12 years. Four patients were normotensive (in 1 of these cases the clinical diagnosis was confirmed neuropathologically). In 2 hypertensive patients the acute development of neurological symptoms was related to considerable fall of systemic RR. On the basis of the CT picture s.a.e. may be diagnosed early, before pronounced neurological and intellectual deficits develop.

Electronic pupillometry in healthy controls. Response to sympathicomimetics.

The ocular sympathetic function was examined in 79 healthy volunteers (37 males and 42 females) by electronic pupillography. Pupillary diameters were first estimated under standard light conditions prior to drug application, these diameters being termed "basal values". Then, the pupils were stimulated pharmacologically by instillation of various sympathicomimetic drugs: tyramine tests were carried out in 42 individuals (24 females; 18 males), hydroxyamphetamine tests in 38 (22 females and 16 males), and phenylephrine tests in 43 individuals (23 females and 20 males). Generally, only a minor variation in the frequency of asymmetries was observed between sexes as well as between the various age groups, both when calculating the increase in pupillary diameter in millimeters, in per cent, and as the anisocoria index. In agreement with previous investigations, pupil size was found to diminish with age. The use of an age-matched control material is mandatory when comparing pupillographic recordings in patients in various categories, in the ordinary condition, as well as after topical drug administration.

[Pathomechanisms of pain attacks in Horton's headache (cluster headache)].

In view of the still unexplained pathomechanism of Horton's cluster headaches 9 patients were given a challenge test with nitroglycerin while at the same time the attacks were blocked with indomethacin. The attacks were provoked with one dose (1 mg) of nitroglycerin. Then during 3 days the patients took 75-100 mg indomethacin which dose was quite sufficient for inhibition of the activity of arachidonic acid cyclooxygenase. On the fourth day another nitroglycerine provocation was done. No inhibitory effect of indomethacin on the attack was observed. It seems doubtful that products of arachidonic acid cyclooxygenation play any role in the pathomechanism of this headache.

Migraine in aspirin-sensitive asthmatics.

The aim of this study was to evaluate the occurrence of migraine in aspirin-sensitive asthmatics, and was performed in 46 aspirin-sensitive asthmatic patients (ASA). The control group consisted of 46 asthmatics without aspirin sensitivity. In all patients allergological and laryngological examinations were performed and histories concerning the occurrence of migraine were collected. Twenty one (45.7%) of aspirin-sensitive asthmatic patients suffered from migraine, 5 from classical migraine and 1 from dietary migraine. In the control group migraine was found in only 6 (13%) persons. A family history of migraine was positive in 11 (52.4%) asthmatics with aspirin-sensitivity and migraine and in only 1 (17%) patient with migraine but without aspirin-sensitivity. The authors conclude that the high incidence of migraine in aspirin-sensitive asthmatics may be related to a defect in their arachidonic acid metabolism.

[Newly described form of idiopathic headache--so-called chronic paroxysmal hemicrania (Sjaastad syndrome)].

The authors discuss the new form of headache described in 1974 by Sjaastad differing from other headaches known as yet occurring in attacks: chronic paroxymal hemicrania. Attacks resembling cluster headaches have a shorter duration (10-20 minutes) and occur more frequently (up to 30 times daily). An important feature is complete disappearance of headache after indomethacine. Two own cases are reported and the pathomechanism of attacks is discussed.

[Bilateral occurrence of symptoms of Horton's headache (cluster headache)].

The authors discuss the problem of bilateral occurrence of symptoms in cluster headache. Literature survey shows that bilateral headache were observed in 10-13% of cases by various authors. In own material of 63 cases the authors found bilateral symptoms in 9.5% of cases and describe in more detail two cases. An own classification of bilateral cluster headaches in suggested.
